Integration of sexually transmitted infection and HIV pre-exposure prophylaxis services in sub-Saharan Africa: a scoping review.

BACKGROUND: Persons living in sub-Saharan Africa (SSA) face disproportionate risk from overlapping epidemics of HIV and bacterial sexually transmitted infections (STIs). Pre-exposure prophylaxis (PrEP) for prevention is gradually being scaled up globally including in several settings in SSA, which represents a key opportunity to integrate STI services with HIV pre-exposure prophylaxis (PrEP). However, there is limited literature on how to successfully integrate these services, particularly in the SSA context. Prior studies and reviews on STI and PrEP services have largely focused on high incom

Cultural Authority and Food Choices: How Intergenerational Power Shapes Maternal Diet in Rural South Africa.

BACKGROUND: Maternal malnutrition remains a persistent public health challenge in South Africa, affecting both maternal and child health outcomes. Dietary practices during pregnancy and lactation are shaped by complex social dynamics that extend beyond individual choice. OBJECTIVES: This study explored maternal dietary practices during pregnancy, defined as the foods women reported consuming, avoiding, substituting, or concealing, and examined how cultural norms and intergenerational power relations influenced these practices within the context of routine antenatal nutrition counseling. Clinical guidance for diagnosis and management of suspected Pediatric Acute-onset Neuropsychiatric Syndrome in the Nordic countries.

Pediatric acute-onset neuropsychiatric syndrome is a clinical concept used to describe a subgroup of children with sudden onset of psychiatric and somatic symptoms. The diagnostic term and especially management of children differs depending on the clinical setting to which they present, and the diagnosis and management is controversial. The aim of this paper is to propose a clinical guidance including homogenous diagnostic work-up and management of paediatric acute onset neuropsychiatric syndrome within the Nordic countries. The guidance is authored by a Nordic-UK working group consisting of p

Implementation of components of a family navigation program for children with autism in a pediatric clinic: a best practice implementation project.

INTRODUCTION: Early diagnosis and intervention assists children with autism to reach their full potential. Children in rural areas experience extended wait times for early intervention, and parents are often concerned about meeting their child's needs with minimal support during the waiting period. Family navigation programs can help fill this gap, providing tools and emotional support to parents while waiting for early intervention to begin. OBJECTIVE: This project aimed to implement components of a family navigation program for children with autism through the promotion of evidence-based pr

Clinical care of the exposed infants of HIV-infected mothers.

Infants born to HIV-infected mothers are at risk for mother-to-child transmission of HIV. Since the beginning of the epidemic, medical advances have dramatically reduced transmission rates from the mother to the child. Clinical care of the HIV-exposed infant involves unique management considerations. Clinicians caring for these infants must be knowledgeable about postexposure antiretroviral prophylaxis, understand the HIV diagnostic testing necessary to determine the infant's HIV status, and be able to provide relevant anticipatory guidance. This article presents the pertinent management consi

The oppositional personality in childhood.

The charts of thirty children having been diagnosed at Washington University Child Guidance Clinic as having oppositional personalities are reviewed and their commonalities are noted. The literature on this syndrome is reviewed and the dynamics discussed. Delineation of this syndrome from other syndromes is considered. The issue of control for autonomy seems to be central in these children who resist authority, but have superego development.

Anesthetic management of a child with Apert syndrome.

In this paper, the authors describe an anesthetic technique for a child with Apert syndrome, presenting to the operating room for a syndactyly separation. The anesthetic approach is innovative for the clinic and is a combination of intravenous anesthesia and two regional techniques (axillary block and transversus abdominis plane block, respectively). They were performed under ultrasound guidance and provided analgesia in the two body regions, which were to be operated.

A longitudinal review of national HIV policy and progress made in health facility implementation in Eastern Zimbabwe.

BACKGROUND: In recent years, WHO has made major changes to its guidance on the provision of HIV care and treatment services. We conducted a longitudinal study from 2013 to 2015 to establish how these changes have been translated into national policy in Zimbabwe and to measure progress in implementation within local health facilities. METHODS: National HIV programme policy guidelines published between 2003 and 2013 (n = 9) and 2014 and 2015 (n = 5) were reviewed to assess adoption of WHO recommendations on HIV testing services, prevention of mother-to-child transmission (PMTCT) of HIV, and pro

Exploring Associations Between Abnormal Weight Classifications and Child Maltreatment Diagnoses.

Child maltreatment poses not only immediate danger, but as a type of toxic stress, it creates higher risk of biologic dysfunction later in life. Pediatricians are in a unique position to diagnose child maltreatment, but they need evidence-based guidance for when to initiate screening when injury is occult. In this retrospective cohort study of 855 pediatric patients diagnosed with child maltreatment, researchers explored whether type or number of diagnoses was associated with abnormal pediatric weight in either direction. Diagnoses and weight assessed at intake were extracted from medical reco

Safely Supporting the Establishment of Breastfeeding in the Setting of Fentanyl Use Before the Birth Hospitalization.

