Providing prescheduled appointments as a strategy for improving follow-up compliance after community-based glaucoma screening: results from an urban underserved population.

To determine if receiving a prescheduled appointment is associated with an increased likelihood of complying with follow-up eye care among individuals identified as at risk for glaucoma during community-based glaucoma screening in an urban underserved population. This study sampled 362 individuals aged ≥30 years without known glaucoma from low-income, predominantly black/Hispanic neighborhoods in New Haven, Connecticut presenting to one of twelve community-based glaucoma screening events from May 2010 to October 2012. A quasi-experimental design systematically assigned 63 individuals identifie

The association between health insurance status and utilization of health services in rural Northern Ghana: evidence from the introduction of the National Health Insurance Scheme.

BACKGROUND: Many households in low- and middle-income countries face financial hardships due to payments for health care, while others are pushed into poverty. Risk pooling and prepayment mechanisms help to lessen the impact of the costs of care as well as assisting to achieve universal health coverage (UHC). Ghana implemented the National Health Insurance Scheme (NHIS) for the promotion of access to health services for all Ghanaians. In this paper, we examined the association between health insurance status and utilization of outpatient and inpatient health services in rural poor communities.

Health insurance non-enrollment among women in Sierra Leone: A cross-sectional analysis of the 2019 Demographic and Health Survey.

BACKGROUND: Health insurance enrollment is a vital component of universal health coverage and access to essential healthcare services. However, in Sierra Leone, enrollment remains persistently low, posing a major public health challenge. Women of reproductive age (15-49 years) represent a critical population for health insurance enrollment due to their unique healthcare needs, particularly related to reproductive health, pregnancy, and childcare. Despite their importance, women face notable barriers to health insurance enrollment, including financial constraints, gender inequalities, and socio

Tropical Laboratory Initiative: An innovative model for laboratory medicine in rural areas.

BACKGROUND: Communities in rural, low-resource settings often lack access to reliable diagnostics. This leads to missed and misdiagnosed cases of disease and contributes to morbidity and mortality. OBJECTIVE: This paper describes a model for providing local laboratory services to rural areas of Ghana, and provides suggestions on how it could be adapted and expanded to serve populations in a range of rural communities. METHODS: The Tropical Laboratory Initiative (TLI) system in Ghana comprises one central laboratory where samples delivered from clinics by motorbike riders are analysed. Test r

Facial Feminization Surgery: The Ethics of Gatekeeping in Transgender Health.

The lack of access to gender-affirming surgery represents a significant unmet health care need within the transgender community, frequently resulting in depression and self-destructive behavior. While some transgender people may have access to gender reassignment surgery (GRS), an overwhelming majority cannot afford facial feminization surgery (FFS). The former may be covered as a "medical necessity," but FFS is considered "cosmetic" and excluded from insurance coverage. This demarcation between "necessity" and "cosmetic" in transgender health care based on specific body parts is in direct opp

From Tanzania to Washington, DC: Application of the Consolidated Framework for Implementation Research and ADAPT-ITT Model to Guide the Development of a Community-Based PrEP Intervention for Black Adults.

While HIV incidence in Washington, DC has declined overall, significant racial disparities persist. Pre-exposure prophylaxis (PrEP) remains underutilized among Black adults due to multiple barriers including medical mistrust, stigma, and structural barriers to access. Using the Consolidated Framework for Implementation Research (CFIR) and ADAPT-ITT model, we conducted in-depth interviews with opinion leaders (n = 28) including community-based organization leaders, health department representatives, healthcare providers, and current/potential PrEP users to guide the adaptation of a community-ba

Harambee! 3.0: Addressing HIV-Stigma and Increasing HIV-Testing in Three African Immigrant Communities in King County.

BACKGROUND: In King County, Washington, the new HIV diagnosis rate is 10 times higher among African immigrants than the general population. Many African immigrants report never testing for HIV due to various reasons, including inaccessible testing, stigma around HIV and testing, and concerns over jeopardizing their immigration status. OBJECTIVES: We partnered with the Ethiopian Community in Seattle, the Eritrean Health Board, and the Congolese United Foundation to deliver community-based HIV testing via health fairs at faith-based organizations in King County. Black Women's Maternal Health: Insights From Community Based Participatory Research in Newark, New Jersey.

