Disorders of consciousness: differential diagnosis and neuropathologic features.

Disorders of consciousness present intriguing challenges to the neurologist and neurorehabilitation specialist. Assessment is constrained by the lack of reliable methods of assessing consciousness, and there are no treatment interventions known to influence the course of recovery from these conditions. In addition, the relationship between the clinical features associated with these disorders and their corresponding pathophysiologic substrate is also unclear. Our understanding of disorders of consciousness has not kept pace with the advances in neurosurgical management that have decreased mort

[Consciousness disorders in patients after carotid artery surgery].

The range of surgical management of the carotid arteries becomes every day wider. The most common cause of carotid arteries disease is atherosclerosis and its consequences for the brain, and more rarely anatomical anomalies. The majority of our patients had cerebral damage prior to the surgical procedure, so we have taken particular care to prevent cerebral perfusion disturbances following the operation. Consciousness disorders are not always the consequence of disturbed cerebral perfusion, but their cause may have effect on it. Our experience and observations are presented with the aim to ale

Spasticity Management in Persons with Disorders of Consciousness.

INTRODUCTION: Spasticity is one of the most frequent neurological impairments affecting persons with disorders of consciousness (DoC). If left untreated, it can mask signs of consciousness by inhibiting one's ability to interact with the environment. The lack of information about spasticity specific to patients with DoC may result in insufficient or even inappropriate treatment. OBJECTIVE: To report spasticity characteristics and management in a large dedicated DoC rehabilitation program. DESIGN: Retrospective chart review. SETTING: An inpatient rehabilitation hospital. PARTICIPANTS: Patie

Electroencephalographic profiles for differentiation of disorders of consciousness.

BACKGROUND: Electroencephalography (EEG) is best suited for long-term monitoring of brain functions in patients with disorders of consciousness (DOC). Mathematical tools are needed to facilitate efficient interpretation of long-duration sleep-wake EEG recordings. METHODS: Starting with matching pursuit (MP) decomposition, we automatically detect and parametrize sleep spindles, slow wave activity, K-complexes and alpha, beta and theta waves present in EEG recordings, and automatically construct profiles of their time evolution, relevant to the assessment of residual brain function in patients

Pain assessment and management in disorders of consciousness.

PURPOSE OF REVIEW: Pain and suffering controversies in persons with disorders of consciousness continue to be debated by the scientific, legal and medical ethics communities. This review examines the current knowledge base for guiding decisions regarding assessment and management of pain in persons with disorders of consciousness. RECENT FINDINGS: Studies have shown that brain processing linked to pain in persons in a vegetative state is incomplete and is processed only at a primary and not higher secondary level. Therefore, such painful stimuli would not reach the threshold for conscious exp

The Dilemma of Hydrocephalus in Prolonged Disorders of Consciousness.

Prolonged disorders of consciousness (DOC) are considered to be among the most severe outcomes after acquired brain injury. Medical care for these patients is mainly focused on minimizing complications, given that treatment options for patients with unresponsive wakefulness or minimal consciousness remain scarce. The complication rate in patients with DOC is high, both in the acute hospital setting, as in the rehabilitation or long-term care phase. Hydrocephalus is one of these well-known complications and usually develops quickly after acute changes in cerebrospinal fluid (CSF) circulation af

Advances in the neurorehabilitation of severe disorder of consciousness.

INTRODUCTION: The paper describes the evolution of knowledge concerning severe brain injury which determines the Vegetative State/Unresponsive Wakefulness Syndrome. BACKGROUND: The term Vegetative State was proposed by Jennet and Plum in 1972. Later on, the Intensive Care Units progresses increased the survival of these patients and, contemporary, decreased their characteristic conditions of cachexia and severe dystonia. In 1994, the disease was conceived as a disconnection syndrome of the hemispheres from the brainstem, mainly due to a temporary or permanent deficit of the functions of the w

Characterizing consciousness states: EEG microstate dynamics in patients with disorders of consciousness during naturalistic movie-viewing.

