Effect of PON1 Q192R genetic polymorphism on clopidogrel efficacy and cardiovascular events in the Clopidogrel in the Unstable Angina to Prevent Recurrent Events trial and the Atrial Fibrillation Clopidogrel Trial with Irbesartan for Prevention of Vascular Events.

BACKGROUND: A recent report suggested that carriers of the Q allele of the PON1 Q192R polymorphism had decreased biotransformation of clopidogrel into its active metabolite and decreased efficacy of clopidogrel in preventing cardiovascular events. Furthermore, PON1 has been reported to have a central role in the antioxidant function of high-density lipoprotein, and the Q192R polymorphism has been previously associated with cardiovascular risk in patients not treated with clopidogrel. METHODS AND RESULTS: Patients (n=5059) from the Clopidogrel in Unstable Angina to Prevent Recurrent Events (CU

Safety and Efficacy of a Direct Oral Anticoagulant and Clopidogrel Versus Aspirin and Clopidogrel in Carotid Artery Stenting: A Propensity-Scored Analysis.

BACKGROUND AND OBJECTIVES: Dual antiplatelet therapy is the gold standard regimen after carotid artery stenting (CAS). However, optimal regimens in patients undergoing this procedure with preexisting conditions requiring anticoagulation remain largely unknown. Here, we compare the safety and efficacy of combining a direct oral anticoagulant (DOAC) with clopidogrel vs aspirin with clopidogrel in patients undergoing CAS. METHODS: We conducted a retrospective cohort comparison study of 469 consecutive patients with carotid artery stenosis who underwent CAS and were prescribed either a DOAC plus

A Case of Hypoglycemia With Concomitant Use of a Sulfonylurea and Clopidogrel.

Hypoglycemia secondary to sulfonylureas and clopidogrel have been independently described in the literature. However, there has been minimal investigation into the risk of clopidogrel-induced hypoglycemia in the setting of long-term or concomitant sulfonylurea use in patients with Type 2 diabetes mellitus. We present a case of a patient with diabetes well managed on glimepiride (second-generation sulfonylurea) for more than 10 years who presented with an episode of hypoglycemia shortly after initiation of clopidogrel for peripheral vascular disease.

Association of lipoprotein(a) with intrinsic and on-clopidogrel platelet reactivity.

Lipoprotein(a) [Lp(a)] is an independent, genetically determined, and causal risk factor for cardiovascular disease. Laboratory data have suggested an interaction of Lp(a) with platelet function, potentially caused by its interaction with platelet receptors. So far, the potential association of Lp(a) with platelet activation and reactivity has not been proven in larger clinical cohorts. This study analyzed intrinsic platelet reactivity before loading with clopidogrel 600 mg and on-treatment platelet reactivity tested 24 h following loading in patients undergoing elective coronary angiography.

Integrative Medicine on Optimizing Clopidogrel and Aspirin Therapy.

This article reviews the available published data on optimizing clopidogrel and aspirin therapy using translational and integrative medicine. Translational and evidence-based medical studies show that the CYP2C19 gene mutation (CYP2C19*2 and CYP2C19*3) could affect > 50% of the Chinese population, and that this mutation is closely associated with clopidogrel resistance and an increased risk of major adverse cardiovascular events, particularly stent thrombosis in patients following percutaneous coronary intervention (PCI). Adjusted-dose warfarin and aspirin reduce stroke in patients with atrial

Effects of aspirin and clopidogrel on neural stem cells.

Cerebral infarction causes severe morbidity and mortality. Most patients with cerebral infarction should take antiplatelet drugs daily, so the effects of those drugs on the regeneration of the brain need to be investigated. Aspirin and clopidogrel are the most widely used antiplatelet drugs for the prevention of ischemic stroke. We investigated the effects of aspirin and clopidogrel on neural stem cells (NSCs). NSCs were dissociated from fetal rat cortex and cultured with basic fibroblast growth factor and N2 medium. To measure the effects of aspirin and clopidogrel on NSCs, NSCs were treated

Purpuric herpes zoster in patients in therapy with clopidogrel.

