BACKGROUND: High rates of clinical alarms in the intensive care unit can result in alarm fatigue among staff. Individualization of alarm thresholds is regarded as one measure to reduce non-actionable alarms. The aim of this study was to investigate staff's perceptions of alarm threshold individualization according to patient characteristics and disease status.
METHODS: This is a cross-sectional survey study (February-July 2020). Intensive care nurses and physicians were sampled by convenience. Data was collected using an online questionnaire.
RESULTS: Staff view the individualization of alar

AIMS AND OBJECTIVES: This study examined the occurrence rate of specific types of premature ventricular complex (PVC) alarms and whether patient demographic and/or clinical characteristics were associated with PVC occurrences.
BACKGROUND: Because PVCs can signal myocardial irritability, in-hospital electrocardiographic (ECG) monitors are typically configured to alert nurses when they occur. However, PVC alarms are common and can contribute to alarm fatigue. A better understanding of occurrences of PVCs could help guide alarm management strategies.
DESIGN: A secondary quantitative analysis fr

BACKGROUND: Alarm fatigue occurs when there is frequent exposure to an increased amount of false or nonactionable alarms. Alarm fatigue can lead to alarm desensitization and reduced clinical response by nurses, posing a significant risk to patient safety. Alarm fatigue is an important and growing safety concern that needs to be explored and addressed.
OBJECTIVES: The purpose of this qualitative study was to better understand how Intensive Care Unit (ICU) nurses make decisions about and respond to alarms, specifically how they recognize different alarms, and how they triage and manage them wit

PURPOSE: Intravenous smart pumps (IVSPs) are designed to safely and accurately deliver IV medications to patients. IVSP programming alerts are relatively well understood; however, little is known about IVSP alarms, their contribution to alarm fatigue, and how to minimize unnecessary ones. The purpose of this study was to describe the volume, frequency, and types of IVSP infusions and alarms that occurred over a 2-year period in 107 US hospitals.
METHODS: A retrospective, descriptive analysis of a large-scale dataset containing IVSP infusion and alarm data was conducted. Additional variables,

BACKGROUND: The excessive number of false alarms in the intensive care unit (ICU) environment constitutes a significant clinical issue, contributing to alarm fatigue among medical staff. Effective alarm management is crucial for reducing the incidence of false alarms and enhancing patient safety in the critical care setting. This study aimed to conduct a quantitative analysis of alarms generated by cardiac monitors, assess ICU staff attitudes toward the alarm system, and implement and evaluate the effectiveness of an intervention designed to improve alarm management in an anaesthesiology and i

ST segment monitoring in the adult population allows for the early detection of myocardial ischaemia. In children admitted to the paediatric intensive care unit (PICU), cardiac intensive care unit (CICU), and cardiac progressive care unit (CPCU), it is unclear if continuous ST segment alarm monitoring is necessary in all patients. All patients admitted to the PICU, CICU, and CPCU during the study period were included. Children with any ST segment alarms were compared with those without an alarm during their stay. The electrocardiogram confirmed true ST segment alarms were compared with all oth

The high rate of false arrhythmia alarms in Intensive Care Units (ICUs) can lead to disruption of care, negatively impacting patients' health through noise disturbances, and slow staff response time due to alarm fatigue. Prior false-alarm reduction approaches are often rule-based and require hand-crafted features from physiological waveforms as inputs to machine learning classifiers. Despite considerable prior efforts to address the problem, false alarms are a continuing problem in the ICUs. In this work, we present a deep learning framework to automatically learn feature representations of ph

We present a standardized test methodology and results for our evaluation of the Carefusion Alaris PC infusion pump, comprising the model 8015 PC Unit and the model 8100 Large Volume Pump (LVP) module. The evaluation consisted of basic suitability testing, internal component inspection, surface temperature measurement of selected internal components, and critical performance testing (infusion rate accuracy and occlusion alarm pressure) during conditions of typical hyperbaric oxygen (HBO₂) treatment in our facility's class A multiplace chamber. We have found that the pumps pose no enhanced risk

Early recognition and treatment of paediatric sepsis has been shown to improve survival. Automated sepsis alerts embedded in electronic medical records (EMRs) offer real-time identification of high-risk patients and may improve timeliness of sepsis care, with possible limitations including overdiagnosis, overtreatment and alert fatigue. It is currently unclear whether automated alerts improve clinical outcomes in paediatric sepsis. We conducted a systematic review and narrative synthesis of studies evaluating clinical outcomes (mortality, PICU admission rate, length of stay), process measures

BACKGROUND: Wireless continuous vital sign monitoring by wearable devices have recently become available for patients on general wards to promote timely detection of clinical deterioration. Many continuous monitoring systems use conventional threshold alarm settings to alert nurses in case of deviating vital signs. However, frequent false alarms often lead to alarm fatigue and inefficiencies in the workplace. The aim of this study was to determine the feasibility of continuous vital sign monitoring without the use of alarms, thereby exclusively relying on interval trend monitoring.

