Iatrogenic Chest Wall Arteriovenous Malformation Following Chest Tube Placement: A Case Report.

Arteriovenous malformations (AVMs) are vascular anomalies characterized by direct arterial-to-venous connections that bypass the capillary system. Chest‑wall AVMs are rare, with only few reports of iatrogenic AVMs following chest‑tube placement. We describe a large, symptomatic chest‑wall AVM developing at the site of prior chest‑tube insertion, highlighting diagnostic, operative, and follow‑up strategies. We present a case of a 37-year-old male patient with a large, symptomatic chest wall AVM centered around a scar from a prior chest tube placement. A thorough preoperative assessment was cond

Are Routine Chest Radiographs After Chest Tube Removal in Thoracic Surgery Patients Necessary?

INTRODUCTION: The routine use of chest x-ray (CXR) to evaluate the pleural space after chest tube removal is a common practice driven primarily by surgeon preference and institutional protocol. The results of these postpull CXRs frequently lead to additional interventions that serve only to increase health care costs and resource utilization. We investigated the utility of these postpull CXRs in thoracic surgery patients and assessed their effectiveness in predicting the need for tube replacement. METHODS: Single-institution retrospective study comprising thoracic surgery patients requiring p

Are radiology residents safe to report feeding nasogastric (NG) tubes on chest X-rays?

OBJECTIVES: The task of issuing reports on whether nasogastric (NG) tubes are safe for enteral nutrition on chest X-ray (CXR) often falls to radiology residents. The aims of this study are to evaluate whether radiology residents are formally trained and their performance in interpreting NG tube position on CXR. METHODS: Radiology residents were invited to participate in an online study evaluating NG tube position on CXR. The CXR images comprised 20 NG tubes, 14 of which were correctly sited, while 4 were in the distal oesophagus and 2 in the lung. RESULTS: Twenty-eight (of 185, 15%) radiolog

The needle-wire-dilator technique for the insertion of chest tubes in pediatric patients.

We evaluated the needle-wire-dilator (NWD) technique, using commercially available sets, for insertion of chest tubes in 24 pediatric patients who were admitted to our pediatric intensive care unit (PICU). Fourteen patients had pneumothoraces, three had hemothoraces, two had pneumonia with empyema, four had pleural transudate effusions, and one had chylothorax. The ages of the patients ranged from four months to 24 years, and the sizes of the inserted chest tubes ranged from 10F to 20F. All insertions were successful, and the time from invasion of the pleural space by the needle to completion

Does pre-injury anticoagulation make chest tubes any less safe? A nationwide retrospective analysis.

BACKGROUND: Oral anticoagulation is becoming more common with the aging population, which raises concern for the risk of invasive procedures that can cause bleeding, such as chest tube placement (thoracostomy). With the increase in CT imaging, more pneumothoraces and hemothoraces are being identified. The relative risk of thoracostomy in the presence of anticoagulation is not well-established. The objective of this study was to determine whether pre-injury anticoagulation affects the relative risk of tube thoracostomy following significant chest trauma. METHODS: This retrospective cohort stud

Reduced survival in patients requiring chest tubes with COVID-19 acute respiratory distress syndrome.

BACKGROUND: Numerous complications requiring tube thoracostomy have been reported among critically ill patients with COVID-19; however, there has been a lack of evidence regarding outcomes following chest tube placement. METHODS: We developed a retrospective observational cohort of all patients admitted to an intensive care unit (ICU) with confirmed COVID-19 to describe the incidence of tube thoracostomy and factors associated with mortality following chest tube placement. RESULTS: In total, 1705 patients with laboratory confirmed COVID-19 patients were admitted to our ICUs from March 7, 202

Enhanced recovery after surgery and chest tube management.

This review documents the relationships between enhanced recovery after surgery (ERAS) pathways, chest tube management and patient outcomes following lung resection surgery. ERAS pathways have been introduced to mitigate the harmful stress response that occurs following all major surgery, including lung resection. Improvements to the entire patient pathway, from the preoperative admission clinic through to discharge and beyond, can have additive or synergistic effects and result in improved patient outcomes, reduced length of stay and lower costs. At the same time, there are some key care elem

A chest tube may not necessary in children thoracoscopic lobectomy.

Drainage of the thorax postoperatively using chest tubes is a standard procedure in thoracic surgery. However, chest tubes can induce pain and immobilization, increase risk of infection, deteriorate the ventilation capacity, and increase difficulty of postoperative management, particularly in children. This study aimed to investigate the safety and effect of excluding chest tubes after performing thoracoscopic lobectomy in selected children.A retrospective review of medical records was performed in West China Hospital of Sichuan University from January 2014 to June 2018. Patients who underwent

Thoracic Trauma: Which Chest Tube When and Where?

Clinical suspicion of hemo/pneumothorax: when in doubt, drain the chest. Stable chest trauma with hemo/pneumothorax: drain and wait. Unstable patient with dislocated trachea must be approached with drain in hand and scalpel ready. Massive hemo/pneumothorax may be controlled by drainage alone. The surgeon should not hesitate to open the chest if too much blood drains over a short period. The chest drainage procedure does not end with the last stitch; the second half of the match is still ahead. The drained patient is in need of physiotherapy and proper pain relief with an extended pleural space

A new use of Fogarty catheter: chest tube clearance.

