Tympanic electrocochleography for diagnosis of Menière's disease.

OBJECTIVE: Tympanic electrocochleography (ECOG) is a noninvasive technique for recording cochlear potentials with an electrode placed on the tympanic membrane. Electrocochleography is used clinically in the evaluation of patients for endolymphatic hydrops. This study was undertaken to obtain normative data for ECOG responses to clicks and tone bursts and to determine clinical indicators that result in high test specificities. DESIGN: Three cochlear potentials were measured; the eighth nerve compound action potential, the cochlear microphonic, and the summating potential. The subjects were 53

Cochlear activation at low sound intensities by a fluid pathway.

In order to assess the mechanisms responsible for cochlear activation at low sound intensities, a semi-circular canal was fenestrated in fat sand rats, and in other experiments a hole was made in the bone over the scala vestibuli of the first turn of the guinea-pig cochlea. Such holes, which expose the cochlear fluids to air, provide a sound pathway out of the cochlea which is of lower impedance than that through the round window. This should attenuate the pressure difference across the cochlear partition and thereby reduce the driving force for the base-to-apex traveling wave along the basila

Audibility, speech perception and processing of temporal cues in ribbon synaptic disorders due to OTOF mutations.

Mutations in the OTOF gene encoding otoferlin result in a disrupted function of the ribbon synapses with impairment of the multivesicular glutamate release. Most affected subjects present with congenital hearing loss and abnormal auditory brainstem potentials associated with preserved cochlear hair cell activities (otoacoustic emissions, cochlear microphonics [CMs]). Transtympanic electrocochleography (ECochG) has recently been proposed for defining the details of potentials arising in both the cochlea and auditory nerve in this disorder, and with a view to shedding light on the pathophysiolog

Effects of furosemide on distortion product otoacoustic emissions and on neuronal responses in the anteroventral cochlear nucleus.

1. The objective of this study was to precisely evaluate the relationship between the threshold of neurons in the anteroventral cochlear nucleus (AVCN) and the properties of distortion product otoacoustic emissions (DPOAEs). Response areas of multiunit clusters in the AVCN and DPOAEs in the ear canal were measured alternately in the adult gerbil during furosemide-induced changes of the endocochlear potential. Stimulus frequencies of the probe tones for DPOAE measurement were in the range of f1 = 1.7-7.6 kHz and f2 = 2.0-9.0 kHz; the ratio f2:f1 was always 1.18. Stimulus amplitudes were varied

Clinical usefulness of electrocochleography in Meniere's disease: an analysis of dehydrating agents.

Both electrocochleography and glycerol dehydration have an important diagnostic and prognostic role in the management of patients with endolymphatic hydrops, whether idiopathic as in Meniere's disease or secondary to certain diseases of the otic capsule. Abnormal electrocochleographic responses in a "normal" ear can be used to predict when Meniere's disease will become bilateral. There is a significant correlation between the effects of glycerol and of endolymphatic sac surgery on the pars inferior, at least over the span of several years. The subject hearing gains, the objective improvements

Cochlea and auditory nerve.

The transduction process in the cochlea requires patent hair cells. Population responses that reflect this patency are the cochlear microphonic (CM) and summating potential (SP). They can be measured using electrocochleography (ECochG). The CM reflects the sound waveform in the form of outer hair cell (OHC) depolarization and hyperpolarization, and the SP reflects the average voltage difference of the OHC membrane potential for depolarization and hyperpolarization. The CM can be measured using ECochG or via the so-called otoacoustic emissions, using a sensitive microphone in the ear canal. Neu

Reverse correlation study of cochlear filtering in normal and pathological guinea pig ears.

