Preexposure Prophylaxis for HIV Infection Integrated With Municipal- and Community-Based Sexual Health Services.

IMPORTANCE: Several randomized clinical trials have demonstrated the efficacy of preexposure prophylaxis (PrEP) in preventing human immunodeficiency virus (HIV) acquisition. Little is known about adherence to the regimen, sexual practices, and overall effectiveness when PrEP is implemented in clinics that treat sexually transmitted infections (STIs) and community-based clinics serving men who have sex with men (MSM). OBJECTIVE: To assess PrEP adherence, sexual behaviors, and the incidence of STIs and HIV infection in a cohort of MSM and transgender women initiating PrEP in the United States.

Intergenerational support and depressive symptoms: Moderating effects of community services and smart health devices.

OBJECTIVE: Prior research suggests that intergenerational support can buffer depressive symptoms among older adults, yet the roles of community services and digital health technologies remain under-explored. We examined whether community service support and use of smart-health devices moderate the association between intergenerational support and depressive symptoms among Chinese older adults. METHODS: We analyzed two waves (2018, 2020) of the Chinese Longitudinal Healthy Longevity Survey (CLHLS), including 4054 older adults. A multilevel mixed-effects model was employed to account for both c

Community-based approaches and partnerships: innovations in health-service delivery in Bangladesh.

In Bangladesh, rapid advancements in coverage of many health interventions have coincided with impressive reductions in fertility and rates of maternal, infant, and childhood mortality. These advances, which have taken place despite such challenges as widespread poverty, political instability, and frequent natural disasters, warrant careful analysis of Bangladesh's approach to health-service delivery in the past four decades. With reference to success stories, we explore strategies in health-service delivery that have maximised reach and improved health outcomes. We identify three distinctive

e-hub: an online self-help mental health service in the community.

Self-help delivered through online websites has been found to be beneficial for people with mental health problems, and consumers find internet support groups helpful. The Australian National University e-hub group delivers automated web interventions (BluePages, MoodGYM, e-couch) and an online bulletin board (BlueBoard) to the public for mental health self-help. The evidence-based web interventions require no direct therapist or other human involvement, while the bulletin board is moderated by trained consumers under the supervision of a clinical psychologist. Users may contact the e-hub serv

Expanding Home- and Community-Based Service Access in the Veterans Health Administration.

OBJECTIVES: The US Department of Veterans Affairs (VA) offers a continuum of home- and community-based services (HCBS) to support veterans' ability to remain in their homes and communities as they age. Although VA delivers some HCBS directly, many HCBS are provided through contracted agencies in the community. We sought to understand the strategies VA staff use to facilitate veterans' HCBS access, particularly as these services become increasingly difficult to access in many areas. DESIGN: Qualitative descriptive study using semistructured interviews. SETTING AND PARTICIPANTS: A purposeful s

The Impact of Certified Community Behavioral Health Clinics on Service Access and Engagement.

The mental health system struggles to engage people with severe mental illnesses (SMI) and substance use disorders (SUD) in care. Certified community behavioral health clinics (CCBHCs) are designed to meet service gaps by providing person-centered, comprehensive, coordinated, accessible, and quality care. This study examines the impact of the CCBHC model on access and engagement of service users utilizing administrative data. There was a significant increase in existing and new service users (χ2 = 5.91; p < .05) after the CCBHC implementation and among existing service users with SMI (χ2 = 43.

Use and sources of payment for health and community services for children with impaired mobility.

A survey was made of the parents of 380 children whose mobility impairments require the use of a wheelchair, walker, or braces. They were asked about equipment, health services, related services, and family support services used during the previous year. There was extensive use of equipment and traditional medical and health services, such as visits to primary care and specialist physicians; there was moderate use of related health services, such as physical or occupational therapy and child counseling; and there was very little use of community-based family support services, such as respite c

Use of a digital application to enhance communication and triage between care homes and national health service community services in the United Kingdom: a qualitative evaluation.

Recent years have seen a rise in digital interventions to improve coordination between care homes and NHS services, supporting remote sharing of data on the health of care home residents. Such interventions were key components in the response to the COVID-19 pandemic. This paper presents findings from the qualitative component of an evaluation of an implementation of the HealthCall Digital Care Homes application, across sites in northern England. The purpose of this qualitative component was to explore issues round feasibility, appropriacy, and acceptability. The implementation commenced prior

Loneliness and social isolation risk factors in community-dwelling older adults receiving home health services.

BACKGROUND: Social isolation and loneliness are highly prevalent in older adults. Older adults who are receiving home health services (HHS) post hospital discharge are at high risk for social isolation and loneliness related to multimorbidity and functional decline. Yet, the prevalence of social isolation and loneliness in this population is not commonly described. METHODS: We analyzed electronic health record (EHR) data from 2,026 community-dwelling older adults (mean age 77.5 ± 8.2, 61.7% female, 35% Black/African American, 42.2% Hispanic) who were discharged with HHS from three acute care
