Child's nervous system : ChNS : official journal of the International Society for Pediatric Neurosurgery2000eng
Post-traumatic seizures (PTS) can be a serious complication of head injury, because they can cause secondary brain damage through increased metabolic requirements, raised intracranial pressure, cerebral hypoxia, and/or excessive release of neurotransmitters. In children, early PTS are more frequent than late ones. In this retrospective study we conducted an epidemiological analysis and tried to identify potential risk factors for the onset of early PTS in children hospitalized for head injury in our Paediatric Intensive Care Unit. The severity of injury was assessed using the Glasgow Coma Scal

A total of 472 consecutive patients with severe head injuries (Glasgow Coma Scale score 8 or below) were studied for assessment of various risk factors affecting the outcome. The patients were divided into two groups - 299 (63 percent) patients who died and 173 (37 percent) patients who survived. Each risk factor pretraumatic, traumatic and post traumatic which might have affected the outcome was compared in each group. The following risk factors significantly increased the chances of mortality : (i) Pretraumatic - advanced age, presence of pre existing disease, (ii) Traumatic - GCS score less

Traumatic brain injury is a significant cause of morbidity and mortality in adults and can be associated with severe secondary complications, including post-traumatic cerebral infarction. One potential cause of post-traumatic cerebral infarction is cerebral fat embolism syndrome (FES). We present a case in which a male in his twenties was involved in a motorcycle collision with a truck. He sustained numerous injuries, including bilateral femur fractures, left acetabular, open left tibial and fibular fractures, and a type A aortic dissection. Before orthopedic fixation, his Glasgow Coma Score (

BACKGROUND: Previous studies have shown that children with isolated linear skull fractures have excellent clinical outcomes and low risk of surgery. We wish to identify other injury patterns within the spectrum of paediatric mild head injury, which need only conservative management. Children with low risk of evolving neurosurgical lesions could be safely managed in primary hospitals.
METHODS: We retrospectively analysed all children with mild head injury (i.e. admission Glasgow coma score 13-15) and skull fracture or haematoma on a head computed tomography scan admitted to Westmead Children's

Long-term prognosis of post-traumatic vegetative state (VS) remains poorly defined. Three kinds of data have been collected for 522 patients in VS 1 month after head injury: pre-traumatic data; early data, collected during the first two weeks after trauma; late data, collected one month or more after trauma. Statistical relationships have been searched for, between these data and outcome one year after trauma, according to the Glasgow Outcome Scale. Some late data appear as essential prognostic factors of VS: threat blink; complications; ventricular dilatation; motor score (Glasgow Coma Score)

INTRODUCTION AND OBJECTIVE: The use of antiplatelet agents is increasing, mainly in elderly patients in whom cranial trauma is a frequent reason for consultation to the emergency department. In this context, discordances have been described regarding the increased risk of post-traumatic injury that involves taking antiplatelet drugs. Therefore, the objective of this present study was to analyse factors associated with intracranial bleeding after mild brain trauma.
PATIENTS AND METHODS: A retrospective study was designed that included all patients who had consulted the emergency department dur

PRIMARY OBJECTIVE: The present study explored the possibility of predicting post-injury fitness to safe driving in patients with severe traumatic brain injury (TBI) (n = 66).
METHODS AND PROCEDURE: Sixteen different measures, derived from four domains (demo/biographic, medico-functional, neuropsychological, and psychosocial) were used as predictor variables, whereas driving outcomes were assessed in terms of driving status (post-TBI drivers versus non-drivers) and driving safety (number of post-TBI car accidents and violations).
MAIN OUTCOMES AND RESULTS: About 50% of the patients resumed dr

Our recent improved understanding of traumatic brain injury (TBI) comes largely from cohort studies of TBI patients with indication for computed tomography (CT). Using CT head as an inclusion criterion may overestimate poor outcomes after TBI with Glasgow Coma Scale (GCS) 13-15. We aimed to compare outcomes after TBI in adults who had a head CT scan (with negative findings) versus those who had no CT when presenting to an emergency department. This was a secondary analysis of a trial that recruited adults with GCS = 13-15 after TBI in Vancouver, Canada. We included 493 participants (18-69 year

Of 221 children with head injuries and resultant deep coma followed for long-term problems, 156 became totally independent, functional individuals, only 61 lacking normal cognition and speech. Reconstructive surgical procedures for residual spasticity was necessary in 45 patients. Achilles lengthenings were the most common procedure performed; after Achilles lengthenings, toe flexor releases were most often required. Twenty children had anoxia from drowning or anesthesia problems; 7 of these had spastic hip deformities or dislocations, all within 6 months of the anoxic event. Progressive cereb

