Postpartum coccydynia: a case series study of 57 women.

BACKGROUND: Although childbirth is a well-known cause of coccydynia, this condition has not been studied previously. AIM: To explore the characteristics of postpartum coccydynia and identify risk factors. DESIGN: A case series study. SETTING: A specialist coccydynia clinic in a department of physical medicine in a university hospital. METHODS: A series of 57 women suffering from postpartum coccydynia was analyzed and compared with a control group of 192 women suffering from coccydynia due to other causes. Dynamic radiography enabled a comparison of the coccygeal mobility in the two groups.

[Chronic coccygodynia].

CHRONIC COCCYGODYNIA. Chronic coccygodynia is defined as pain in the c occyx which has been present for more than six months and which is aggravated by sitting down and/or rising. Although the diagnosis is primarily clinical, additional imaging examinations (MRI, even scintigraphy) are necessary to look for symptomatic forms of curable pathologies such as tumors in the region. These investigations can also help to elucidate the pathophysiological mechanisms responsible for the pain. Dynamic X-rays (sitting and standing) of the coccyx remain the reference exam. Coccygeal ultrasound with a dynam

The influence of coccygeal dynamic patterns on ganglion impar block treatment results in chronic coccygodynia.

Objective Ganglion impar block (GIB) can be performed in patients with chronic coccygodynia who do not respond to conservative treatments. We investigated the effect of coccygeal dynamic patterns on the treatment outcome in patients with chronic coccygodynia treated with GIB. Materials and methods We retrospectively analyzed the data for patients diagnosed with chronic coccygodynia who underwent GIB only once by a transsacrococcygeal method under fluoroscopy guidance in our Pain Medicine Clinic. Patients were assessed with standard and dynamic coccyx radiographs and classified according to coc

Squamous cell carcinoma over pilonidal chronic disease. A new therapeutic approach.

INTRODUCTION: Pilonidal sinus is a very common disease. Malignant transformation occurs in 0,1% of patients. We present a case of squamous cell carcinoma arised from recurrent pilonidal disease, managed by multimodal treatment. PRESENTATION OF CASE: We present a 70-year-old man with chronic pilonidal sinus. Inflammation had worsened in previous months and exploration revealed a large ulcerative mass which biopsy showed a squamous cell carcinoma. CT scan and MRI imaging showed tumoral invasion of the coccyx and both gluteus major muscles. Neoadjuvant radiotherapy, chemotherapy as radiosensitiz

Anatomical reevaluation of the anococcygeal ligament and its surgical relevance.

BACKGROUND: With the development and spread of surgical procedures for total mesorectal excision and intersphincteric resection, rectal surgeons have gained frequent opportunities to observe connective tissues around the anal canal. However, uncertainty remains as to the exact identity and location of these structures. OBJECTIVE: The aim of this study was therefore to identify and describe the morphology of connective tissue structures extending between the coccyx and anal canal. DESIGN AND SETTING: This was a descriptive study carried out at university facilities for anatomic research. The

The relationship between the perineal length measurements and the urodynamic stress incontinence.

OBJECTIVE: The purpose of this study was to assess the relationship between the length of perineal body, the anal position index, the total fourchette-coccyx distance and the anal-coccyx length and female urodynamic stress urinary incontinence (USUI). PATIENTS AND METHODS: The study included 57 women with USUI (group A) and 45 women without USUI as control (group B). All patients underwent a urodynamic evaluation and measurement of the length of perineal body (PB) and the distance between the fourchette and the inferior margin of coccyx (FC). PB is the distance between the fourchette and the

Iatrogenic rectal perforation after coccygeal radiofrequency ablation.

Coccygodynia (pain of the coccygeal bone) can be treated locally with anti-inflammatory drugs, local steroid injections, surgical removal of the coccyx and, more recently, with radiofrequency thermal ablation. Complications, such as perforation of the colon, can occur as a consequence of the close relationship between the rectum and the sacrococcyx and with the heat from the thermal ablation expanding to the surrounding tissue causing delayed damage with severe consequences. The treatment of this complication requires the combined effort of the gastrointestinal surgeon as well as a gastroenter

The Influence of Coccygeal Dynamic Patterns in Ganglion Impar Block for the Treatment of Chronic Refractory Coccydynia.

INTRODUCTION: Patients with persistent coccygodynia who do not respond to conventional therapies may undergo ganglion impar block (GIB). We looked at how GIB therapy for individuals with persistent coccygodynia affected their coccygeal dynamic patterns. MATERIALS AND METHODS: Patients with persistent coccydynia with GIB once using a trans-Sacro-coccygeal technique with fluoroscopy supervision were taken up for the study. Out of 14 patients, they were categorized as Group I- mobile coccyx (8 patients) and Group 2- immobile coccyx (6 patients) using static and dynamic coccyx radiographs, based

COCCYGODYNIA.

The coccyx, the last segment of the spine, joins the sacrum at the base. It has three to five vertebrae, which are typically fused. In front of the sacrococcygeal junction is the ganglion impar, the only unpaired autonomic ganglion. The two sympathetic chains come to a close there. The impar ganglion is traversed by sympathetic nerves carrying pain fibers from the perineum, distal sections of the rectum, the vagina and the urethra. The painful disorder known as coccygodynia, which affects the tail end of the spine, is frequently brought on by birth trauma or is caused by an unidentified factor

Redefining the coccygeal plexus.