Clopidogrel is an adenosine diphosphate receptor antagonist used for the prevention of vascular events in patients with atherothrombotic diseases manifested by recent myocardial infarction, ischemic stroke or peripheral arterial disease. Diarrhoea, rash and pruritus are rather common side effects of clopidogrel. Other side effects include epistaxis, nausea, abdominal pain, vomiting, gastritis, gastric and duodenal ulcer. Thrombocytopenia is the most common laboratory abnormality. Leucopenia and neutropenia are rare. We report three cases of purpuric herpes zoster in patients in therapy with cl

Genetic and nongenetic factors influencing the response to clopidogrel.

The antiplatelet drug clopidogrel is a commonly prescribed therapy in patients with acute coronary syndrome. However, its clinical efficacy is hampered by a wide inter-patient response variability, with over 30% of patients treated with this drug experiencing an inadequate antiplatelet response. There are growing evidences that clopidogrel response variability is associated with cytochrome P450 (CYP) enzyme genetic polymorphisms, primarily CYP2C19 which is responsible for the conversion of clopidogrel into its active metabolite. All of the CYP2C19 polymorphism data suggest that carriers of all

A patient with HIV and tuberculosis with diminished clopidogrel response.

Patients with HIV are at an increased risk for cardiovascular disease, both as a result of treatment with protease inhibitors and from the disease itself. The medication regimens of patients with HIV and cardiovascular comorbidities are complex and require careful assessment for potentially serious drug-drug interactions. We report a case of clopidogrel non-responsiveness in a patient with HIV, latent tuberculosis and cardiovascular disease with a history of myocardial infarction. To our knowledge, this is the first report of significant drug interactions between clopidogrel, isoniazid and rit

Reversal of the anti-platelet effects of aspirin and clopidogrel.

BACKGROUND: Guidelines recommend stopping aspirin and clopidogrel 7 to 10 days before surgery to allow time for replacement of permanently inhibited platelets by newly released uninhibited platelets. OBJECTIVES: The purpose of the present study was to determine the rate of offset of the anti-platelet effects of aspirin and clopidogrel after stopping treatment and the proportion of untreated donor platelets that are required to reverse their anti-platelet effects. METHODS: Cohort 1 consisted of 15 healthy subjects who received aspirin 81 mg day(-1) or clopidogrel 75 mg day(-1) for 7 days and

Prediction of clopidogrel efficacy and safety: phenotyping and/or genotyping?

In patients undergoing percutaneous coronary intervention the role of platelet function assessment (phenotyping) while on-clopidogrel and/or genotyping for outcome prediction is being intensively investigated. The study under evaluation reported the diagnostic accuracy of phenotyping versus genotyping for prediction of ischemic and bleeding events in 416 such patients during 1-year follow-up. Their data shows that the phenotyping of platelet response to clopidogrel by multiple electrode aggregometry was a better predictor of stent thrombosis than other platelet function assays used and even ge

Cytochrome p-450 polymorphisms and response to clopidogrel.

BACKGROUND: Clopidogrel requires transformation into an active metabolite by cytochrome P-450 (CYP) enzymes for its antiplatelet effect. The genes encoding CYP enzymes are polymorphic, with common alleles conferring reduced function. METHODS: We tested the association between functional genetic variants in CYP genes, plasma concentrations of active drug metabolite, and platelet inhibition in response to clopidogrel in 162 healthy subjects. We then examined the association between these genetic variants and cardiovascular outcomes in a separate cohort of 1477 subjects with acute coronary syndr

The use of clopidogrel in patients with coronary artery disease.

Coronary thrombosis plays a central role in the development, progression, and complications of atherosclerotic heart disease. As a result, pharmacologic manipulation of the hemostatic system has been the mainstay of treatment for coronary artery disease. Since platelets are the most important cellular element in the development of arterial thrombosis, many of the most effective therapies have involved the use of various antiplatelet agents. This article focuses on clopidogrel, an antiplatelet agent belonging to the class of drugs known as the thienopyridines, in the treatment of patients with

The role of clopidogrel in the management of acute coronary syndromes.

BACKGROUND: Despite significant advances in the management of coronary heart disease, myocardial infarction (MI) is still associated with a mortality rate of 45%. Acetylsalicylic acid (ASA) has been the oral antiplatelet drug of choice until recently. Thienopyridines such as clopidogrel have been shown to provide significant benefits in the management of acute coronary syndromes (ACS), either as an alternative to or in combination with ASA therapy. OBJECTIVE: The purpose of this article was to review the available scientific literature evaluating the use of clopidogrel in the management of AC
