Interdisciplinary behavioral health provider perceptions of implementing the Collaborative Chronic Care Model: an i-PARIHS-guided qualitative study.

BACKGROUND: The evidence-based Collaborative Chronic Care Model (CCM), developed to help structure care for chronic health conditions, comprises six elements: work role redesign, patient self-management support, provider decision support, clinical information systems, linkages to community resources, and organizational/leadership support. As the CCM is increasingly implemented in real-world settings, there is heightened interest in understanding specific influences upon implementation. Therefore, guided by the Integrated Promoting Action on Research Implementation in Health Services (i-PARIHS)

Application of Large Language Models in Chronic Disease Care: Mixed Methods Systematic Review and Thematic Synthesis.

BACKGROUND: Chronic diseases account for nearly three-quarters of global deaths and demand continuous, personalized long-term management; yet traditional care models often fall short in delivering such sustained support. Large language models, with advanced conversational and analytical capabilities, present promising opportunities to address the problem by offering scalable, interactive support. However, a comprehensive synthesis of evidence across diverse study designs, which moves beyond isolated technical metrics to evaluate large language models through a structured, theory-driven lens, r

Physicians' mental models regarding appropriate goals of care in chronic critical illness: a qualitative interview study.

BACKGROUND: There is a need to improve shared decision making about goals of care (GOC) for patients with chronic critical illness (CCI). Understanding the mental models that inform how physicians approach GOC decisions in CCI is necessary to improve shared decision making. RESEARCH QUESTIONS: What factors prompt physicians to revisit existing GOC decisions for patients with CCI and influence their judgments of appropriate GOC in CCI? How do external health system factors, including access to long-term ventilator facilities, influence physicians' mental models about GOC in CCI? STUDY DESIGN:

Development of a Model to Predict the Risk of Chronic Opioid Use After Entry Into Long-Term Care Facilities.

OBJECTIVES: Chronic opioid use is common in residents of long-term care facilities (LTCFs) and is associated with an increased risk of falls, fractures, and cognitive impairment. However, predictors of chronic opioid use in this cohort remain unclear. This study developed a prediction model for new chronic opioid use within 1 year of entry into LTCFs and identifies key predictors. DESIGN: This study was a retrospective cohort analysis using data from the Registry of Senior Australians National Historical Cohort. SETTING AND PARTICIPANTS: The cohort included non-Indigenous individuals 65 year

Modelling jurisdictional disparities in the cascade of care for chronic hepatitis B in Australia: impact of treatment uptake on mortality.

OBJECTIVE: Investigate the cascade of care for chronic hepatitis B (CHB) and estimate impacts of increasing treatment uptake on attributable burden, according to jurisdiction. METHODS: A mathematical model of CHB in Australia was utilised, combined with notifiable disease and Medicare data. We estimated the proportion with CHB who were diagnosed, engaged in care and receiving treatment in each state/territory, and projected future mortality. RESULTS: The highest uptake of all measures was in New South Wales, however, the largest increase over time occurred in Northern Territory. No jurisdict

Managing dry eye disease in primary Sjögren's syndrome using the chronic care model: a single-group pre-post study.

BACKGROUND: Dry eye is the most prevalent ocular complaint among patients with primary Sjögren's syndrome, manifesting as pronounced eye discomfort and frequently complicated by mental disorders including anxiety and depression. Prior research has demonstrated that the chronic care model improves both clinical indicators and psychological status. This study compared changes in clinical parameters and psychological status in patients with primary Sjögren's syndrome following a 24-week chronic disease management intervention. METHODS: This single-group pre-post study was conducted at a tertiary

Implementation of a multidisciplinary, team-based model to treat chronic hepatitis C in the primary care setting: Lessons learned.

Hepatitis C virus (HCV) is the most common blood-borne virus in the U.S., and its incidence continues to rise. With approval of direct-acting antiviral medications, treatment for Chronic Hepatitis C (CHC) has become highly efficacious with a minimal side effect profile. Primary care physicians are well-positioned to address this increased demand, yet most do not feel comfortable treating CHC. In this case report, we describe implementation of a multidisciplinary team-based approach for treating CHC at multiple primary care sites across a large safety net health system. We focus on the evolving

Implementation of chronic illness care in German primary care practices--how do multimorbid older patients view routine care? A cross-sectional study using multilevel hierarchical modeling.

BACKGROUND: In primary care, patients with multiple chronic conditions are the rule rather than the exception. The Chronic Care Model (CCM) is an evidence-based framework for improving chronic illness care, but little is known about the extent to which it has been implemented in routine primary care. The aim of this study was to describe how multimorbid older patients assess the routine chronic care they receive in primary care practices in Germany, and to explore the extent to which factors at both the practice and patient level determine their views. METHODS: This cross-sectional study used

Stepped Care for Patients to Optimize Whole Recovery (SC-POWR): An Effectiveness Trial Evaluating a Stepped Care Model for Individuals With Opioid Use Disorder and Chronic Pain.

Many patients who receive treatment for opioid use disorder (OUD) report experiencing chronic pain (CP), which is associated with high levels of ongoing nonmedical opioid use and low retention in OUD treatment. In pilot studies of patients with OUD receiving buprenorphine or methadone who had CP, cognitive behavioral therapy (CBT) attenuated nonmedical opioid use compared with treatment-as-usual (TAU), but patients in both treatment arms exhibited similar pain improvements. Adding exercise and stress reduction to this model may augment pain-related outcomes. With funding from National Institut

Health Care Models for Persons with Multiple Chronic Conditions from Populations that Experience Health Disparities: A Scoping Review.

Almost half of U.S. adults have multiple chronic conditions (MCC), and the prevalence of MCC has significantly increased for racial and/or ethnic minority groups, especially among those aged 45-64 years. Yet, little is known about evidence-based health care models for managing MCC in these populations. The overall objective of this scoping review was to identify the breadth of literature testing health care delivery models or components of models to improve the management of MCC for populations that experience health disparities. The databases of CINAHL Plus, Embase, PubMed, and Scopus were se

Integrated palliative care in Europe: a qualitative systematic literature review of empirically-tested models in cancer and chronic disease.

BackgroundIntegrated Palliative Care (PC) strategies are often implemented following models, namely standardized designs that provide frameworks for the organization of care for people with a progressive life-threatening illness and/or for their (in)formal caregivers. The aim of this qualitative systematic review is to identify empirically-evaluated models of PC in cancer and chronic disease in Europe. Further, develop a generic framework that will consist of the basis for the design of future models for integrated PC in Europe.MethodsCochrane, PubMed, EMBASE, CINAHL, AMED, BNI, Web of Science

The Collaborative Chronic Care Model for Mental Health Conditions: From Evidence Synthesis to Policy Impact to Scale-up and Spread.

BACKGROUND: Extensive evidence indicates that Collaborative Chronic Care Models (CCMs) improve outcome in chronic medical conditions and depression treated in primary care. Beginning with an evidence synthesis which indicated that CCMs are also effective for multiple mental health conditions, we describe a multistage process that translated this knowledge into evidence-based health system change in the US Department of Veterans Affairs (VA). EVIDENCE SYNTHESIS: In 2010, recognizing that there had been numerous CCM trials for a wide variety of mental health conditions, we conducted an evidence