Consciousness assessment in disorders of consciousness (DoC) patients remains clinically challenging. Dynamic brain activities responsive to sensory stimulations have been suggested to contain consciousness-related information. However, primary sensory processing can occur unconsciously, necessitating evaluation of residual higher-order cognitive functions for effective assessment. In this study, we introduced a movie-viewing paradigm incorporating a scrambled version to control for primary sensory processing and applied electroencephalography (EEG) microstate analysis to capture higher-order

Gut Microbiome Differentiates Levels of Consciousness in Patients with Prolonged Disorders of Consciousness after Brain Injury.

Differentiation of levels of consciousness in patients with prolonged disorders of consciousness (DOC) remains a major challenge, especially differentiation between vegetative state (VS) and minimally conscious state (MCS). This study was to investigate the alterations of gut microbiota in patients with DOC and to identify potential microbiome biomarkers that can differentiate levels of consciousness. In this study, we collected fecal samples from patients with prolonged DOC, including 19 patients in MCS and 14 patients in VS; 16S-rRNA sequencing was used to investigate the gut microbiome of p

Modulation of consciousness in disorders of consciousness by HD-tDCS targeting frontal and posterior cortical regions.

The search for neural correlates of consciousness has always been a hot topic in the field of consciousness. Previous studies have shown that high-density transcranial direct current stimulation (HD-tDCS) can promote the recovery of consciousness level in patients with disorders of consciousness (DOC). This study intends to explore the neuromodulatory effects of HD-tDCS at different target from the perspectives of brain spatiotemporal dynamics and directed information flow, thereby clarifying the key brain areas for the origin of consciousness. The study prospectively used a high-density trans

Sleep in disorders of consciousness: behavioral and polysomnographic recording.

BACKGROUND: Sleep-wakefulness cycles are an essential diagnostic criterion for disorders of consciousness (DOC), differentiating prolonged DOC from coma. Specific sleep features, like the presence of sleep spindles, are an important marker for the prognosis of recovery from DOC. Based on increasing evidence for a link between sleep and neuronal plasticity, understanding sleep in DOC might facilitate the development of novel methods for rehabilitation. Yet, well-controlled studies of sleep in DOC are lacking. Here, we aimed to quantify, on a reliable evaluation basis, the distribution of behavi

Therapeutic interventions in patients with prolonged disorders of consciousness.

The management of patients with severe brain injuries and prolonged disorders of consciousness raises important issues particularly with respect to their therapeutic options. The scarcity of treatment options is challenged by new clinical and neuroimaging data indicating that some patients with prolonged disorders of consciousness might benefit from therapeutic interventions, even years after the injury. Most studies of interventions aimed at improving patients' level of consciousness and functional recovery were behavioural and brain imaging open-label trials and case reports, but several ran

Disorders of consciousness terminology: history, evolution and future directions.

There has been a significant evolution of nomenclature with regards to classification of persons with disorders of consciousness (DoC) over the last 100 years. This paper provides a review of the evolution of this terminology with discussion of the advantages and disadvantages of historical and current terms. Recommendations for how this evolution should continue moving forward in the best interest of patients, their families, society, clinical care, and research will also be addressed. The taxonomy we choose, hopefully by international consensus, has multifaceted implications that go well bey

Technology-based assessment in patients with disorders of consciousness.

INTRODUCTION: A number of studies highlight the difficulty in forming a diagnosis for patients with disorders of consciousness when this is established merely on behavioral assessments. BACKGROUND: Positron emission tomography (PET), functional magnetic resonance imaging (fMRI), diffusion tensor imaging (DTI), and electroencephalography combined with transcranial magnetic stimulation (TMS-EEG) techniques are promoting the clinical characterization of this challenging population. With such technology-based "objective" tools, patients are also differentially able to follow simple commands and i

Willful modulation of brain activity in disorders of consciousness.

BACKGROUND: The differential diagnosis of disorders of consciousness is challenging. The rate of misdiagnosis is approximately 40%, and new methods are required to complement bedside testing, particularly if the patient's capacity to show behavioral signs of awareness is diminished. METHODS: At two major referral centers in Cambridge, United Kingdom, and Liege, Belgium, we performed a study involving 54 patients with disorders of consciousness. We used functional magnetic resonance imaging (MRI) to assess each patient's ability to generate willful, neuroanatomically specific, blood-oxygenatio
