Polyarteritis Nodosa with Concurrent CMV Infection: A Case Report and Brief Literature Review.

A previously healthy 36-year-old male presented to the ED with a six-week history of severe migratory abdominal pain, nightly fevers, fatigue, and weight loss. Labs showed elevated LFTs, CRP, D-dimer, and lymphocytosis along with positive serology for cytomegalovirus (IgM, IgG, DNA). Abdominal CT showed a superior mesenteric venous thrombosis, portal mesenteric venous thrombosis, and multiple splenic infarcts. Diagnostic angiography revealed numerous arterial microaneurysms consistent with polyarteritis nodosa. CMV infection is typically self-limited with complete recovery within days to weeks

Concurrent Polycythemia Vera with Newly Diagnosed Multiple Myeloma: Case Report and Literature Review.

Concurrent polycythemia vera with newly diagnosed multiple myeloma is extremely rare. We described a 70-year-old woman with concurrent polycythemia vera and newly diagnosed multiple myeloma. Genetic testing confirmed the JAK2 V617F mutation for the first time, while determination of serum erythropoietin decreased. A retrospective review of our patient's case was conducted thereafter, and related literature was systemically reviewed. We totally identified eight cases with concurrent polycythemia vera with newly diagnosed multiple myeloma, which were further analyzed and compared. The present ca

Concurrent chronic myelomonocytic leukemia and gastric carcinoma: a case report and literature review.

BACKGROUND: Chronic myelomonocytic leukemia (CMML) is a rare, malignant, clonal hematopoietic disorder with features of both myelodysplastic syndrome (MDS) and myeloproliferative neoplasm (MPN). It is classified as MDS/MPN overlap syndrome by the World Health Organization (WHO), and the prognosis is generally poor. Solid tumors are rarely associated with or are secondary to CMML. CASE DESCRIPTION: We here reported a case of a 75-year-old female patient with persistent peripheral blood monocytosis and bone marrow blasts ≤20%. A diagnosis of CMML was made. Unexpectedly, she presented with recur

Urethrovaginal fistula repair with or without concurrent fascial sling placement: A retrospective review.

INTRODUCTION: We sought to review outcomes of urethrovaginal fistula (UVF) repair, with or without concurrent fascial sling placement. METHODS: All patients diagnosed with UVF at our center from 1988-2017 were included in this study. Patient charts were reviewed from a prospectively kept fistula database, and patient characteristics and surgical outcomes were described. Descriptive statistics were applied to compare complication rates between patients with or without fascial sling placement at the time of UVF repair. RESULTS: A total of 41 cases of UVF were identified, all of which underwent

Moyamoya disease concurrent with dural arteriovenous fistula: A case report and literature review.

Moyamoya disease (MMD) and dural arteriovenous fistula (DAVF) are two distinct types of intracranial lesion that share different pathogenic mechanisms. Under rare circumstances, patients with MMD have been reported to have concurrent DAVF. The present case study reports on a 47-year-old male admitted due to sudden headache. Head CT revealed hemorrhage of the right thalamus with ventricular extension. On CT angiography, the normal vasculature in the anterior and posterior circulation disappeared and was replaced by moyamoya-like vessels. The patient received conservative management and was disc

Acute Leukemia and Concurrent Mediastinal Germ Cell Tumor: Case Report and Literature Review.

There is a known association of primary nonseminomatous mediastinal germ cell tumors (NSMGCT) and hematologic malignancy in younger males not linked to treatment. When combined these two rare entities convey a very poor prognosis. Here we report a 16-year-old male with an anterior mediastinal mass diagnosed as a malignant germ cell tumor based on elevation of serologic markers. He was found to have acute leukemia with megakaryocytic differentiation several days later. We focus our report on the pathologic findings, including a review of the literature, and a novel molecular analysis of the ger

A Rare Concurrence of Anaplastic and Papillary Thyroid Carcinomas: A Case Report and Literature Review.

BACKGROUND: Anaplastic thyroid carcinoma (ATC) is a rare, undifferentiated thyroid malignancy with a poor prognosis and rapid clinical progression. It accounts for less than 10% of thyroid cancers but contributes disproportionately to thyroid cancer-related mortality. Histologically, ATC exhibits significant pleomorphism and can present with diverse morphological patterns, often coexisting with differentiated thyroid carcinoma. CASE PRESENTATION: We report a case of a 39-year-old female with a 20-year history of anterior neck swelling, which rapidly progressed over six months. Imaging reveale

Risk of bleeding with the concurrent use of amiodarone and DOACs: a systematic review and meta-analysis.

BACKGROUND AND AIMS: Amiodarone is frequently prescribed alongside direct oral anticoagulants (DOACs) in atrial fibrillation. There are concerns regarding drug-drug interactions (DDIs) between amiodarone and DOACs. The literature is conflicting on the clinical implications of this DDI, hence we conducted a meta-analysis to compare bleeding risk among patients receiving DOACs, with and without concurrent amiodarone. METHODS AND RESULTS: A systematic search was conducted for studies published between 1 January 2009 and 26 June 2024 in MEDLINE via PubMed, Embase, and CENTRAL. Included studies co

Management of Concurrent Immune Thrombocytopenia and Ulcerative Colitis: A Case Report and Literature Review.

