Journal of neuro-ophthalmology : the official journal of the North American Neuro-Ophthalmology Society2018eng
BACKGROUND: Traditional orthoptic therapy used by ophthalmologists, orthoptists, and optometrists is directed at improving visual acuity, ocular alignment, or both. For example, convergence exercises are used to treat convergence insufficiency (CI). However, other forms of "vision therapy" are directed at improving "visual processing and efficiency." The therapeutic regimen often entails repetitive ocular motor tasks performed during multiple office visits with a behavioral optometrist. These ocular motor tasks are used to treat diverse conditions such as learning disabilities, poor reading ab

This study sought to test the hypothesis that significant differences would be observed in clinical measures, symptoms, and objective assessments of vergence eye movements between children with typically developing convergence insufficiency (TYP-CI) and children with persistent post-concussion symptoms with convergence insufficiency (PPCS-CI). Data from age-matched binocularly normal controls (BNC) were used for comparison. Data from three groups of children 11 to 17 years of age are presented: BNC (N = 11), TYP-CI (N = 10), and PPCS-CI (N = 15). Clinical measures of vergence, accommodation, a

INTRODUCTION: The vergence oculomotor system possesses two robust adaptive mechanisms; a fast "dynamic" and a slow "tonic" system that are both vital for single, clear and comfortable binocular vision. The neural substrates underlying these vergence adaptive mechanisms in humans is unclear.
METHODS: We investigated the role of the posterior cerebellum in convergence adaptation using inhibitory continuous theta-burst repetitive transcranial magnetic stimulation (cTBS) within a double-blind, sham controlled design while eye movements were recorded at 250hz via infrared oculography.

Vergence prism adaptability was evaluated in subjects with high symptomatic and asymptomatic heterophoria and compared to individuals with a heterophoria within normal range (the control group). A computer haploscope was used to measure phoria values and changes in the eye position after introducing 6 prism diopters base out in front of the right eye. Phoria values were measured with a nonius paradigm every minute for a period of 10 minutes during adaptation. The results showed that subjects with symptomatic heterophoria are characterized by a lower rate of prism adaptation and adapted to a sm

This study sought to determine the linearity and amplitude of both proximally induced accommodation (PIA) and vergence (PIV) by comparing the open-loop accommodation and vergence responses, respectively, to individual stimuli located at viewing distances ranging from 0.20 m to 1,500 m. For the measurement of PIA, the vergence and accommodation loops were opened by subjects (N = 10) monocularly viewing the target through a 0.5-mm pinhole, while accommodation was assessed concurrently using an infrared optometer. In the assessment of PIV, the accommodation loop was opened either by subjects (N =

Convergence insufficiency (CI) is a well-known syndrome of binocular visual dysfunction. In a review of 58 papers, considerable variation was noted in the criteria used to define the condition. Symptoms and decreased positive fusional vergences (both at the nearpoint) were the only criteria named in more than one-half of the studies. An extended nearpoint of convergence (NPC) and increased exophoria at near were criteria in about one-third of the papers. Examination of data in the reviewed papers shows that although considerable variability was noted, the distance exodeviation, distance negati

The synoptophore was used to measure torsional interocular disparity. This, in turn, was used to compute how much the angle between the Listing's plane (LP) of the two eyes changes as a function of the vergence angle. The ratio of these two angles was defined as G. We measured G in normals and in patients suffering from intermittent horizontal strabismus. Consistent with previous search-coil experiments and with our previous visual test measures, the results using the synoptophore suggest that, for normals, G is less than 1. In the patient group the mean G was similar in magnitude but more var

PURPOSE: Post-rotatory nystagmus has been used to detect autism spectrum disorders in clinical settings. Although previous studies have focused on eye movements, they did not evaluate the change in ocular refraction during post-rotatory nystagmus. This study aimed to evaluate the changes in ocular refraction during post-rotatory nystagmus in healthy individuals.
METHODS: A total of 34 healthy volunteers (mean age ± standard deviation, 20.9 ± 0.6 years) participated in this study. The ocular refraction during post-rotatory nystagmus was measured using MR-6000 (Tomey Inc.) on quick mode with a

PURPOSE: To investigate differences in the stimulus accommodative convergence/accommodation (AC/A) ratio using various techniques and accommodative stimuli, and to describe a method for determining the stimulus AC/A ratio.
METHODS: A total of 81 subjects with a mean age of 21 years (range, 20-23 years) were enrolled. The relationship between ocular deviation and accommodation was assessed using two methods. Ocular deviation was measured by varying the accommodative requirement using spherical plus/minus lenses to create an accommodative stimulus of 10.00 diopters (D) (in 1.00 D steps). Ocular

