INTRODUCTION: Humanitarian crises exacerbate the vulnerability of already fragile healthcare systems and significantly increase the risk of infectious disease outbreaks in low- and middle-income countries (LMICs). This systematic review aims to evaluate strategies and interventions implemented in LMICs to prevent and manage infectious diseases outbreaks during humanitarian crises from 2018 to 2023.
METHODS: A comprehensive literature search was conducted across Scopus, PubMed, and Web of Science, adhering to the PRISMA guideline and the SPIDER framework to identify relevant studies. The revie

Pandemic influenza A (H1N1) commenced in April 2009. Robust planning and preparedness are needed to minimize the impact of a pandemic. This study aims to review if key elements of pandemic preparedness are included in national plans of European countries. Key elements were identified before and during the evaluations of the 2009 pandemic and are defined in this study by 42 items. These items are used to score a total of 28 publicly available national pandemic influenza plans. We found that plans published before the 2009 influenza pandemic score lower than plans published after the pandemic. P

Biological emergencies strain health care facilities that are typically designed to accommodate routine surges. The SARS-CoV-2 pandemic exposed vulnerabilities in hospital oxygen delivery systems worldwide, resulting in ventilator failures and disruptions in patient care. At the American University of Beirut Medical Center (AUBMC), the Emergency Department experienced significant challenges in maintaining the oxygen delivery system due to the unprecedented oxygen demand, over-frosting of pipelines, and system-wide depressurization. In response, the AUBMC established the Emergency Engineering R

Nursing home residents experience death and injury at a disproportionately higher rate than other populations during and after a disaster. This paper addresses the complex chronic conditions of vulnerable residents that will become even more challenging due to compounding effects from continued disasters and rapid population aging. In 2016, the Centers for Medicare and Medicaid Services (CMS) established national emergency preparedness requirements for long-term care (LTC) and skilled nursing (SN) facilities. The problems facing nursing facilities created by an increasing population to serve,

Multiple institutions, such as donor registries, donor centers, transplantation centers, collection centers, and courier companies, are involved in the international exchange of hematopoietic stem cells. The ability to safely and efficiently ensure continued operation of a donor registry relies on an organization's resiliency in the face of an incident that could impede donor search, donor selection, stem cell collection, or transportation. The Quality Assurance Working Group of the World Marrow Donor Association has developed guidelines on how to establish an organizational resiliency program

BACKGROUND: In the event of a release of a pathogen such as smallpox, which is human-to-human transmissible and has high associated mortality, a key question is how best to deploy containment and control strategies. Given the general uncertainty surrounding this issue, mathematical modelling has played an important role in informing the likely optimal response, in particular defining the conditions under which mass-vaccination would be appropriate. In this paper, we consider two key questions currently unanswered in the literature: firstly, what is the optimal spatial scale for intervention; a

Radiological incidents are rare, but can have significant public health consequences. Coordination across jurisdictions, government agencies, and different fields is critical to ensure an effective response that minimizes health impacts. State and local government agencies face challenges in responding to radiological incidents such as constrained resources, siloed communications, and gaps in workforce training and capacity. The National Alliance for Radiation Readiness (NARR) is a network of experts composed of 20 member organizations and 10 federal partners. The NARR seeks to advance the nat

Unplanned radiotherapy interruptions can reduce treatment effectiveness, increase recurrence risk, delay symptom palliation, and complicate completion of the prescribed course. Effective preparedness requires strategies to prevent interruptions, limit their duration, and support safe completion of treatment when disruptions occur. This review frames emergency preparedness for external beam radiotherapy (EBRT) machines as a treatment-continuity challenge within the radiation oncology workflow. Relevant disruptions may arise from the treatment machine, simulator, imaging or image-guidance system

Upon request from tribal nations, and as part of the Centers for Disease Control and Prevention's (CDC's) emergency response, CDC staff provided both remote and on-site assistance to tribes to plan, prepare, and respond to the COVID-19 pandemic. From April 2, 2020, through June 11, 2021, CDC deployed a total of 275 staff to assist 29 tribal nations. CDC staff typically collaborated in multiple work areas including epidemiology and surveillance (86%), contact tracing (76%), infection prevention control (72%), community mitigation (72%), health communication (66%), incident command structure (55

The prolongation of the Ebola epidemic may have allowed some countries to prepare and respond to the coronavirus disease (COVID-19) outbreak. In Uganda, the surveillance structure built for Ebola virus disease (EVD) has become a pillar in the COVID-19 response. This testing and tracing apparatus has limited disease spread to clusters with zero mortality compared with the neighboring East African countries. As more sub-Saharan countries implement social distancing to contain the outbreak, the interventions should be phased and balanced with health risk and socioeconomic situation. However, havi

Infectious diseases resulting in epidemics and pandemics have moulded human history and continue to do so even today. Cholera, plague, human immunodeficiency virus (HIV), and influenza have been some of the most brutal killers. The advancement of medical science has helped us win the war against several of these deadly agents. However, despite all our efforts, new agents continue to emerge. Studying the pandemics of the past helps us respond better to those affecting the globe today. In this narrative review, we present the greatest pandemics of the past, and discuss how the lessons from histo

Through the National Center for Integrated Civilian-Military Domestic Disaster Medical Response, the Yale New Haven Center for Emergency Preparedness and Disaster Response and US Northern Command have initiated a study to determine the requirements of a national operational epidemiological modelling process. During a public health emergency, decision makers often require a variety of epidemiological information, including prospective forecasts that can be made available in the form of models. Currently, there is no formal process across US Government agencies and departments to synchronise epi

Can the same numbers and letters which are the life blood of modern business and government computer systems be harnessed to protect computers from attack against known information security risks? For the past seven years, Foreign Service officers and technicians of the US Government have sought to maintain diplomatic operations in the face of rising cyber attacks and test the hypothesis that an ounce of prevention is worth a pound of cure. As eight out of ten attacks leverage known computer security vulnerabilities or configuration setting weaknesses, a pound of cure would seem to be easy to

Five years after the US anthrax attacks, and more than two years after BioShield legislation was ratified, a survey reveals that biodefense funding has thus far produced only a handful of products for clinical development.

The tragic loss of lives on September 11 followed by the Anthrax contaminations reinforced the need for work already underway through the Metropolitan Medical Response System (MMRS). Through leadership from the Medical Director for the Medical Control Board and EMSA, Oklahoma City and Tulsa have been the beneficiaries of nearly $1.5 million dollars in funding to prepare for nuclear, chemical or bioterrorism. The two Oklahoma cities were among the second wave of cities invited in 1999 to participate in the federally funded program for Metropolitan Medical Response. Many of the requirements of t

Despite the long-standing efforts to promote whole-of-government and whole-of-society models, health emergency preparedness is still challenged by siloed institutional or sectoral approaches. In practice, no unified mechanism has existed to systematically bring different sectors, communities, countries or regions to strengthen preparedness capacities. In response to these persistent gaps, World Health Organization (WHO) introduced the Universal Health and Preparedness Review (UHPR) as one of its Post COVID-19 initiatives. This commentary examines the place of UHPR in the sphere of health emerg
