Pharmaceutical profile and prescribing practices in compounding pharmacies in Brazil: a cross-sectional study.

INTRODUCTION: The industrialization expanded drug access but standardized formulations often fail to meet individual clinical specificities. Compounding pharmacies fill this gap by providing personalized therapies. In Brazil, although Federal Pharmacy Council Resolutions grant pharmacists the autonomy to prescribe over-the-counter drugs and compounded formulations, this clinical practice faces legal instability and structural challenges. This study aimed to outline the profile of pharmacists and analyze the clinical practice of pharmaceutical prescribing in compounding pharmacies across Brazil

Convenience without disclosure: a formative research study of a proposed integrated methadone and antiretroviral therapy service delivery model in Dar es Salaam, Tanzania.

BACKGROUND: Though timely initiation of antiretroviral therapy (ART) is a vital component of effective HIV prevention, care and treatment, people who inject drugs are less likely to receive ART than their non-drug using counterparts. In an effort to increase access to ART for people who inject drugs, we examined perceived benefits, challenges, and recommendations for implementing an integrated methadone and ART service delivery model at an opioid treatment program (OTP) clinic in Dar es Salaam, Tanzania. METHODS: We conducted in-depth interviews with 12 providers and 20 HIV-positive patients

Safety and efficacy of sodium ferric gluconate complex in patients with chronic kidney disease.

BACKGROUND: We hypothesized that intravenous iron will improve hemoglobin (Hgb) concentrations in anemic patients with chronic kidney disease (CKD), and the response would be greater if the underlying erythropoietin deficiency also was treated. METHODS: Charts of 58 CKD veterans (glomerular filtration rate < 80 mL/min) administered at least 125 mg of sodium ferric gluconate complex in sucrose (SFGC) during a period of 1 year for the primary outcome of an increase in Hgb level by at least 0.5 g/dL were reviewed. RESULTS: Mean Hgb level at baseline was 10.5 +/- 1.4 (SD) g/dL (105 +/- 14 g/L) i

Implementing a medicines dispensing service in a university pharmacy: A qualitative study of experiential learning and work system factors.

BACKGROUND: Dispensing can be a strategic clinical service when organized around the care process. Such models are relevant in university affiliated pharmacies (UPh), where service delivery is integrated with experiential education. However, their implementation may be challenged by organizational resistance. This study explored barriers and facilitators to implementing a medicines dispensing service model from a sociotechnical perspective. METHODS: A qualitative study was conducted in a UPh. Participants were pharmacist preceptors, faculty members, and pharmacy interns (n = 18). Data were co

[Use of a pharmaceutical decision support system in the valuation of hospital stays: Evaluation through 3 examples in collaboration with the department of medical information].

INTRODUCTION: Pharmacy decision support systems (PDSS) help clinical pharmacists to prevent and detect adverse drug events. The coding of hospital stays by the department of medical information (DMI) requires expertise, as it determines hospital revenues and the epidemiological data transmitted via the French national hospital database. The aim was to study the interest and feasibility of using a PDSS, in collaboration with the DMI, to help with the coding of hospital stays. METHODS: Over 5 months, three rules were implemented in the PDSS to detect gout, Parkinson's disease and oro-pharyngeal

Harnessing private sector strategies for family planning to deliver the Dual Prevention Pill, the first multipurpose prevention technology with pre-exposure prophylaxis, in an expanding HIV prevention landscape.

INTRODUCTION: The Dual Prevention Pill (DPP) combines oral pre-exposure prophylaxis (PrEP) with oral contraception (OC) to prevent HIV and pregnancy. Noting the significant role played by the private sector in delivering family planning (FP) services in countries with high HIV burden, high level of private sector OC uptake, and the recent growth in self-care and technology-based private sector channels, we undertook qualitative research in Kenya, South Africa and Zimbabwe to prioritize private sector service delivery approaches for the introduction of the DPP. METHODS: Between March 2022 and

Pharmacist Interventions and Drug-Related Problems Classification Systems in the Inpatient Setting: A Systematic Review of Psychometric Properties.

