The presence of chyle in a hemodialysis catheter is a rare complication, usually observed in case of chylothorax secondary to central vein stenosis, particularly of the superior vena cava and the left brachiocephalic vein. We report the exceptional case of a 55-year-old woman in maintenance hemodialysis presenting with chyle in her left subclavian tunneled cuffed catheter during each interdialytic period, without any recent history of trauma or tumoral disease. The chest CT scan was unremarkable. The milky fluid was systematically observed by the nurses at the beginning of each hemodialysis se

BACKGROUND: The aim of this study was to investigate the impact of thoracic duct embolization (TDE) on the management of postoperative chyle leakage.
METHODS: We retrospectively reviewed the electronic medical record database of 4171 patients who underwent curative resection for lung or esophageal cancer between January 2015 and June 2017. We classified the period before the introduction of TDE as the first period and the period after the introduction of TDE as the second period.
RESULTS: A total of 105 patients who developed chyle leakage after surgery were included. In the first period, 49

Postoperative chyle leak is an exceedingly rare complication following breast and axillary surgery. We present the first described case of chyle leak following breast-conserving surgery and sentinel lymph node biopsy. Management should begin with appropriated conservative measures aimed at reduction of lymph production and flow. Intervention is warranted when conservative strategies fail and include sclerotherapy, lymphangiography, embolization, and surgery. Breast surgeons should be mindful of this potential complication when operating in the axilla and be familiar with its stepwise managemen

Graft-related complications, such as pseudoaneurysms and seromas, are rare following aortic replacement. The occurrence of localized chyle leakage is exceptionally uncommon. A 59-year-old male presented with chest pain due to an expanding 5-cm perigraft fluid collection 5 years after emergent ascending aortic replacement for acute aortic dissection. Surgical intervention was indicated as a pseudoaneurysm could not be ruled out. Intraoperatively, a chronic, localized chyle leakage (chyloma) was confirmed around the graft, showing high triglyceride and total cholesterol levels on biochemical ana

Descending necrotizing mediastinitis, a serious complication of odontogenic infections, rarely leads to chyle leakage postdrainage, with few cases reported. We describe this complication in a 62-year-old woman following mediastinal drainage. By analyzing this and previous cases, we identify early warning signs to enhance clinical recognition of this rare occurrence.

Vascularized lymph node transfer is a physiologic microsurgical technique used for the treatment of lymphedema. As vascularized lymph node transfer is becoming more common, it is essential that one is aware of all potential complications associated with vascularized lymph node transfer and know how to avoid and manage them when they do occur. The authors recently encountered a complication after supraclavicular vascularized lymph node transfer that has not been previously reported. A patient developed a recalcitrant high-output (>500 ml/day) chyle leak in the neck donor site after supraclavicu

The occurrence of postoperative chyle leak after neck dissection or vascular surgery of the subclavian and vertebral artery is a well-known but rare complication. The management of chyle leak ranges from utilizing a myriad of conservative procedures to surgical exploration. Occasionally on surgical exploration the chyle leak is diffuse and ligation of the bed of thoracic duct is not adroit to stopping the chyle leak. We describe a technique, which will be useful in this circumstance and in recurrent chyle leak after a previous exploration.

BACKGROUND: Chyloma is a rare pseudocystic collection of chyle resulting from leakage of the thoracic duct or its branches. While sporadically reported in humans, it has not been previously described in veterinary medicine. This report describes the first case of an idiopathic mediastinal chyloma in a cat. CASE PRESENTATION: A three-year-old, castrated male, domestic shorthair cat presented with acute dyspnea and lethargy. Thoracic computed tomography (CT) revealed a well-circumscribed cystic mass in the right caudal mediastinum causing compressed the adjacent lungs and heart. The lesion was s

Chyle leak (CL) secondary to inadvertent thoracic duct injury (TDI) is an exceptionally rare complication of a right hemithyroidectomy. TDI occurs in 0.5-1.4% of patients undergoing a thyroidectomy, and of those, only one-fourth are attained following right-sided neck surgeries, as the terminal thoracic duct (TD) typically terminates in the left side of the neck. Anatomical variations of the terminal TD are well renowned, and a right-sided terminal TD has been previously described. Although CL following right-sided neck dissection has been described in the literature, to our knowledge, this is

Patients undergoing esophagectomy are at risk of malnutrition and benefit from perioperative enteral feeding. Esophagectomy carries a risk of chyle leak, and this risk may be influenced by early enteral feed composition. We evaluated the impact of early enteral medium-chain triglyceride-rich feed on the prevalence and severity of chyle leak post-esophagectomy, length of stay, and postoperative weight change. This retrospective study included consecutive patients undergoing esophagectomy at a single center between January 2015 and December 2022. Patients received enteral feed on postoperative d

BACKGROUND: Scarring is the product of natural restoration, yet its treatment remains challenging. Both collagen and fibroblasts are abnormally abundant in scars, leading to scar hyperplasia or contracture. Several clinical studies have reported that wrinkles at the recipient site are reduced, pores are narrowed, pigmentation is decreased, and skin is softened after autologous fat transplantation. In this study, we investigated the ability of autologous chyle fat injection to normalize the fibroblasts and collagen of scar tissue in 80 adult patients with hypertrophic scars resulting from sever

OBJECTIVE: To identify the value of lymphography in the location and treatment decision of chyle leakage.
METHODS: The clinic data of 177 patients suffered from chyle leakage admitted in 6 medical centers in Shanghai from February 1998 to December 2014 was analyzed retrospectively. There were 94 male and 83 female patients aging from 9 to 84 years with a mean of 49 years, including 128 cases of chyluria, 34 cases of primary chylothorax and 15 cases of other chyle leakage. All patients had failed to conservative treatment more than 2 weeks. Pedal lymphography was performed in every patient to

A 63-year-old male smoker developed a refractory cervical chylous fistula following neck surgery for metastatic squamous cell carcinoma. Despite conservative management with a low-fat diet and subsequent surgical thoracic duct ligation, high-output chyle drainage persisted. On postoperative day 14, percutaneous thoracic duct embolization with lipiodol and n-BCA was successfully performed, resulting in immediate and sustained resolution of the leakage. The patient was discharged 48 hours later without recurrence. This case highlights thoracic duct embolization as a safe, minimally invasive, and

OBJECTIVE: Few cases of seizures associated with olanzapine therapy and even fewer with mirtazapine have been published, most of them in patients with confounding risk factors. Our objective was to report a case of Status epilepticus in a patient receiving olanzapine and mirtazapine, with no previous history of seizure and no confirmed underlying cause for seizure.
CASE SUMMARY: A 48-year-old white, psychotic woman developed generalized tonic-clonic seizures that progressed to Status epilepticus during hospitalization. 4 days before the incident, mirtazapine (30 mg) was added to the treatment

Chyle leak is a relatively rare post-operative complication of breast and axilla surgery, and the majority of these cases can be managed conservatively. We present the case of a 42-year-old female with chyle leak within the external thoracic cavity following elective augmentation mammoplasty and mastopexy. This patient failed initial operative management consisting of implant removal, washout, suture plication, and intra-operative drain placement as well as subsequent conservative management including diet modification, total parenteral nutrition, and percutaneous aspiration. Ultimately, this