The coccygeal plexus is variably described in anatomy texts and has rarely been studied despite the idiopathic nature of coccydynia in up to one-third of affected patients. The plexus was therefore investigated using a combination of microdissection and histology. The distal sacrum and coccyx in continuity with ischiococcygeus were removed en bloc from 16 embalmed cadavers (mean age 78 ± 10 years, 7 females) with no local disease. Ten specimens underwent microdissection of the coccygeal plexus and the remaining six were examined histologically after staining with hematoxylin and eosin and S100

[Coccygodynia--a diagnostic and therapeutic problem in orthopedics].

Coccygodynia means a lot of complaints with the most important of them, namely the spontaneous pain in the coccyx, in the top of the coccyx respectively. One can differ posttraumatic coccygodynias from others, which are the symptom of other diseases like lumbar pain, sciatica, tumors in the spinal canal, visceral diseases, that means disturbances in the gynaecologic system. By the mean of 19 predominantly female patients remarkable facts of the history, of the x-ray and clinical findings are listed up and compared in three different groups. Automatic identification of lumbar level with ultrasound.

An ultrasound-based system is created to label the lumbar vertebrae for the purpose of percutaneous needle insertion. Many lumbar punctures have a preferable vertebral level for needle insertion, but the traditional method of manual palpation is known to be inaccurate for determining the level. Needle insertion for epidural anesthesia in obstetrics is preferably performed at the L3-L4 interspace and miscalculation can lead to complications such as nerve damage and paralysis. Similar risks occur for other spinal needle insertions. In this paper, an ultrasound-based system is devised that create

Coccydynia relieved by a tarsal tunnel block: a case series.

BACKGROUND: This case series describes, for the first time, to the author's knowledge, a novel treatment for coccydynia. Tarsal tunnel block with lignocaine only brought relief of chronic coccydynia lasting more than 6 months in three patients. The author adopts the theory that the myelin sheath of the posterior tibial nerve will convey the lipid-soluble lignocaine upward toward the dorsal root ganglia and the nerve roots of the lumbar spine through the uninterrupted myelin sheath, which is itself mainly formed of lipids. The author thinks that most coccyx pain is actually a radiating pain fro

A new approach for pudendal nerve exposure and its clinical significance.

Analgesia of the pudendal nerve is used in patients undergoing gynecologic, obstetric and penile surgery. Stimulation techniques are used to determine the functions of the nerve. In these interventions, different landmarks are used to describe the localization of this nerve. Although some authors use perineal approaches, others prefer a transvaginal (rectal in males) approach. Knowledge regarding the anatomy of the pudendal nerve and its blockade allows an easy approach to the clinical problems like perineal neuralgia, neurogenic bladder dysfunction and penile implant operations. However, in t

Dynamic MRI of the pelvic floor muscles in an upright sitting position.

Since the pelvic floor muscles are situated inside the pelvis, the actual function is difficult to observe. Magnetic resonance imaging (MRI) is a new method that may prove to give additional information about pelvic floor muscle function during contraction. The purpose of the present study was to assess pelvic floor muscle function during contraction and straining in an upright sitting position by use of MRI. Sixteen women, nine continent and seven with urodynamically and clinically proven genuine stress incontinence participated in the study. MRI was performed in an open-configured GE Signa S

A Review of Current Treatment Options for Coccygodynia.

PURPOSE OF REVIEW: Coccygodynia is pain within the coccyx area. The diagnosis is made clinically with symptoms of pain in the coccyx region and worsening pain in sitting position. The initial treatment is conservative therapy. For patients who do not respond to conservative therapies, there are further interventions available. This includes local injection of local anesthetics and steroids, neurolysis of sacral nerve roots, caudal epidural block, pulse radiofrequency (PRF), intra-rectal massage and manipulation, ganglion impar block, levator ani massage and stretching, coccyx manipulation, and

Use of coccygeal vertebra autograft and platelet-rich plasma for treating a distal radial nonunion fracture in a small-breed dog.

A 4.3-kg, 11-month-old, spayed female poodle was presented for treatment of a right radio-ulnar nonunion fracture. Clinical history included failed surgical correction of a radius fracture with internal fixation 1 mo before presentation. Radiographic examination revealed a 1.5-cm lytic lesion on the right distal radius. Surgery was planned with a coccyx autograft and platelet-rich plasma. A 2.8-cm-long bone defect was created, and the lytic lesion was removed. Caudectomy was performed; the 6th and 7th coccygeal bones were harvested, placed into the defect, and fixed to the radius with a lockin

Laparoscopic Approach to Presacral Recurrence of Sacrococcygeal Teratoma.

A 6-month-old girl was presented to our hospital due to a presacral mass found 5 months after surgery of sacrococcygeal teratoma. The original tumor was a 63 x 50 mm sized round cyst connecting to the coccyx, observed with computed tomography. The initial operation was performed with en bloc removal of the tumor along with the coccyx in the prone position. During a routine follow up, ultrasonography indicated a possible local recurrence, 5 months after the initial operation. The magnetic resonance imaging revealed a polycystic formation with a diameter of 20 x 11 x 17 mm in the presacral space
