Hereditary nonpolyposis colorectal cancer (HNPCC) is an autosomal dominant disease characterized by the early occurrence of colorectal cancer--usually located in the right colon--frequent synchronous and metachronous colorectal malignancies, and association with tumors of other organs (especially endometrium, skin and stomach). In typical cases the diagnosis may be easy, but in many other instances small size of families, possible low penetrance and frequency of phenocopies may render the identification of these families extremely more complex. To provide a basis of uniformity in assessing HNP

HNPCC is an autosomal dominantly inherited disorder with proclivity to early onset colorectal cancer in the absence of multiple polyps of the colon. There is a predilection for proximal colonic location (70 per cent) and an excess of synchronous and metachronous colorectal cancers. HNPCC is subdivided into Lynch syndrome I, which is restricted to site-specific colon cancer susceptibility, and Lynch syndrome II, which shows all of the features of Lynch syndrome I, but in addition, patients are at inordinately increased risk for carcinoma of the endometrium, ovary, and other anatomic sites. Of the estimated 150,000 colorectal cancer (CRC) cases diagnosed annually, approximately 30% have a familial basis and 3% to 5% are from high-penetrance inherited cancer syndromes. Lynch syndrome, or hereditary nonpolyposis colorectal cancer, caused by inherited germline mutations in mismatch repair (MMR) genes, is the most commonly inherited CRC syndrome. It is characterized by young-onset CRC and an increased risk for extracolonic tumors, including gynecologic, urinary tract, and other gastrointestinal cancers. Commercial testing is available for mutations in the MMR genes, but testing all p

The hereditary forms of colorectal cancer have been given many names historically as the manifestations have been gradually understood. Lynch syndrome has had several names, most prominently 'Hereditary Nonpolyposis Colorectal Cancer' or HNPCC. Clarification of the genetic basis and full phenotypic expression of this disease mandates a more clinically useful name that clarifies the consideration of non-colonic cancers in a family history, and unifies the diagnosis around the germline mutation in a DNA mismatch repair (MMR) gene. The term 'Lynch syndrome' is proposed for the autosomal dominant

BACKGROUND AND STUDY AIMS: The most important causes of hereditary colorectal cancer are Lynch syndrome (LS) and the adenomatous polyposis syndromes (familial adenomatous poly- posis syndrome or FAP, attenuated FAP or AFAP and MUTYH associated polyposis syndrome or MAP). The aim of this study was to investigate whether all patients with a hereditary syndrome within one center receive uniform advice regarding surveillance and treatment.
PATIENTS AND METHODS: A retrospective analysis was performed of all electronic patient health records of patients with LS, FAP, AFAP and MAP who received genet

BACKGROUND: Patients with a colorectal neoplasm are at risk for metachronous neoplasia. This risk usually is stratified according to the number, size, and histology of the index lesion(s). This study was performed to search for factors contributing not only to a very high risk of metachronous lesions but also to a very low risk.
METHODS: An extensive neoplasia follow-up database was used to identify patients who were neoplasia prone and neoplasia resistant. Groups were defined as having consecutive colonoscopies that were either all positive or all negative for adenoma(s). Subgroups with two,

Mutations affecting human mismatch repair (MMR) genes (MLH1, MSH2, PMS1, PMS2, and MSH6) cause tumour predisposition in hereditary nonpolyposis colorectal cancer (HNPCC) syndrome, and an association has been demonstrated with the replication error (RER) phenotype in most colorectal and some extracolonic neoplasms. A pathogenetic model for RER+ tumours through inactivation of suppressor genes has been hypothesised, and TGF beta RII, BAX and IGFIIR genes have recently been proposed as targets of such inactivating mutations. In this study, a series of 47 tumours developed in patients with known M

BACKGROUND: Tumor-to-tumor metastasis is a rare event. Rectal cancer to primary thyroid neoplasm metastasis is extremely rare. Herein, we reported a case of metastatic rectal adenocarcinoma to a papillary thyroid carcinoma. The incidence and clinicopathological characteristics of metastatic colorectal cancer to a thyroid gland neoplasm were described, and the pertinent literature was reviewed.
CASE PRESENTATION: A 34-year-old female patient had curative treatment of initial rectal adenocarcinoma in 2012, and was found to have lung metastases by follow-up CT scan 3 years later. In 2018, she wa

In the USA, colorectal cancer (CRC) is the 2nd leading cause of cancer-related deaths. Certain groups in the USA are at an increased risk of developing CRC, including those with a genetic risk and family history. The purpose of this project was to synthesize primary and secondary prevention interventions for individuals who are at high risk of CRC due to family history or genetic predisposition. This study systematically reviewed articles from PubMed, Google Scholar, and EBSCO using specific search terms to find relevant articles. Sixteen articles were identified for full-text review, which we

Hereditary Colorectal Cancer: Clinics, Diagnostics and Management Abstract. About five percent of all colorectal cancers are caused by highly penetrant, autosomal dominantly or recessively inherited tumour predispositions. Current molecular genetic diagnostics, making use of high-throughput sequencing technologies, allow a cost-efficient, prompt and comprehensive investigation. In contrast to sporadic colorectal cancer hereditary forms are often associated with an elevated lifetime risk for additional, extracolonic tumours which necessitate specific long-term interdisciplinary prevention measu

