Surgery for cholesteatoma should be tailored to individual patients, considering demographic and disease factors, to obtain a dry, safe, and functional ear. The EAONO/JOS classification and staging system provide a valuable framework for data collection and outcome assessment. Canal wall-up and canal wall-down surgical approaches each have their advantages and disadvantages, though it is not definitive that one approach is clearly more advantageous than the other. Mastoid obliteration techniques show promise in reducing recidivistic disease rates but require further research and standardizatio

PURPOSE OF REVIEW: The management of paediatric patients with cholesteatoma is complex, as the disease process is more aggressive in children than adults. New technologies and practice adaptations currently help optimize the surgical management and monitoring of these challenging patients.
RECENT FINDINGS: Several options of surgical management are available and are associated with equal if not improved outcomes. Technologic advancements in endoscopic ear surgery, the discovery of chemical-assisted dissection of cholesteatoma and advanced imaging protocols have enabled surgeons to better care

Introduction. Intracranial extension of cholesteatoma is rare. This may occur de novo or recur some time later either contiguous with or separate to the site of the original cholesteatoma. Presentation of Case. A 63-year-old female presented to a tertiary referral hospital with a fluctuating level of consciousness, fever, headache, and right-sided otorrhoea, progressing over several days. Her past medical history included surgery for right ear cholesteatoma and drainage of intracranial abscess 23 years priorly. There had been no relevant symptoms in the interim until 6 weeks prior to this pres

BACKGROUND: Mounting evidence has shown that long noncoding RNAs (lncRNAs) can play a substantial role in gallbladder cancer (GBC) development as tumor promotors or suppressors, and their abnormal expression is relevant to GBC patient outcomes. We completed this systematic review and meta-analysis to explore the clinical significance and mechanisms of lncRNAs in GBC.
METHODS: We conducted a comprehensive literature search and selected eligible records according to the inclusion and exclusion criteria. Hazard ratios (HRs) and odds ratios (ORs) were extracted or calculated to estimate the relat

Labyrinthine fistula is a potentially serious complication of cholesteatoma. During or after surgery, cholesteatoma can induce partial or total destruction of cochleovestibular functions. We operated 38 patients from 1983 to 1996. The site of the fistula was the external semi-circular canal in 90% of the cases. The facial nerve canal was eroded in 66% of the cases. We performed 11 CT scans; only 7 evidenced the fistula. We removed the matrix of the cholesteatoma during the initial surgery in 35 cases and in 3 left the fistula in situ for subsequent excision at a second operation. Postoperative

Ventilation tubes (VTs) have been extensively used to treat chronic serous otitis media over the last 30 years. However, it is not known if this practice has affected the incidence of cholesteatoma. A retrospective study was carried out at two Regional ENT Centres in Northern Ireland, examining the clinical notes of all patients undergoing major ear surgery between 1965 and 1988. The incidence of cholesteatoma per operatively in this period varied from 0.01 to 0.16 per 1,000 of population per year. The total number of cholesteatomas in the first five years of the study was exactly equal to the

43 cases of large labyrinthic fistulas caused by a cholesteatoma of the middle ear were studied (33 External Semi-circular Canal fistulas, 4 External Semi-circular Canal amputations with opening of the vestibule, 3 of the promontory, 2 of the oval window, 1 of the Semi-circular Canal). The audiometric, preoperative, radiological data was analyzed. The radiological exam is often disappointing for the average-sized fistulas. Preoperative labyrinthization is moderate for External Semi-circular Canal fistulas and even for certain vestibular amputations; it is often incomplete in the other location

1. In Shambaugh's primary acquired cholesteatoma, the surgical approach of choice is the direct endaural transcanal modified radical mastoidectomy and tympanoplasty in continuity. 2. In Shambaugh's classification of the secondary acquired cholesteatoma developing in a previously pneumatic mastoid with the infection of short duration, the postauricular transcortical mastoidectomy and facial recess approach and tympanoplasty in continuity is worthy of consideration if there is a reasonable possibility that the eustachian tube function may return to normal. 3. In Shambaugh's classification of a s

We recently reviewed the records of 1024 operations for aural cholesteatoma, of which 181 were in children. Our surgical management was the same as for adults. Complications of the disease (labyrinthine fistula, facial paralysis, total sensorineural impairment, and meningeal complications) were less common in patients under 16 years of age. This appears to be related to duration of disease rather than to age. The disease in children tends to be more difficult to eradicate. Staging the operation is required more frequently and there is a higher incidence of residual disease at planned second-st

A case of cholesteatoma, clearly delineated by preoperative air contrast retrograde pyelography, was treated like stone disease by removal of the obstructing material from the renal pelvis and pelvic ureter. A 7-year followup suggests that conservative surgical management may be the preferred treatment, since there is no evidence to date that cholesteatoma is a pre-malignant lesion nor necessarily a recurrent condition.

Acute coalescent mastoiditis is an uncommon sequela of acute otitis media. It occurs principally in the well-pneumatized temporal bone. The findings of fever, pain, postauricular swelling, and otorrhea are classic. Cholesteatoma, on the other hand, being associated with chronic infection, usually occurs in the sclerotic temporal bone. The signs and symptoms are isidious in nature and consist of chronic discharge and hearing loss which result from its mass, bone erosion, and secondary infection. Of 17 consecutive cases of acute mastoiditis over a six-year period, four were atypical because they

Intracranial cholesteatoma is a rare entity most commonly present in the middle ear region. Due to this, a consensus on serial imaging and when to intervene is not well established. Here, we present a rare case of an HIV-positive patient without any prior history of trauma or infection who developed a T1 and T2 hyperintense lesion of the sphenoid wing, confirmed as cholesteatoma, that nearly doubled in size over six years, causing mass effect on the left orbit and ultimately requiring resection. To the best of our knowledge, this is the first reported case of sphenoid wing cholesteatoma withou

Middle ear cholesteatoma has been extensively studied. Theories of cholesteatoma pathogenesis involving previous tympanic membrane retraction are the most widely accepted, but the contralateral ear in patients with cholesteatoma remains unstudied. This study aimed to investigate the contralateral ear in patients with cholesteatoma, and to determine whether the characteristics of it differ according to patient age and cholesteatoma growth patterns. This study was cross sectional. We evaluated 356 patients with middle ear cholesteatoma in at least one ear, and no history of surgery, between Augu

Cholesteatoma of the maxillary sinus is a rare condition, but it should be considered in the differential diagnosis of any slowly expanding lesion of the maxillary sinus. Because the keratinizing squamous epithelium continues to desquamate and expand, thereby causing erosion of the surrounding structures, the cholesteatoma sac must be removed completely. We describe a case of cholesteatoma of the maxillary sinus in an 18-year-old woman.