The filtering properties of single cochlear fibres have been determined in normal and kanamycin-treated guinea pigs using the reverse correlation technique. This method allows investigation of filtering over a wide dynamic range. For normal guinea pig fibres, the near threshold filter functions obtained with this method correspond to the tone derived frequency threshold curves ((FTCs). The 10 dB bandwidth of the filter functions increased monotonically with increasing noise levels above threshold. Thus with noise levels at approximately 50 dB above threshold, the 10 dB bandwidth had increased

Tympanic electrocochleography for evaluation of endolymphatic hydrops.

There has been increased interest in electrocochleography for the diagnosis and intraoperative monitoring of patients with endolymphatic hydrops. Attention has been focused on the determination of the summating potential:action potential (SP:AP) ratio from alternating polarity clicks. Review of this technique at the University of Minnesota led to a reevaluation of the usual interpretation of these recordings. Separate examination of the rarefaction and condensation click recordings provides insight into abnormal response patterns. Cases are presented to illustrate a variety of normal and abnor

The UmboMic: Characterization and Testing of a Middle Ear Microphone.

PURPOSE: We report developments on the "UmboMic," a piezoelectric microphone for fully implanted cochlear implants. Internal implantation of the microphone for fully implantable cochlear implants is the goal of future technology, as it has the potential to significantly improve the device performance and user experience. METHODS: The UmboMic is designed for implantation in the middle ear cavity, where it detects the motion of the umbo via the piezoelectric effect. The UmboMic sensor is made from two layers of the piezoelectric material called polyvinylidene difluoride (PVDF). Steps towards bi

Intrinsic oscillations in spike trains indicate non-renewal statistics due to convergence of inputs in dorsal cochlear nucleus neurons.

The occurrence of intrinsic oscillations (IOs) in a unit's discharge is reflected by a prominent peak in the power spectrum (i.e., Fourier transform of the autocorrelation function) of spike trains obtained from single-unit discharge, at a frequency independent of stimulus spectral characteristics. IOs have been reported by researchers in the dorsal cochlear nucleus (DCN) of both the cat and the Mongolian gerbil. It has been hypothesized that IOs are related to inter-spike interval (ISI) regularity (e.g., [Hear. Res. 58 (1992) 153]). This hypothesis is tested in this paper. Responses to multip

Glycerol dehydration tests in Ménière's disorder using extratympanic electrocochleography.

Eleven patients with classical Ménière's disorder were examined electrophysiologically using extratympanic electrocochleography. All had been fully investigated audiologically and by electronystagmography. In addition to the classical triad of symptoms, 7 complained also of fullness or blockage in the ear, this symptoms being associated with fluctuation of the deafness. All patients showed the characteristic enhancement of the SP component of the AP/SP waveform. Following glycerol the SP component was reduced in amplitude, producing a decrease in the width of the waveform. Maximum reduction oc

The effect of injection of high K+ solution into scala media.

Thirty guinea pig ears were studied to investigate the effect of endolymphatic hydrops on the cochlea. High K+ solution was injected into the scala media, and cochlear microphonics (CM) and endocochlear potential (EP) were observed before, during and after the injection. The CM amplitude decreased rapidly after injection, ending in a depressed plateau value. By contrast, EP remained almost unchanged. By changing the composition of the solution it was suggested that the effect of the injection was mechanical one, rather than biochemical. In three ears, spontaneous recovery of CM was observed du

[Evoked acoustic oto-emissions: effects of atropine].

The Evoked Otoacoustic Emissions (EOE) are a sinusoidal wave complex coming from the contractile properties of the cochlea and particularly of the outer hair cells. This activity is influenced by the centrifugal pathways whose transmitter is acetylcholine. In order to study the effects of the functional block of the cochlea, ten young normal subjects were examined recording the EOE before and ten mins after the i.v. infusion of 1 mg of atropine. The statistical analysis of the results has shown a significant decrease only in EOE appearance threshold in absence of modifications of the saturatio

Single-unit response at the round window of the guinea pig.