The increasing prevalence of nonprescribed fentanyl use during pregnancy presents a clinical dilemma for lactation counseling, because pharmacokinetic data on fentanyl transfer into breast milk remain limited. For families who choose it, human milk for newborns and infants is the gold standard; clinicians' deviation from this recommendation requires a clear rationale and supporting evidence. The 2023 Academy of Breastfeeding Medicine's clinical protocol, "Breastfeeding in the Setting of Substance Use and Substance Use Disorder" summarizes available evidence regarding substance use and breastfe

Semilobar holoprosencephaly associated with maternal diabetes in a toddler: A case report.

Holoprosencephaly is a rare congenital malformation of the forebrain caused by incomplete division of the prosencephalon during early embryonic development, and semilobar holoprosencephaly represents the intermediate form of the spectrum. We report the case of a 3-year-old child presenting with developmental delay, seizures, feeding difficulties, and midline facial abnormalities. The child was born to a mother with poorly controlled maternal diabetes mellitus. The MRI scan of the brain showed partial fusion of the frontal lobes, incomplete separation of the hemispheres of the cerebrum, presenc

[Expert consensus on the detection of genome-wide copy number variations in abortive tissues and family reproductive consultation].

Chromosomal aberrations including numerical abnormalities and segment duplications/deletions, as genome-wide copy number variations (CNVs), are a leading cause for spontaneous abortion. Analysis of abortive tissues for such CNVs can detect potential genomic variations in the couple and provide guidance for the choice of appropriate method to avoid further miscarriage or birth of child with chromosomal disorders. With evidence-based clinical data, an expert group jointly formed by the Genetic Disease Prevention and Control Group, Committee for Birth Defects Prevention and Control, Chinese Assoc

Parental influences on contact lens uptake among school-aged individuals with myopia in Kenya.

SIGNIFICANCE: With the global rise in myopia among school-aged children, effective management strategies, such as contact lenses (CLs), are essential. In Kenya, where myopia prevalence is increasing, understanding parental influence on CL uptake is critical because of parents' key role in healthcare decisions involving their children. This study addresses this significant gap by exploring the parental factors that either promote or hinder CL uptake among school-aged children in Kenya. PURPOSE: This study investigated the parental factors influencing CL uptake among school-aged children with m

Consensus Statement of the IAP - Neurodevelopmental Chapter On Neurodevelopmental Disorders Habilitation Process: Strategic Plan for Prevention, Early Detection and Early Intervention.

JUSTIFICATION: Neurodevelopmental disorders, as per DSM-V, are described as a group of conditions with onset in the development period of childhood. There is a need to distinguish the process of habilitation and rehabilitation, especially in a developing country like India, and define the roles of all stakeholders to reduce the burden of neurodevelopmental disorders. PROCESS: Subject experts and members of Indian Academy of Pediatrics (IAP) Chapter of Neurodevelopmental Pediatrics, who reviewed the literature on the topic, developed key questions and prepared the first draft on guidelines. Dyspnea in Children with Serious Illness.

Dyspnea is a common and distressing symptom in children living with serious illnesses, particularly near the end of life, and requires thoughtful, family-centered assessment and management. A stepwise nursing approach to pediatric dyspnea is explored, which includes clinical assessment, identification of potentially reversible causes, and shared decision-making grounded in the child's symptoms, goals of care, and family context. Using a case study of a child with metastatic osteosarcoma, practical strategies are highlighted for symptom management, including nonpharmacologic interventions, comm

[An asymmetrical face due to torticollis in two young children].

In 2 girls, 4 years and 8 months of age, respectively, torticollis had been diagnosed a few weeks after birth. Kinesiotherapy, given for a period of 1 year and 6 months, respectively, had little effect. Both girls were then referred to the Orthopaedic Outpatient Clinic where a shortened sternocleidomastoid muscle and an asymmetrical face were diagnosed. The shortened muscle was cleaved surgically. Over half a year later, the face of the older child was still asymmetrical but that of the younger child symmetric. It is recommended that children with torticollis due to a shortened sternocleidomas

MindTS-Net: A multimodal mindfulness-based intervention framework for children with Tourette syndrome.

Tourette syndrome affects children's daily functioning and creates a need for supervised support that responds to tic behaviors, emotional states, and interaction contexts. We developed MindTS-MMD, a Chinese multimodal dataset capturing emotion, tic-related behaviors, interaction context, and Valence-Arousal ratings, and MindTS-Net, a supervised digital adjunct linking multimodal emotion perception, mindfulness strategy planning, child-appropriate guidance generation, and dialogue-state tracking. Evaluation included dataset benchmarking, a 14-day primary clinical study with 60 children, and an
