Relationship Between the Cochlear Aqueduct and Internal Auditory Canal: Surgical Implications for Transcanal Transpromontorial Approaches to the Lateral Skull Base.

HYPOTHESIS: The cochlear aqueduct (CA) is subject to considerable anatomical variability. We hypothesize a topographical relationship between the CA and the internal auditory canal (IAC). BACKGROUND: The CA represents the lower limit of dissection during transcanal transpromontorial approaches to the lateral skull base due to its close relationship to the lower cranial nerves and jugular vein. METHODS: Three-dimensional models from high-resolution computed tomography scans of normal human temporal bones were created using threshold-based segmentation. The CA was classified into four categori

Histopathological study of the cochlea with altered perilymph metabolism.

In 41 guinea pigs an experiment was conducted to determine the effect of kanamycin on ears with or without blockage of the cochlear aqueduct. In 24 animals the cochlear aqueduct was blocked on one side, and after 1 to 21 days, kanamycin (300 mg/kg) was given daily and subcutaneously until the pinna reflex began to decrease. The remaining animals were used for controls: exposure of and trauma to the aperture of the cochlear aqueduct and kanamycin administration (8), blockage of the cochlear aqueduct only (3 plus 17 previous cases), and kanamycin administration to normal animals (6). The surviva

[Deafness caused by malformation of the inner ear. Current contribution of x-ray computed tomography].

Hearing loss due to malformations of the auditory system may involve morphologic abnormalities of the external canal, the middle ear, or the inner ear. Various combined malformations are possible. Audiologic assessment and computed tomography make it possible to perform a precise diagnostic evaluation of inner ear malformations. In a series of 71 children with congenital sensorineural hearing loss, 31 had computed tomographic evidence of malformations. Since 19 children had bilateral lesions, a total of 50 ears were studied. The cochleovestibular abnormalities were typed and correlated to the

[Clinical review of inner ear malformation].

We had 126 patients with inner ear malformation diagnosed with temporal bone computed tomography (CT) scans at Azabu Triology Hospital between 1996 and 2002. We classified cases of inner ear malformation according to Jackler et al. The incidence of inner ear malformation in our series was as follows; 1. Labyrynthine anomalies 61% (isolated lateral semicircular canal dysplasia 56%, compound semicircular canal dysplasia 4%, semicircular canal aplasia 1%), 2. Cochlear anomalies 24%, 3. Enlargement of the vestibular aqueduct 12%, 4. Narrow internal auditory canal 2%, 5. Complete labyrinthine aplas

Cochlear and cerebrospinal fluid pressure: their inter-relationship and control mechanisms.

The patency of the cochlear aqueduct is a key factor in intra-cochlear hydromechanics. If patent, the cerebrospinal fluid (CSF) provides the reference pressure for the perilymph and also to a large extent the endolymph, since Reissner's membrane can only withstand a relatively small pressure differential. The aqueduct often becomes sealed as a natural process of ageing. In this instance the reference pressure is from a source, its position unknown, within the boundaries of the cochlea itself. Relatively large and rapid changes in the cerebrospinal fluid pressure may result from everyday events

[Inner ear and subarachnoid spaces: relations and diseases].

The purpose of this article is to describe, with 5 clinical cases, the physiological communications between the inner ear and the subarachnoid spaces (SAS) and present the imaging features with regard to. Therefore we briefly illustrate abnormal communications between SAS and perilymphatic fluids in certain cochlear and internal acoustic meatus (IAM) malformations and their consequences. Imaging features may depict diffusion pathway of bacterial meningitis to membranous labyrinth via the cochlear aqueduct or via the IAM. Rarely, in some patients referred for cochleovestibular symptoms, imaging

An overview of congenital/developmental sensorineural hearing loss with emphasis on the vestibular aqueduct syndrome.

The evaluation of children with congenital/developmental hearing loss has undergone substantial transformation in recent years. This communication focuses on the most common cause of this type of hearing loss for which there are imaging findings: the vestibular aqueduct syndrome. The normal anatomy of the vestibular aqueduct is discussed in detail. The cochlear aqueduct with which it is commonly confused is also discussed. An overview of other congenital defects and abnormal middle/inner ear communications is also included. A brief discussion of cochlear implants is also included for completen

Transmission of complex pressure waves through the perilymphatic fluid in cats.