Lynch syndrome is the most common hereditary colorectal cancer syndrome, and colonoscopy remains the cornerstone of prevention. However, surveillance is limited by missed lesions, variable procedural quality, accelerated neoplastic progression, and the continued occurrence of post-colonoscopy colorectal cancers. This review examines gene-specific differences in colorectal cancer and adenoma risk, quality standards for surveillance colonoscopy, emerging artificial intelligence technologies, and complementary non-endoscopic strategies. Contemporary studies conducted in expert settings have repor

A 19-year-old male patient with abdominal pain and diarrhea was presented to our hospital. He was diagnosed with advanced colorectal cancer based on imaging. Lynch syndrome was suspected, considering his family history. After a laparoscopic subtotal colectomy, genetic testing confirmed a pathogenic variant in the MLH1 gene, leading to a definitive diagnosis of Lynch syndrome. The onset of colorectal cancer is extremely rare in the teenage years. Herein, we discuss the potential involvement of genetic anticipation, emphasize the importance of early surveillance in individuals at risk, and consi

BACKGROUND: Colorectal cancer (CRC) is the second cause of cancer death in the world and is estimated to have been responsible for almost 935,000 deaths during 2020.
OBJECTIVE: Describe clinicopathological features, overall survival (OS) and progression-free survival (PFS) in CRC patients under 30 years.
METHOD: This is a retrospective cohort study in patients under 30 years diagnosed with CRC.
RESULTS: From 2017 to 2021, 1823 patients were diagnosed with CRC, of which 54 (2.96%) were under 30 years. The OS, during 4 years, was 41.5%. The clinical stage found IV (hazard ratio [HR]: 6.212; 9

This paper is a review of work done on colorectal cancer in Nigeria over the last 40 years showing geographic spread, age and sex ratios, predominant histopathology and paucity of polyposis coli syndromes. The male/female ratio is averagely equal, the peak age remains around 44 years, there is a significant subgroup of the under-30s and there are more rectal cancer cases than colon cancer cases. Of the colon cancer cases, the caecum seems to be the more favoured site. There seems to be quite a significant incidence of mucin-secreting adenocarcinoma subgroups which are said to carry a worse pro

Approximately 25% of colorectal cancers occur in younger individuals or those with a personal or family history of the disease, suggesting a heritable susceptibility. The minority of these cases are accounted for by one of the well-described hereditary colorectal cancer syndromes, familial adenomatous polyposis (FAP) and hereditary nonpolyposis colorectal cancer (HNPCC). The recent identification and cloning of the genes responsible for FAP and HNPCC, along with other colon cancer susceptibility genes, has led to the wide-spread availability of genetic testing for hereditary colorectal cancer.

Lynch syndrome (LS), also known as hereditary non-polyposis colorectal cancer, is an inherited cancer syndrome which increases the risk of developing colorectal cancer and endometrial cancer. Individuals with LS have an increased risk of cancers of the ovary, urinary tract, stomach, small intestine, pancreas, biliary tract, brain and skin. Cancer risk reduction is recommended through chemoprevention (aspirin), surveillance (colonoscopy, assessment of the endometrium and ovaries via USS, aspiration biopsy and tumour marker monitoring; CA125) or risk reduction surgery, that is, total hysterectom

This case was a 73-year-old woman who previously underwent a partial colectomy for ascending colon cancer at the age of 70. She had a history of cancer of the uterus, descending colon, bladder, and left ureter. She had a family history of colorectal cancer and met the Amsterdam Ⅱ criteria for Lynch syndrome. She was diagnosed as Lynch syndrome with a MSH2 germline mutation by genetic analysis. One year later, a partial colectomy was performed for sigmoid colon cancer. Six months later, colonofiberscopy revealed early-stage cancer in the rectum, and EMR was performed. Despite adequate surveilla

Pheochromocytoma is a catecholamine-producing neuroendocrine tumor arising from chromaffin cells of the adrenal medulla. The detection of these tumors is extremely important because they are associated with high cardiovascular morbidity and mortality. Progress in molecular genetics has revealed that up to 35% of pheochromocytomas are inhereted. Lynch syndrome (hereditary nonpolypous colorectal cancer - HNPCC) is an autosomal dominant genetic condition that is associated with a high risk of colorectal cancer or other extracolonic tumors (adenocarcinoma of endometrium, stomach, ovarian carcinoma

BACKGROUND & AIMS: Affected individuals with biallelic MYH mutations are believed to display multiple adenomatous polyps without evidence of vertical transmission. Our goal was to determine the detection rate of germline MYH mutations in a high-risk gastrointestinal cancer clinic population by using polyp number as a selection criterion.
METHODS: Patients were screened for the 2 most common MYH mutations: Y165C and G382D. The complete MYH coding region was sequenced in cases with a heterozygous mutation.
RESULTS: Among 45 patients with more than 15 adenomatous polyps not diagnosed with famil
