Evidence-based suction management in accident and emergency: a vital component of airway care.

Effective airway management aims to establish and maintain a patent airway to ensure adequate alveolar ventilation and patient survival. Suction therapy must be regarded as a vital component of airway care, with all staff who are required to perform this procedure being aware of the principles of safe, effective suctioning. This paper provides an outline of the types of suction catheters available for use in the emergency care setting, and describes the safe procedures to be followed for both endotracheal and oro-pharyngeal suctioning. Potential complications are discussed other headings of re

An integrative review of rural and remote nursing graduate programmes and experiences of nursing graduates.

AIMS AND OBJECTIVES: To examine international studies that specifically focus on transition to practice for graduate registered nurses in rural and remote areas. BACKGROUND: Supportive graduate nursing programmes are essential for enabling nursing graduates' transition to practice and reducing attrition rates. Literature examining support measures for nursing graduates within metropolitan areas is abundant. However, there is a paucity of evidence on effective graduate programmes for rural and remote-based nursing graduates. DESIGN: A systematic approach was used to identify robust research w

How to make research happen: working with staff.

Implementing a research project in a clinical nursing unit requires the researcher to gain cooperation and support from administration, physicians, and the nursing staff. In this article the site coordinators from the Transition of the Preterm Infant to an Open Crib research-utilization project share their strategies for generating support, energizing the staff nurses, assuring compliance with the study, resolving problems, and bringing the project to closure. Many of the strategies used can be applied to other research projects by nurse researchers interested in using clinical nursing units a

Starting a career in research nursing during a global pandemic.

In the spring of 2020, two nurses (KR and AJ) commenced their research nurse careers amid the SARS-Cov-2 (COVID-19) global pandemic. This reflective article discusses their experiences of beginning a clinical research nursing career, presented as a case study of their learning journey, rather than detailing the randomised controlled trial they delivered via GP practices. The main study compared standard care to nurse-led management of irritable bowel syndrome, the details of which will be published separately. The article identifies three overarching concepts: 'Green as grass, keen as mustard'

Measuring and communicating blood loss during obstetric hemorrhage.

Accurate quantification of blood loss is an essential skill necessary to prevent maternal morbidity and mortality associated with obstetric hemorrhage. Visual estimation of blood has been consistently shown to be extremely inaccurate. The nurse plays a pivotal role in quantifying blood loss after birth, recognizing triggers, mobilizing needed interventions, and providing essential communication.

From noticing to reflection: A qualitative exploration of rapid cycle deliberate practice effects on electrocardiographic monitoring judgment in critical cardiac care nurses.

BACKGROUND: Electrocardiographic interpretation competency among critical cardiac care nurses remains inadequate despite technological advances, creating patient safety concerns and alarm fatigue. Traditional nursing education utilizing didactic knowledge transmission has proven insufficient for developing time-critical electrocardiographic interpretation skills. Rapid cycle deliberate practice emerges as an innovative methodology integrating deliberate practice principles, immediate feedback, and psychologically safe environments through pause-coach-resume mechanisms. METHODS: Semi-structure

"Ask me what I need": A call for shifting responsibility upwards and creating inclusive learning environments in clinical placement.

BACKGROUND: Ontario's human-rights legislation and institutions' support for students with disabilities have influenced these students' increased enrolment in the postsecondary environment. Yet more attention is paid to in-classroom than to out-of-classroom experiences. Students with disabilities enrolled in nursing are least likely to be accommodated. This has significant implications for access and equity upon graduation, as clinical placements are a bridge between school and employment. OBJECTIVES: To explore clinical instructors' and students who identify with disabilities' perspectives o

Experiences and perceptions of nurses working night shift: a qualitative systematic review.

OBJECTIVE: The objective of this review was to examine the available evidence on the experiences and perceptions of nurses working the night shift within any specialty in the acute care, subacute, or long-term care setting. INTRODUCTION: Nurses are required for around-the-clock patient care. Night shift nurses can experience detrimental effects because of their work hours, which disrupt their normal circadian rhythm. Understanding nurses' experiences and perceptions when working night shift will facilitate the development of strategies to minimize the potential negative effects of working at

Nursing clinical reasoning cognitive strategies used in a learning-by-concordance modality: A qualitative descriptive study.

AIMS: The primary aim of this study is to describe the cognitive strategies employed by nursing students during a Learning-by-Concordance (LbC) activity. A secondary aim is to compare these strategies with those used by experienced nurses. BACKGROUND: Technological advancements have facilitated the integration of diverse pedagogical modalities into nursing education. However, the mechanisms by which digital modalities support the development of cognitive strategies for clinical reasoning remain insufficiently understood. DESIGN: A descriptive qualitative design was adopted. METHODS: Content

A theoretical analysis of Carper's ways of knowing using a model of social cognition.

Although Carper's model of the ways of knowing in nursing has played a critical role in delineating the body of knowledge that comprises the discipline, questions remain regarding the defining properties of the knowledge structures, how they relate to each other, and how they function in the process of knowing. In this article, we propose that the cognitive model of social knowledge developed within the field of social psychology can be used to extend our understanding of the form and function of the four types of nursing knowledge. Empirical, personal, esthetic and ethical knowledge are compa

Seeking quality care for patients with pressure ulcers.

Improving quality care for patients with pressure ulcers resulted in a two-year intensive effort to educate staff, keep morale high, standardize care plans by stage of ulcer, and participate in a comparison study of two support surfaces (Thera-Pulse, Kinetic Concepts and Geo-Matt, SpanAmerica). When all pressure ulcers were considered, the analysis of covariance revealed no statistically significant difference in the healing of pressure ulcers with respect to type of support surface used (F[1, 78] = 0.35, p > .05). For patients with stage III and IV pressure ulcers, the proportion of patients

Episiotomy: does it have to hurt so much afterwards?

Episiotomy is one of the commonest surgical procedures in the UK. Many women suffer pain for several days sometimes weeks afterwards and dyspareunia may be a problem. Absorbable suture materials appear to be preferable to non-absorbable ones. Infiltrating the perineum with normal saline before suturing may help reduce postpartum perineal pain. Omitting sutures just below the skin surface may cause less pain. More research is needed. The skill of the operator seems to be a factor. Midwives need to evaluate their practice in repairing the perineum

Nursing Understanding and Perceptions of Delirium: Assessing Current Knowledge, Attitudes, and Beliefs in a Burn ICU.

The number of delirium days in hospitalized patients directly correlates with mortality and long-term cognitive dysfunction. Burn patients are at greater risk for delirium due to prolonged mechanical ventilation, high sedative and analgesic medication requirements, and the common need for multiple operations. Limited research exists on nurses' understanding and comfort using delirium screening tools and preventive interventions. A process improvement project was developed in a single, regional burn intensive care unit (BICU) with the goal of increasing RN staff awareness of delirium, delirium

AIM: To explore the learning experiences of post-anaesthesia care unit nurses after their participation in a delirium care for older people education intervention. BACKGROUND: Post-operative delirium is a common, yet major complication for people 65 years or older who have undergone surgery. Yet, post-operative delirium in older people continues to go undetected, has delayed detection or is entirely misdiagnosed. Inadequate delirium recognition and management is in part due to the lack of knowledge, competence and confidence of post-anaesthesia care unit nurses caring for older people. Multim