INTRODUCTION: Post-traumatic locked-in syndrome may be particularly difficult to recognize, especially when it follows a state of coma and presents the clinical feature of a "total" locked-in syndrome.
PATIENT AND METHODS: A 56-year-old male with a closed head injury was admitted in intensive care unit (ICU) with GCS=4 (V1, M2, E1). Computed tomography (CT) scan disclosed a limited subarachnoid haemorrhage in the sylvian region without any brain oedema or ventricular shift. The GCS did not change until day 6. At the same time EEG showed a reactivity to acoustic stimuli consisting in the parad

The clinical course of patients admitted following minor head injuries (Glasgow Coma Score [GCS] 13-15) has been studied less extensively than in severely head injured patients. Admission criteria, methods and indications for radiological evaluation are controversial. To study this further, a retrospective review of 633 patients admitted following such injuries to King Khalid University Hospital between 1986 and 1993 was undertaken. Their ages ranged from one month to 80 years (average 17 years). The mechanisms of injury were mainly falls in 339 (53.5%) cases and road traffic accidents in 234

BACKGROUND: The effect of pre-injury antiplatelet treatment in the risk of intracranial lesions in subjects after mild head injury (Glasgow Coma Scale (GCS) 14-15) is uncertain.
METHODS: The potential risk was determined, considering its increasing use in guidelines on cardiovascular disease prevention, and ageing of the trauma population in Europe.
PATIENTS: The interaction of antiplatelet therapy with the prediction variables of main decision aids was analysed in 14,288 consecutive adolescent and adult subjects with mild head injury.
MEASUREMENTS: Any intracranial lesion at CT scan was se

INTRODUCTION: The injured brain is subjected to anaesthetic agents and various intravenous fluids which may exacerbate haemodynamic instability, increase cerebral blood volume and intracranial pressure.
OBJECTIVE: The study examined factors that influenced the immediate postoperative outcome in head injured patients who underwent general anaesthesia in a tertiary health care facility.
METHODS: The demographic and outcome data of all head injured patients who had neurosurgical procedures under general anaesthesia in our centre between January and December 2010 were compiled prospectively. Out

OBJECTIVE: Hospital-based clinical decision tools support clinician decision-making when a child presents to the emergency department with a head injury, particularly regarding CT scanning. However, there is no decision tool to support prehospital clinicians in deciding which head-injured children can safely remain at scene. This study aims to identify clinical decision tools, or constituent elements, which may be adapted for use in prehospital care.
DESIGN: Systematic mapping review and narrative synthesis.
DATA SOURCES: Searches were conducted using MEDLINE, EMBASE, PsycINFO, CINAHL and AM

UNLABELLED: background: Optimal airway management protocols for the prehospital battlefield setting have not been defined. Airway management strategies in this environment must take into account the injury patterns, the environment and training requirements of military prehospital providers.
METHODS: This is a post-hoc, sub-group analysis of the Registry of Emergency Airways Arriving at Combat Hospitals or REACH database. This study examines only those patients who had advanced airways placed for trauma by an enlisted military medic at the point of injury. results: Twenty (100%) of the patien

Traumatic brain injury (TBI) is caused by an external mechanical force to the head resulting in alteration of brain function. However, the injury to neural tracts and the connections between them is difficult to diagnose using traditional imaging techniques. A 54-year-old woman visited our clinic because of insufficient coordination of her body. Her personal history included severe TBI with a 10-day coma medically treated 10 years previously. She presented with memory impairment and insufficient coordination of her body, suggesting post-concussion syndrome. Her Glasgow Coma Scale score was 15

BACKGROUND: Several studies using diffusion tensor tractography (DTT) have demonstrated injury of the dentato-rubro-thalamic tract (DRTT) in various brain pathologies. However, no study on traumatic brain injury (TBI) has been reported. This case study attempted to demonstrate injury of the DRTT in a patient with mild TBI, using DTT.
CASE DESCRIPTION: A 41-year-old female patient suffered from head trauma resulting from flexion-hyperextension injury by being hit from behind by a running car while stopped at an intersection. The patient lost consciousness and experienced post-traumatic amnesia

Bilateral facial paralysis due to basilar skull fracture involving the temporal bone is rare and, unlike unilateral facial palsy, it can be difficult to recognize because of a lack of facial asymmetry. Thorough clinical history and physical exam, high-resolution CT scan and electrodiagnostic tests can help to make the diagnosis of bilateral facial nerve palsy and early detection, evaluation and intervention may be important for optimal functional recovery. A 16-year-old male sustained closed head injury after motor vehicle collision. The initial head CT scan showed bilateral temporal bone frac
