Barriers and facilitators for community surgery in general practice / family medicine settings: a scoping review.

BACKGROUND: Rising healthcare demand and costs have prompted policymakers to explore strategies for delivering appropriate care closer to patients. In Ireland, Sláintecare supports the transfer of suitable hospital activity to community-based providers, while the United Kingdom's Fit for the Future: 10-Year Health Plan promotes a shift towards community-based care. Community surgery may contribute to this transition, but successful implementation requires appropriate resources, training, governance, and collaboration. This scoping review aimed to synthesise existing evidence on barriers and fa

Evaluation of an Invisible Illness Communication Strategy Curriculum Among Internal Medicine Interns.

INTRODUCTION: Patient-physician communication in invisible illness is challenging. Invisible illnesses (e.g., fibromyalgia, myalgic encephalomyelitis/chronic fatigue syndrome, and disorders of gut-brain interaction) are clinical syndromes that are not readily visible or measurable by physical exam or diagnostic testing, and patients outwardly appear healthy. Patients with invisible illness often feel dismissed, while physicians often feel helpless or powerless when facing invisible illness. Invisible illness communication strategies can improve patient-physician relationships, patient and phys

Ecotoxicological Impacts of Heavy Metals on Medicinal Plant Quality and Rhizosphere Microbial Communities.

With the rapid expansion of industrial activities, the accumulation of heavy metals in the environment has become a serious threat to ecological security and public health. Rhizosphere microorganisms play a crucial role in supporting the growth and quality of medicinal plants by facilitating nutrient uptake and regulating hormonal balance. However, medicinal plants can absorb heavy metals from contaminated soils during growth, resulting in toxic metal accumulation in plant tissues and reduced efficacy of active compounds. At the same time, excessive heavy metal levels suppress rhizosphere micr

Teaching Abolition Medicine: Best Practices for Centering Criminalized Communities in Medical Education.

Mass incarceration is a significant structural determinant of health, affecting incarcerated individuals, their families, and communities, with profound racial disparities. Health care professionals have an opportunity to reduce these inequities through abolition medicine. Abolition in health care means rewriting how doctors relate to patients labeled as criminal and is not a new checklist that can be imposed on the existing curriculum. Beyond changing individual clinical practice, abolition medicine also provides a critical framework for dismantling unjust policies. However, published medical

Managing medicines in the time of COVID-19: implications for community-dwelling people with dementia.

COVID-19 has changed life beyond recognition for millions of individuals, as countries implement social distancing measures to prevent disease transmission. For certain patient groups, such as community-dwelling older people with dementia (PwD), these restrictions may have far-reaching consequences. Medicines management may be adversely affected and deserves careful thought. PwD face unique challenges with medicines management compared to other older people, often relying upon support from family/carers and primary healthcare professionals. This article considers potential issues that PwD may

Developing a method to identify medicines non-adherence in a community sample of adults with epilepsy.

The aim is to propose a simple way of identifying patients at risk of antiepileptic drug (AED) non-adherence during epilepsy review (a scheduled consultation to review the patient and their condition). The use of a multi-modal approach to the problem of non-adherence is necessary because of the limitations of existing methods. A mixed methodology was developed in a nested study using a case record review to calculate the medicine possession ratio (MPR) from the AED medication records of a community sample, a literature review and a consensus panel to develop a questionnaire to address how peop

Human-first, AI-assisted Peer Review: A Proposed Workflow for Community Medicine and Public Health Journals.

Growth in community medicine and public health publications has increased peer-review workload and the risk of missed methodological and statistical issues. Because peer review is largely voluntary and often performed alongside clinical, teaching, administrative, and research duties, reviews may be delayed or rushed. Generative artificial intelligence (AI) may help, but unstructured use raises concerns about confidentiality, integrity, and automation bias. This viewpoint proposes a human-first, AI-assisted workflow in which reviewers first complete an independent manual review. With editor per