Chest tubes are commonly used for patients who have undergone a cardiothoracic procedure to avoid the complications related to the accumulation of blood and serous fluid in the chest. Although the traditional methods such as milking, stripping or active chest tube clearance devices are used to establish patency of the chest tubes, they can become clogged at any time after their placement. Our technique may re-establish the tube patency with utilising Fogarty catheter and without any detriment to tissues.

Migration of the chest tube into the esophagus in a case of Boerhaave's syndrome.

Boerhaave's syndrome is the spontaneous transmural rupture of the esophagus. Patients can have a variety of manifestations. Boerhaave's syndrome has to be considered in acutely ill patients with no other explanations for their illness. Computed tomography scan of the chest is emerging as a useful tool for the evaluation of these patients. Surgical repair is the standard of care. Adequate drainage of the pleural fluid is necessary to prevent pulmonary complications. Esophageal perforation should be considered whenever thoracostomy tube drainage assumes an enteric character. Curved chest tube for drainage of malignant pleural effusion.

Thoracentesis with a chest tube insertion and drainage of large pleural effusion is widely performed in patients with malignant lung diseases. One potential problem with a conventional chest tube placement is occasional incomplete evacuation of effusion owing to inappropriate position of the tip where the drainage holes opened. We have developed a curved chest tube and evaluated the position of tip placement just after the placement and before removal on plain chest X-ray in 20 patients with massive pleural effusions due to lung cancer. In 15 of the 20 patients, the tip of the tube was success

Horner syndrome occurring as a complication of chest tube placement.

A case of Horner syndrome occurring secondary to the high insertion of a chest tube is reported. Horner syndrome from this cause can easily be avoided. The tip of the chest tube should be kept at or below the third posterior rib unless the clinical situation dictates otherwise.

Unusual position of a chest tube with elevation of the contralateral hemidiaphragm.

We present a patient with emphysema and a right-sided pneumothorax in whom a chest tube was inadvertently positioned in the lower portion of the right major fissure. The tube migrated across the midsaggital plane, resulting in left shoulder pain with elevation of the left hemidiaphragm. Malfunctioning of the tube did not result, and following its removal the diaphragm returned to its original position.

Iatrogenic Horner's syndrome secondary to chest tube thoracostomy.

We report a 22-year-old male who underwent a left thoracostomy and left lower lobectomy for a shotgun wound of the left anterior chest. During surgery, two chest tubes were placed in the left hemithorax at the level of the first and second intervertebral spaces. On the 10th postoperative day, the patient developed a left-sided miosis and ptosis. The diagnosis of a left preganglionic Horner's syndrome was confirmed by pharmacologic testing. The Horner's syndrome was unchanged 3 months after discharge. We conclude that the chest tubes damaged the preganglionic sympathetic fibers of the left orbi

Complications during chest tube drainage for iatrogenic pneumothorax.

BACKGROUND: Iatrogenic pneumothorax is a common complication of diagnostic and therapeutic pulmonary procedures. New guidelines on primary spontaneous pneumothorax suggest ambulatory approaches may be suitable. However, guidance on iatrogenic pneumothorax occurring in patients with impaired lung function, increased age, comorbidity and frailty is lacking, and the safety profile of ambulatory management is not known. The objective was to study the safety of iatrogenic pneumothorax treated with chest tubes and to identify the risks of life-threatening events. METHODS: In a retrospective cohort

Rapid disappearance of pneumorrhachis after chest tube placement.

INTRODUCTION: We present a rare case of traumatic pneumorrhachis with the combination of hemothorax which resolved rapidly after insertion of a chest tube. CASE PRESENTATION: A 55 year old male was admitted to our emergency department after falling from a ladder. His general condition was well, GCS was 15 with no motor deficits. On his spinal CT a fracture on multiple ribs leading to right sided hemothorax was observed with air in the T6-T8 spinal canal. A chest tube was placed and as he did not have any neurological deficits surgical intervention to the pneumorrhachis was not considered. On

Pleural controversy: optimal chest tube size for drainage.

In recent years, a higher and higher percentage of patients with pleural effusions or pneumothorax are being treated with small-bore (10-14 F) chest tubes rather than large-bore (>20 F). However, there are very few randomized controlled studies comparing the efficacy and complication rates with the small- and large-bore catheters. Moreover, the randomized trials that are available have flaws in their design. The advantages of the small-bore catheters are that they are easier to insert and there is less pain with their insertion while they are in place. The placement of the small-bore catheters

Cardiac tamponade in a preterm neonate secondary to a chest tube.

In a preterm infant, chest tubes were inserted for treatment of bilateral pneumothoraces. Hemorrhagic pericardial effusion with cardiac tamponade developed, probably resulting from traumatic injury by the left chest tube. The infant survived due to timely diagnostic and therapeutic intervention. No recurrence of pericardial effusion was seen and follow-up showed normal psychomotor development.