In guinea pigs the unit contribution (unit action potential, ap) to the response of the round window was computed following the method used by Kiang and co-workers (1976), i.e., fibre discharges registered by a microelectrode in the nerve were used as trigger pulses for the averaging process of the corresponding ap registered with a gross electrode at the round window. Normally the ap was independent of the fibre-CF, had a diphasic waveform, and its amplitude was about 0.1 microV. Small inter-animal differences were found in waveform and amplitude of the ap. In a pilot experiment exploring pat

Prolongation of the cochlear microphonic in man. Cochlear microphonic ringing.

During routine extra-tympanic electrocochleographic (ECochG) recording we have observed that in many cases more cycles are seen in the cochlear microphonic (CM) recording than are present in the acoustic stimulus. We have termed this phenomenon CM 'ringing'. Ringing is present in 69% of our normal subjects and in 43% of patients with deafness of different aetiologies. In the cochlear and Ménière subgroups those cases showing ringing have a significantly larger CM amplitude (p less than 0.001 and p less than 0.01 respectively). The possible origin of this phenomenon is discussed.

TRPA1 deficiency affects cochlear potentials days after noise exposure.

TRPA1 channels are master sensors of tissue damage. We recently showed that, after acoustic trauma, TRPA1 activation in cochlear supporting cells regulates hearing sensitivity and is a component of the temporary threshold shift. In the neonate organ of Corti, TRPA1 activation in the Hensen's cells leads to prolonged calcium responses that propagate across the organ of Corti and cause long-lasting tissue displacements. In the adult cochlea, such tissue shape changes would be expected to affect the geometry and/or stiffness of the cochlear partition and, consequently, cochlear amplification. How

Basilar-membrane motion in the alligator lizard: its relation to tonotopic organization and frequency selectivity.

In the alligator lizard the entire basilar membrane is accessible for measurements of its velocity by the Mössbauer method. Tests of the method indicate (1) the Mössbauer source can be placed on the basilar membrane without altering the signal-transmission properties of the cochlea, and (2) the source adheres to the basilar membrane. Isovelocity curves (IVCs) were constructed by plotting (as a function of tone frequency) the sound-pressure level at the tympanic membrane required to produce a specified velocity amplitude. IVCs from 21 lizards for source locations spanning the length of the basi

Inner hair cell dysfunction in Klhl18 mutant mice leads to low frequency progressive hearing loss.

Age-related hearing loss in humans (presbycusis) typically involves impairment of high frequency sensitivity before becoming progressively more severe at lower frequencies. Pathologies initially affecting lower frequency regions of hearing are less common. Here we describe a progressive, predominantly low-frequency recessive hearing impairment in two mutant mouse lines carrying different mutant alleles of the Klhl18 gene: a spontaneous missense mutation (Klhl18lowf) and a targeted mutation (Klhl18tm1a(KOMP)Wtsi). Both males and females were studied, and the two mutant lines showed similar phen

Case of recurrent, reversible, sudden sensorineural hearing loss in a child.

This paper describes audiologic, electrophysiologic, and medical test results for a now 10-year-old girl who has had 45 episodes of reversible, sudden sensorineural hearing loss over the last 8 years. Episodes have lasted from 6 to 72 hours and often have been accompanied by a mild illness. Acoustic immittance measures have been consistent with normal middle-ear function with the exception of absent ipsilateral and contralateral acoustic reflexes. Mechanically evoked perioral reflex activity was markedly asymmetric following lower lip stimulation. The asymmetry of R1 activation between right a

Clinical implications of experiments on alteration of the labyrinthine fluid pressures.

Hearing is affected by changes in hydrostatic pressure of the perilymph, but very high pressures are required to produce a significant loss. Among the observed findings in cats are a rapidly reversible decrease in the sensitivity of the cochlear microphonic that is greater for low frequencies, harmonic distortion and reduction of dynamic range as measured by the input-output function for the cochlear microphonic, and a change in the summating potential from negative to positive. These changes are attributed to a temporary reduction in the size of the cochlear duct due to a shift of water molec