This paper is a review of the research work that has been carried out over the past few years investigating the ability of the oculomotor system to adapt to prism-induced heterophoria. Our results show that subjects with normal binocular vision can adapt to horizontal and vertical prism-induced heterophorias whether fixating at distance or near. Further studies have shown that subjects with symptomatic abnormal binocular vision have an abnormal adaptation mechanism. Finally, we have found that when orthoptic treatment results in relief from the symptoms, there is an associated improvement in t

This study was performed to test the hypothesis that accommodation and convergence differences precede the onset of myopia in children. Records of children who were emmetropic and became myopic (became myopic group) and children who remained emmetropic (remained emmetropic group) were collected from seven private optometry practices. The practices were located in Illinois, Iowa, Oklahoma, and Oregon. Near-point phorias and positive relative accommodation (PRA) findings were significantly different in the two groups by t-test. Negative relative accommodation (NRA) did not show a statistically s

CLINICAL RELEVANCE: Optometric management of neurodegenerative diseases is essential since visual signs, such as double vision, visual acuity reduction, or oculomotricity dysfunctions, are usually present in these subjects over the course of the disease. The present paper can guide clinicians in better managing their patients with multiple sclerosis.
BACKGROUND: Patients with multiple sclerosis present near vision symptoms that may be related to binocular anomalies, but these symptoms have not been investigated and related to specific signs. The aim of the present study was to evaluate the bi

A number of previous studies have examined the effects of age and the development of presbyopia on either the accommodative convergence to accommodation (AC/A) or convergent accommodation to convergence (CA/C) ratios. However, changes in the interaction of these oculomotor crosslinks with increasing age have received relatively little attention, especially over a wide age distribution. Accordingly, the present study examined both crosslink ratios in 42 subjects ranging from 22 to 65 years of age. A dual haploscope-optometer was used to measure accommodation and vergence. The response AC/A rati

PURPOSE: Accommodation and convergence can adapt to blur and disparity stimuli and to age-related changes in accommodative amplitude. Does this ability decline with age? The authors investigated short-term adaptation to first near-spectacle reading correction on the accommodative-stimulus response (ASR) function, accommodative amplitude (AA), AC/A, and CA/C ratios in a pre-presbyopic and an incipient presbyopic population and determined whether changes in these functions recovered after discontinuation of the use of near spectacles.

Patients with convergence insufficiency (CI) are often symptomatic during activities that require near fixation, such as reading. Indeed, CI has been associated with reading impairment and poor academic performance. However, these associations do not prove a causal relationship between these conditions. The current evidence regarding the relationship between CI and its treatment, reading ability, and academic performance is discussed.

It is well known that in convergence insufficiency (CI) prism adaptation is reduced in response to base-out (BO) prisms at near. There have also been some suggestions in the literature that adaptation is reduced at other distances as well. The present data show that in CI adaptation is not only reduced in response to BO at near, but also in response to base-in (BI) at near and for both BI and BO at distance. This raises the interesting question whether distance adaptation becomes reduced because of reduced near adaptation or whether these subjects have a generally reduced horizontal adaptation

In the absence of an adequate visual stimulus, the eyes are typically converged by approximately 0.25 to 0.75 meter angles (MA). This vergence response (VR) was believed to reflect the level of tonic innervation to the extraocular muscles, and accordingly has been termed tonic vergence (TV). However, this estimation fails to consider the magnitude of the anatomical position of rest. The true typical value of TV is approximately 23 degrees. This paper will consider various aspects of this parameter, including both clinical and laboratory methods of measurement, and the relationship between TV a

Tait described four categories of binocular disorders including convergence excess, convergence insufficiency, divergence excess, and divergence insufficiency. These disorders are defined by the distance where the largest heterophoria occurs (distance or near), and the amplitude of the accommodative vergence ratio (AC/A). Insufficiency corresponds to a low AC/A ratio, whereas excess corresponds to a high AC/A. The magnitude of the AC/A ratio, which may be influenced by the adaptability of the accommodation and vergence systems, has been shown to be reciprocally related to adaptability of accom

To track a small visual target in 3-D space, the two eyes have to move in different directions and/or at different velocities. This tracking might be accomplished by a disjunctive pursuit system, which uses separate motion processing of each individual eye but no disparity signal (hypothesis 1), or by the conjugate pursuit and the vergence system (hypothesis 2). To test the validity of the two hypotheses we recorded eye movements in five healthy human subjects with the scleral search-coil method. A small dim laser stimulus was presented on an earth horizontal platform. A position-ramp stimulus