This study on Black women's maternal health engaged a group of six community members in a community based participatory research project in a state with one of the largest racial disparities in maternal mortality and severe maternal morbidity in the United States. The community members conducted 31 semi-structured interviews with other Black women who had given birth within the past 3 years to examine their experiences throughout the perinatal and post-partum period. Four main themes emerged: (1) challenges related to the structure of healthcare, including insurance gaps, long wait times, lack

Epidemiology and Recurrence of Sigmoid Volvulus: Analysis of Health Insurance Claims Data in Japan.

OBJECTIVES: Sigmoid volvulus (SV) is rare in Japan, and most studies have been limited to small numbers or case series reports. We aimed to explore the incidence, risk factors, and recurrence rates of SV in Japan using community-based health insurance claims data. METHODS: We used health insurance claims data from the Toyama Prefecture from April 2018 to March 2022. Individuals aged ≥ 40 years who had no diagnosis of SV (International Statistical Classification of Diseases, 10th Revision) in 2018 were included in our study. Patients who were newly diagnosed with SV in 2019 were identified. Th

Universal health coverage in Rwanda: dream or reality.

INTRODUCTION: Universal Health Coverage (UHC) has been a global concern for a long time and even more nowadays. While a number of publications are almost unanimous that Rwanda is not far from UHC, very few have focused on its financial sustainability and on its extreme external financial dependency. The objectives of this study are: (i) To assess Rwanda UHC based mainly on Community-Based Health Insurance (CBHI) from 2000 to 2012; (ii) to inform policy makers about observed gaps for a better way forward. METHODS: A retrospective (2000-2012) SWOT analysis was applied to six metrics as key indi

CSHCN with hearing difficulties: Disparities in access and quality of care.

BACKGROUND: Hearing difficulties (HD) affect a sizable minority of children in the United States and can have a significant impact on child development. Children with HD may face barriers around language development, communication abilities, learning, and social interactions, which in turn can affect multiple aspects of their lives. OBJECTIVE/HYPOTHESIS: Little is known about the extent to which children with HD have access to a system of care and we hypothesize that this population may experience significant unmet needs. METHODS: In this cross-sectional study, using bivariate and multivaria

[Trends of work force participation of patients with rheumatic diseases : results from German social insurance data and the national database of the German collaborative arthritis centers].

Positive therapeutic effects on the work force participation derived from international clinical trials may not be directly transferable to the community based care in Germany. Therefore recent changes of data regarding sick leave (SL), work disability pension (WDP) and employment from the social insurance and from the national database of the German collaborative arthritis centers were analyzed covering a time period of at least 10 years. Health insurance data showed a steeper decline in the average duration of SL caused by rheumatoid arthritis (RA), ankylosing spondylitis (AS) and systemic l

The Role of Social Determinants of Health in Self-Reported Access to Health Care Among Women Undergoing Screening Mammography.

Background: Social determinants of health (SDOH) contribute to health care disparities, with social and economic barriers often leading to difficulties in obtaining necessary care. We evaluated barriers to receiving health care, focusing on caretaker responsibilities, health insurance and cost, and transportation. Materials and Methods: We included women ages ≥40 years receiving screening mammography across three Breast Cancer Surveillance Consortium registries from 2012 to 2017. Women self-reported social and financial barriers to receiving health care in the 12 months before their screening

Use and sources of payment for health and community services for children with impaired mobility.

A survey was made of the parents of 380 children whose mobility impairments require the use of a wheelchair, walker, or braces. They were asked about equipment, health services, related services, and family support services used during the previous year. There was extensive use of equipment and traditional medical and health services, such as visits to primary care and specialist physicians; there was moderate use of related health services, such as physical or occupational therapy and child counseling; and there was very little use of community-based family support services, such as respite c

Perception of access to health care by homeless individuals seeking services at a day shelter.

Homeless individuals experience significant physical, mental health and substance abuse issues. This study describes the prevalence of health issues and perceptions of access to care among 300 homeless individuals who use a day shelter. Approximately 43% described a serious/chronic physical health problem, 53% a serious mental health problem, and 49% a substance use disorder. Those reporting a serious problem were more likely to have insurance and to report greater perceived access to care but perceived access to care was less than expected. Having insurance was also related to longer duration