We herein report a case of concurrent immune thrombocytopenia (ITP) and ulcerative colitis (UC) that achieved remission following mesalazine treatment. A 16-year-old girl presented with severe thrombocytopenia, abdominal pain, and bloody stool. She was initially diagnosed with ITP and then was treated with prednisolone, resulting in an immediate improvement of symptoms. Upon tapering the steroids, the symptoms recurred, thus leading to a subsequent diagnosis of UC via colonoscopy. Treatment with mesalazine promptly induced the remission of both ITP and UC, which was sustained. We reviewed 24 p

Gestational trophoblastic neoplasia with concurrent metastasis to the mother and child: a systematic literature review.

Gestational trophoblastic neoplasia (GTN) arising in the placenta and presenting as a metastatic disease concurrently in the mother and the baby is extremely rare. GTN poses a diagnostic dilemma to the treating clinicians. In the current review, an electronic search of Scopus, PubMed, Embase and other databases was conducted for case reports and case series of GTN co-existing or metastatic to both the mother and the baby, published to date. Globally, a total of twenty-two cases of GTN with metastasis to both the mother and baby was found. The previous history of histopathology confirmed molar

Concurrence of familial Mediterranean fever and Behçet's disease: a case report and review of the literature.

BACKGROUND: Familial Mediterranean fever and Behçet's disease are distinct disorders that are prevalent in the Mediterranean and Middle Eastern populations. They are characterized by unprovoked inflammatory episodes caused by overexpression of proinflammatory cytokines. Although reported previously, the overlapping presentation of familial Mediterranean fever and Behçet's disease remains uncommon. CASE PRESENTATION: A 46-year-old Lebanese-Canadian man who presented with recurrent oral and genital ulcers, polyarticular synovitis, ocular swelling, recurrent infections, and fevers was later foun

Concurrent radical cystectomy and nephroureterectomy indications and outcomes: a systematic review and comparative analysis.

INTRODUCTION: Urothelial carcinoma can arise from the urinary bladder or from the upper urinary tract. In some instances, urinary bladder cancer (UBC) and upper tract urothelial carcinoma (UTUC) can be concurrently diagnosed, necessitating a combined radical cystectomy (RC) with radical nephroureterectomy (RNU). A systematic review was done on the combined procedure exploring outcomes and indications, in addition to a comparative analysis between the combined procedure and cystectomy alone. METHODS: For the systematic review, three databases (Embase, PubMed, and Cochrane) were queried, select

Concurrent thyrolipomatosis and thymolipoma in a patient with myasthenia gravis: a case report and review of the literature.

We present a case of a man with a background of myasthenia gravis who presented with a neck lump, which was diagnosed as thyrolipomatosis in continuity with a very large thymolipoma. Following removal of these lesions, the patient's myaesthenic symptoms improved. While thymolipomas are often seen in the context of myasthenia gravis, thyrolipomatosis is a rare entity and to our knowledge the concurrent finding of both lesions with myasthenia gravis has never been reported. We highlight the important imaging features of both entities and the clinical importance of recognising them.

Levofloxacin-induced rhabdomyolysis in a patient on concurrent atorvastatin: Case report and literature review.

WHAT IS KNOWN AND OBJECTIVE: The combination of HMG-CoA reductase inhibitors (statins) and fluoroquinolones generally is not considered a significant risk factor for rhabdomyolysis. Rhabdomyolysis is a known risk associated with statin therapy but has seldom been described with fluoroquinolone use. We describe a case of acute rhabdomyolysis involving the co-administration of atorvastatin and levofloxacin. CASE DESCRIPTION: A 65-year-old white male presented with clinical and laboratory evidence of rhabdomyolysis after approximately 19 days of levofloxacin therapy for treatment of a prosthetic

Preoperative imaging for parathyroid localization in patients with concurrent thyroid disease: A systematic review.

BACKGROUND: Thyroid disease occurs more frequently in patients with hyperparathyroidism than the general population and hinders parathyroid localization. Identifying thyroid pathology before operating improves management and avoids the risks of reoperation in the neck. This review assesses imaging studies in patients with hyperparathyroidism and thyroid pathology to identify the ideal imaging methodology for patients with multigland disease. METHODS: Systematic review of original articles reporting sensitivity or positive predictive value (PPV) for one or more imaging modalities in patients w

Concurrent eosinophilia and IgG4-related disease in a child: A case report and review of the literature.

The current study presents the case of a 9-year-old Chinese boy who presented with eosinophilia and elevated serum levels of immunoglobulin G4 (IgG4). A bone marrow puncture identified an elevated eosinophil rate of 23% (normal range, <5%), which indicated eosinophilia. However, gene analysis, fluorescent in situ hybridization and other examinations, including bone marrow aspiration, blood routine, auto-antibody tests and parasitic and allergens screening, contradicted a diagnosis of secondary or clonal eosinophilia. Furthermore, the patient exhibited multiple lymph node swelling and a lymph b
