Endoscopic Fascia Release for Forearm Chronic Exertional Compartment Syndrome: Case Report and Surgical Technique.

BACKGROUND: Chronic exertional compartment syndrome (CECS) of the forearm is traditionally treated with open compartment release requiring large incisions that can result in less than optimal esthetic results. The purpose of this study is to describe a case report of 2 professional motocross patients with forearm CECS treated endoscopically using a minimally invasive technique. METHODS: Two professional motocross racers presented with a history of chronic proximal volar forearm pain when motocross riding. Other symptoms included paresthesia and weakness, which, at times, led to an inability t

The Outcomes of Single-Portal Endoscopic Fasciotomy for Chronic Exertional Compartment Syndrome of the Forearm.

OBJECTIVE: Chronic exertional compartment syndrome (CECS) of the forearm is a rare and underdiagnosed condition that mainly affects young athletes. Minimally invasive endoscopic surgical treatment has been shown to reduce complication rates and allow for an expedited return to activity. Evidence on the outcomes of single-portal endoscopic fasciotomy for forearm CECS remains limited. The aim of this study was to describe the results of single-portal endoscopic-assisted fasciotomy in treating forearm CECS. METHODS: A total of 17 patients (21 forearms) diagnosed with forearm CECS and treated wit

Single minimal incision fasciotomy for the treatment of chronic exertional compartment syndrome: outcomes and complications.

INTRODUCTION: Chronic exertional compartment syndrome (CECS) is a common injury in young athletes, causing pain in the involved leg compartment during strenuous exercise. The gold standard treatment is fasciotomy, but most of the reports on its effectiveness include relatively small cohorts and relatively short follow-up periods. This study reports the long-term results of a large cohort of young athletes who underwent single-incision fasciotomy for CECS. MATERIALS AND METHODS: This a retrospective case-series study. All patients treated by fasciotomies performed for CECS between 2007 and 201

The curvature of the crural fascia is different in anterior leg chronic exertional compartment syndrome.

OBJECTIVES: Patients possibly having chronic exertional compartment syndrome of the anterior leg require confirmatory intracompartmental pressure measurements, but this test is invasive, patient unfriendly and imprecise. Ultrasound studies suggested that the morphology of the leg crural fascia covering the muscle might be changed. Aim of this study was to analyse the crural fascia curvature in chronic exertional compartment syndrome of the anterior leg in patients and healthy controls. DESIGN: Observational. METHODS: From February 2023 to February 2024 patients and controls matched for sex,

Bilateral Lower Extremity Chronic Exertional Compartment Syndrome With Recurrent Symptoms After Prior Fasciotomy: A Case Report.

Chronic exertional compartment syndrome (CECS) is an exercise-related cause of lower extremity pain characterized by reproducible pain, tightness, cramping, and neurologic symptoms that improve with rest. Diagnosis may be delayed because chronic lower leg pain has a broad differential diagnosis, and currently used diagnostic strategies remain imperfect. We report the case of a young woman who began experiencing bilateral lower extremity pain during high school sports that progressed to pain with prolonged standing. Treadmill-provoked bilateral four-compartment pressure testing reproduced sever

Ultrasound-Guided Fasciotomy for Chronic Exertional Compartment Syndrome: A Case Report With Long-Term Sonographic Outcomes.

We report the case of a 38-year-old recreational female runner with anterior and lateral chronic exertional compartment syndrome treated with ultrasound-guided fasciotomy using a V-shaped meniscotome. She began her return-to-run protocol 8 days postprocedure and achieved full symptom resolution. At 6 months, ultrasound revealed persistent fascial separation without complications, representing the first report of long-term sonographic outcomes following fasciotomy for chronic exertional compartment syndrome. These findings support further prospective studies to evaluate the clinical and sonogra

Association of Creatine Use with Chronic Exertional Compartment Syndrome in a Distance Runner: A Case Report.

Chronic exertional compartment syndrome (CECS) of the lower extremity is a condition that affects runners and can result in significant time loss from athletic activity. While the etiology of CECS is often multifactorial, research on the relationship between CECS and supplementation is sparse. This case report describes a runner who developed lower leg CECS following use of creatine and provides considerations of risk factors and differential diagnoses for CECS.