BACKGROUND: Pharmacist intervention (PI) is an important strategy for reducing drug-related problems (DRP; events or circumstances related to drug therapy that actually or potentially interfere with desired health outcomes) and improving efficiency, quality, and clinical outcomes, thereby avoiding unnecessary health care costs. This systematic review aims to identify and analyze studies that have validated PI and/or DRP classification systems within inpatient hospital settings, and to assess the validity and quality of the methodological procedures adopted in these systems. METHODS: This rese

International expert perspectives on pharmacogenomics in pharmacy education: a qualitative study with implications for South Africa.

INTRODUCTION: Pharmacogenomics (PGx) has the potential to transform healthcare in South Africa through the delivery of precision medicine. However, the successful implementation of PGx services depends on a healthcare workforce equipped with appropriate PGx competencies. Given pharmacists' central role in medication management, they are key members of the PGx team and require targeted education to support PGx-guided clinical decision-making. Insights from international experts can inform the development of pharmacy curricula, strengthening pharmacists' capacity to facilitate PGx implementation

Risky alcohol use among patients dispensed opioid medications: A clinical community pharmacy study.

BACKGROUND: Included among the significant risk factors for opioid overdose is concomitant use of other central nervous system depressants, particularly alcohol. Given the continued expansion of community pharmacy in the continuum of care, it is imperative to characterize alcohol use among pharmacy patients dispensed opioids in order to establish a foundation for identification and intervention in these settings. METHODS: This secondary analysis utilized data from a one-time, cross-sectional health assessment conducted among patients dispensed opioid medications in 19 community pharmacies in

Cost comparison of Transcatheter and Operative Pulmonary Valve Replacement (from the Pediatric Health Information Systems Database).

Outcomes for transcatheter pulmonary valve replacement (TC-PVR) and operative pulmonary valve replacement (S-PVR) are excellent. Thus, their respective cost is a relevant clinical outcome. We performed a retrospective cohort study of children and adults who underwent PVR at age ≥ 8 years from January 1, 2011, to December 31, 2013, at 35 centers contributing data to the Pediatric Health Information Systems database to address this question. A propensity score-adjusted multivariable analysis was performed to adjust for known confounders. Secondary analyses of department-level charges, risk of re

The importance of the pharmacist's expanding role on the diabetes team: reinforcing nutritional guidelines for improved glycemic control.

The role of the pharmacist on the diabetes care team is expanding due to the increasing number of patients diagnosed with diabetes, limited health care dollars, and the education related to and required for patients by managed care organizations and insurance companies. In the past, training of patients in diabetes self-management skills has been inadequate, and this continues to be the case. Clinical pharmacists, in cooperation with physicians, have increased opportunities to provide education about medications and may include instructions for patients regarding the interaction of food consum

A stewardship framework for incretin-based therapies in weight management.

BACKGROUND: Use of incretin-based therapies, including GLP-1 and GIP receptor agonists, for weight management and related cardio-kidney-liver-metabolic comorbidities has expanded rapidly, yet real-world practice is challenged by inconsistent prescribing, access barriers, limited long-term safety data, low treatment persistence, and concerns regarding equity and financial sustainability. These gaps highlight the need for a structured, coordinated stewardship model to guide safe and evidence-based use of incretin-based therapies. OBJECTIVES: To develop a scalable, multidisciplinary stewardship

Automated outreach to increase primary adherence to cholesterol-lowering medications.

BACKGROUND: Primary nonadherence occurs when new prescriptions are not dispensed. Little is known about how to reduce primary nonadherence. We performed a randomized controlled trial to evaluate an automated system to decrease primary nonadherence to statins for lowering cholesterol. METHODS: Adult members of Kaiser Permanente Southern California with no history of statin use within the past year who did not fill a statin prescription after 1 to 2 weeks were passively enrolled. The intervention group received automated telephone calls followed 1 week later by letters for continued nonadherenc