BACKGROUND: Technological progress improves health care efficiency, quality, safety, and cost, supporting clinical activity in various scenarios, such as Intensive Care Unit (ICU). A timely response to alarms from monitors and other ICU electromedical devices is therefore crucial. The number of false alarms tend to desensitize care providers increasing the risk of experiencing alarm fatigue and, at times, lead to severe consequences for patients.
OBJECTIVES: To assess the psychometric properties of the Italian version of the Alarm Fatigue Questionnaire and to explore the phenomenon of alarm f

BACKGROUND: A recent study found that activation of disconnection and low-pressure alarms is common during mouthpiece ventilation and may represent a major limitation to its use. The aim of this bench study was: (1) to investigate the technical aspects that can influence the setting of the ventilator during mouthpiece ventilation and (2) to provide a practical setting strategy to avoid the alarm activation.
METHODS: Eight life-support ventilators able to deliver volume controlled ventilation were tested in a bench study using a single-limb non-vented circuit configuration connected to a stand

BACKGROUND: Continuous, wearable-based vital sign monitoring can enhance patient safety and reduce intensive care demands but is not routinely used on standard neurosurgical wards. This study assessed alarm events and technology acceptance during the implementation of such a system for postoperative patients in a university hospital's neurosurgical ward.
METHODS: In this pilot feasibility study, selected patients were continuously monitored for 12 to 24 hours after elective neurosurgery on a 44-bed ward. Phase 1 (June to December 2019) used 12 mobile units with manual alarm documentation; Pha

OBJECTIVE: Intensive care unit (ICU) clinicians respond to over 900 alarms per day, resulting in sensory overload that leads to delayed and inaccurate clinician responses. Integrating vital signs into object-based visual displays has been shown to yield quicker and more accurate responses than standard ICU alarm displays. We designed a ring-based visual alarm system that integrates heart rate (HR), blood pressure (BP), and peripheral oxygen sat-uration (SpO2) and evaluated its effectiveness using a virtual reality (VR) testing environment. VR offers significant advantages over traditional setu

BACKGROUND: Conditioning with an alarm is one of the first-line treatments for enuresis. It is safe and has well-documented efficacy. It has been debated, if conditioning leads to dryness by converting enuresis to nocturia at least in some children. The aim of this study was to evaluate the outcome of alarm treatment in children with treatment naïve, monosymptomatic enuresis (MNE).
METHODS: We utilized data from the prospective RCT DRYCHILD and analyzed a case series of treatment-naïve children aged 6-14 years with MNE who received alarm treatment for 8 weeks. Inclusion required three or more

Enuresis (bedwetting) is common in school-aged children and can impact health, psychosocial well-being and quality of life. Although effective treatment is available, treatment resistance is encountered in about 50%. This paper discusses the management of treatment-resistant enuresis from a multidisciplinary perspective. Causes of treatment resistance include lower urinary tract problems, constipation, incorrect alarm training techniques, sleep disorders including sleep apnoea and psychological comorbidities. Practical suggestions to address treatment resistance are offered utilising expertise

This report consists of two separate studies on the use of continuous capnography monitoring conducted in an effort to improve patient safety at Virtua Health System. The desire for improved patient safety is motivating continuous monitoring and improved surveillance in clinical areas not traditionally equipped for such monitoring. We explored the use of remote monitoring of capnography, using enterprise middleware, in patients recovering from surgery in a medical-surgical unit. Continuous monitoring traditionally has been used in higher-acuity settings, such as intensive care units. Patients

BACKGROUND: Diagnostic and interventional procedures are often facilitated by moderate procedure-related sedation. Many studies support the overall safety of this sedation; however, adverse cardiovascular and respiratory events are reported in up to 70% of these procedures, more frequently in very young, very old, or sicker patients. Monitoring with pulse oximetry may underreport hypoventilation during sedation, particularly if supplemental oxygen is provided. Capnometry may result in false alarms during sedation when patients mouth breathe or displace sampling devices. Advanced monitor use du