Fasciotomy for chronic exertional compartment syndrome of the leg: clinical outcome in a large retrospective cohort.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an overuse disorder typically affecting an athletic population. CECS is a diagnosis based on history and intracompartmental pressure (ICP) testing. CECS patients can be treated surgically by fasciotomy; however, research on the relationship between ICP and patient symptoms and also between ICP and patient-reported outcome post-fasciotomy is limited. This study aims to (1) assess functional outcome and patient satisfaction post-fasciotomy and (2) identify any potential correlation between ICP and reported levels of pain. METHODS: 13

Noninvasive Diagnosis of Chronic Exertional Compartment Syndrome With Shear Wave Elastography and Subharmonic-Aided Pressure Estimation.

OBJECTIVE: Chronic exertional compartment syndrome (CECS) is an underdiagnosed condition that affects young athletes. CECS is caused by increased compartmental pressure in the fascial spaces during exercise. CECS is diagnosed by direct pressure readings (in mmHg), which is a painful and invasive test. Noninvasive shear-wave elastography (SWE) and subharmonic-aided pressure estimation (SHAPE) using contrast-enhanced ultrasound were compared to compartment pressure testing for the diagnosis of CECS. METHODS: Ten healthy volunteers and five CECS patients enrolled in this prospective, IRB-approve

Botulinum Toxin A for Chronic Exertional Compartment Syndrome Evaluated With Shear Wave Elastography: A Case Report.

This case presentation offers supportive evidence that shear wave elastography may provide an alternative method of diagnosis of chronic exertional compartment syndrome (CECS). A 39-year-old female runner presented with bilateral anterior shin pain on exertion. She initially underwent compartmental pressure testing confirming the diagnosis of CECS but declined fasciotomy. When her symptoms recurred, she was referred for botulinum toxin therapy. Shear wave muscle elastography was performed in the bilateral anterior and lateral compartments following symptom provocation treadmill testing and com

Dual-incision minimally invasive fasciotomy of the anterior and peroneal compartments for chronic exertional compartment syndrome of the lower leg.

To evaluate the risk of iatrogenic injury when using a dual-incision minimally invasive technique to decompress the anterior and peroneal compartments of the lower leg. Forty lower extremities from 20 adult cadavers, embalmed with Thiel's method, were subject to fasciotomy of the anterior and peroneal compartment using a dual-incision minimally invasive fasciotomy. The first incision was made 12 cm proximal to the lateral malleolus to identify and protect the superficial peroneal nerve (SPN). The second incision was made at the mid-point of the Fibula (half-way between the fibular head and the

Ultrasound-Guided Fasciotomies of the Deep and Superficial Posterior Leg Compartments for Chronic Exertional Compartment Syndrome: A Cadaveric Investigation.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is a type of leg pain related to elevated intracompartmental pressure with activity in one or more of the four compartments of the leg. Open fasciotomy is the definitive treatment for CECS but has a reported complication rate of up to 15.7% and return to full activity reported up to 16 weeks. Ultrasound-guided (USG) fasciotomy of the anterior and lateral compartments has been translated into clinical practice. OBJECTIVE: To determine the safety and feasibility of a USG fasciotomy of the deep posterior compartment (DPC) and superficial

Bilateral Lower-Leg Chronic Exertional Compartment Syndrome Treated With In-Office Needle Endoscopic 4-Compartment Fasciotomy in Collegiate Sprinter: A Case Report.

Chronic exertional compartment syndrome (CECS) is a condition which involves sudden and painful increases in pressure of the musculofascial compartments of the lower leg in association with increased physical activity. A variety of surgical options, including endoscopic fasciotomy, exist to mechanically decompress the fascia. Recent progress in in-office needle endoscopy has shown potential for the diagnosis and treatment of various orthopaedic pathologies. We present the case of a 20-year-old collegiate sprinter with a 3-month history of CECS that was treated with bilateral in-office needle e
