Special uncertainty in diagnosis and operative therapy of infrequent hepatic and/or choledochal cyst is discussed by an own case report and recent literature. Indication for operation persists always. Best operative treatment is total excision of the cyst with Roux-en-Y hepatico-(choledocho-) jejunostomy.

Choledochal cysts are uncommon congenital or acquired lesions of the biliary tree. The incidence of biliary tract carcinoma in patients with choledochal cysts is 5-35 times greater than that of the general population. Factors responsible for the increased risk of carcinoma are unknown. The case of a young woman who underwent excision of a choledochal cyst 16 years after initial diagnosis and treatment by choledochocystduodenostomy is reported. INTRODUCTION: Surgery to remove the gallbladder (laparoscopic cholecystectomy (LC)) is the standard treatment for symptomatic gallbladder disease. One potential complication of gallbladder disease is that gallstones can pass into the common bile duct (CBD) where they may remain dormant, pass spontaneously into the bowel or cause problems such as obstructive jaundice or pancreatitis. A 16 month-old girl presented with congenital common bile duct stenosis revealed by vague faintness. Percutaneous transhepatic cholangiography and surgical biliary opacification showed an abnormal junction of the pancreatico biliary ductal systems with an abnormally long common channel. These anatomic findings seem to be similar to those seen in congenital choledocal cysts and biliary tract dilatation.

The Authors report on a rare congenital anomaly of the biliary tract characterized by an accessory choledochus, which drained the inferior paramedian segments of the liver. Such anomalous duct emerged from the hepatic hilum right behind the cystic artery, the cystic duct and the gallbladder to join the primary choledochus few millimeters from the papilla of Vater. At the point of junction, in the posterior aspect of the II duodenal portion, a choledochal diverticulum containing cholesterin and bilirubin stones was also present.