The response of the perilymphatic fluid to complex pressure waves, composed of low-frequency sine waves superimposed on square-wave pressure pulses of varying amplitude was studied. In cats with a patent cochlear aqueduct a pronounced positive pressure change could be induced in the perilymphatic fluid when complex pressure waves were used. The time constants associated with the stabilization of the perilymphatic pressure after the application of pressure complexes were longer than those associated with square-wave pulses alone. The results indicate that the functional patency of the cochlear

Temporal bone histopathology related to cochlear implantation in congenital malformation of the bony cochlea.

HYPOTHESIS: Histopathologic findings in temporal bones with congenital malformations of the bony cochlea may provide insight into cochlear implantation planning, surgical approach, and complications. BACKGROUND: Patients with congenitally malformed cochleae account for an increasing percentage of candidates for cochlear implantation. Few studies on the relationship between histopathologic findings of temporal bones with malformation of the bony cochlea and cochlear implantation have been reported. METHODS: We studied 21 temporal bones from 12 cases with congenital malformations of the bony c

[Enlarged vestibular aqueduct syndrome in patients with hypoacusis and deafness].

UNLABELLED: In 1978 Valvassori and Clemis have reported that enlarged vestibular aqueduct can cause progressive perceptive hearing loss and finally deafness. AIM: Analysis of CT scans of patients with cochlear implants. Analysis of radiological data of 120 implanted patients was performed. CT examinations were performed during qualification to cochlear implantation. Most of the CT scans were performed using high resolution computed tomography (HRCT). A special attention was paid to patients with progressive perceptive hearing loss and CT of two patients with "gusher" during CI surgery. In one

Prevalence of Cochlear-Facial and Other Non-Superior Semicircular Canal Third Window Dehiscence on High-Resolution Temporal Bone CT.

BACKGROUND AND PURPOSE: The radiologic prevalence of superior semicircular canal dehiscence in the asymptomatic population has been widely studied, but less is known about the rates of other forms of third window dehiscence. Per the existing literature, the radiologic prevalence of cochlear-facial nerve dehiscence, for example, exceeds that seen in histologic studies, suggesting that conventional CT is unreliable for cochlear-facial dehiscence. These studies relied on nonisometric CT acquisitions, however, and underused multiplanar reformatting techniques, leading to false-positive findings. O

Misdiagnosis of otosclerosis in a patient with enlarged vestibular aqueduct syndrome: a case report.

INTRODUCTION: In the present case we report on the mismanagement of a patient misdiagnosed with otosclerosis, who was subsequently found to have enlarged vestibular aqueduct syndrome bilaterally. This highlights the need to not only be vigilant in pre-operative assessment of otosclerosis but also in post-operative investigations of stapedectomy failures. CASE PRESENTATION: Our patient, a 56-year-old Caucasian Australian woman, lost the hearing in her right ear following a stapedectomy approximately 25 years ago. It is thought that preoperative imaging was not conducted, while an inadequate (u

Perilymphatic pressure dynamics following posture change in patients with Menière's disease and in normal hearing subjects.

The hydrostatic pressure of the inner ear depends on the cerebrospinal fluid pressure through the cochlear aqueduct. The time-course of inner ear pressure change following rapid change in cerebrospinal fluid pressure is related to the aqueduct patency. In this study the patency of the cochlear aqueduct in 27 patients with Menière's disease (28 affected ears, 16 non-affected ears) and in 12 normal hearing subjects (18 control ears) was assessed non-invasively by means of the MMS-10 Tympanic Displacement Analyser. Following a rapid change in body position, changes in intracranial cerebrospinal f

The petromastoid canal on computed tomography.

The objective was to assess visibility and anatomy of the petromastoid canal in high-resolution CT. Computed tomography images of 188 patients were reviewed for delineation of the petromastoid canal. This bony canal connects the mastoid antrum with the cranial cavity and houses the subarcuate artery and vein. The diameter, obtained in the middle portion of the canal, was compared with the diameter of the vestibular and cochlear aqueduct in all patients, and absolute values measured in 20 cases. Collimation was 1 mm in 164 and 2 mm in 24 examinations. Additionally, temporal bone of a cadaver wa

Contralateral deafness following unilateral suboccipital brain tumor surgery in a patient with large vestibular aqueduct--case report.

A 68-year-old female developed contralateral deafness following extirpation of a left cerebellopontine angle epidermoid cyst. Computed tomography showed that large vestibular aqueduct was present. This unusual complication may have been caused by an abrupt pressure change after cerebrospinal fluid release, which was transmitted through the large vestibular aqueduct and resulted in cochlear damage.
