<p>Background: Percutaneous cervical cordotomy (PCC) is a lesioning procedure that targets the anterolateral quadrant of the spinal cord to interrupt spinothalamic pain pathways. It provides rapid and durable analgesia for patients with unilateral, medically refractory cancer pain below the C5 dermatome. Although its utilization has declined with the expansion of pharmacological and neuromodulatory therapies, PCC remains a critical palliative intervention for patients in whom opioid therapy is ineffective or intolerable. Summary: This narrative review delineates the neuroanatomical underpinnin

Objective:The aim of this study is to investigate the efficacy of posterior cordotomy with partial arytenoidectomy in bilateral vocal fold paralysis(BVFP). Methods:Medical records of 37 patients with BVFP were retrospectively collected, and they were all admitted to the Department of Otolaryngology-Head and Neck Surgery, West China Hospital of Sichuan University, from Jan. 2019 to Dec. 2023. Preoperative and postoperative evaluations of respiratory function, voice quality, and swallowing function were conducted. Results:Postoperatively, 32 out of 37 patients achieved significant relief in brea

Endoscopic laser cordotomy, also known as Kashima operation, is used for the treatment of bilateral vocal cord palsy where the glottis chink is made posteriorly, sufficient enough for patient to breathe comfortably without any stridor. The objective of the study was to assess outcome of unilateral transverse cordotomy (Kashima operation) for bilateral vocal cord immobility. This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, Bangladesh Medical University, Dhaka, Bangladesh from January 2020 to June 2021 with 12 patients having bilateral v

OBJECTIVES/HYPOTHESIS: Carbon dioxide laser transverse posterior cordotomy (TPC) aims to improve airway aperture in patients with glottic obstruction. Postoperative complications may worsen airway compromise and necessitate additional interventions. We sought to identify factors impacting outcomes after TPC.
STUDY DESIGN: Retrospective chart review.
METHODS: Medical records of patients who underwent TPC for glottic airway obstruction at a tertiary-care medical center between 2008 and 2018 were reviewed. Demographics, comorbidities, and intra- and postoperative management strategies were anal

A 75-year-old man presented to our Interventional Cancer Pain service for consideration of a percutaneous cervical cordotomy (PCC) to control severe chest wall pain secondary to malignant mesothelioma. His medical history included a neuroinflammatory disorder, neuromyelitis optica, for which he had previously had a prolonged hospital admission, with ongoing neurological deficit. Little information is available regarding the safety of PCC in a patient with this condition, specifically the risk of neurological relapse, and we were initially wary about going ahead. After discussion with the patie

PURPOSE: This study aimed to assess the outcomes of posterior cordotomy in cases with bilateral abductor vocal fold immobility (BAVFI), either by radiofrequency or CO2 laser.
METHODS: This prospective comparative randomized study included 80 patients with BAVFI of different etiologies. They were divided randomly into two groups. Group A included 44 patients for whom radiofrequency was used for posterior cordotomy, while the other group (group B) included 36 patients managed by CO2 laser-assisted posterior cordotomy.
RESULTS: The postoperative respiratory chink improved significantly in both

Background/Objectives: Open anterolateral cordotomy (OALC) is a surgical intervention that has been performed to treat patients with persistent pain for more than a century. In recent decades, its application has been reduced in favor of other less invasive treatments. The present article aims to define indications, safety, and the efficacy profile of this procedure for the contemporary neurosurgeon. Methods: A systematic review of articles published from 2010 to 2025 has been performed. Only patients who underwent OALC for cancer pain were included. Results: Eleven articles were included in t

DESIGN: Prospective randomized clinical trial at a university medical center.
SETTING: Zagazig University Hospitals, Zagazig, Egypt.

OBJECTIVE/HYPOTHESIS: A clinical evaluation of CO2 laser endoscopic posterior partial transverse cordotomy (EPPTC) in patients with severely compromised airway due to bilateral paralysis of the vocal fold.
STUDY DESIGN: An inception cohort of 25 patients over a 10-year period.
METHODS: The CO2 laser EPPTC was unilateral in 15 patients and bilateral in 10. Variables were tested for potential statistical relation to successful rehabilitation of the airway.
RESULTS: The use of the CO2 laser never resulted in adverse side effects. Complications were not encountered. A one-step, successful resto

BACKGROUND: Vocal cord paralysis is a rare cause of obstructive sleep apnea syndrome (OSAS). Recurrent laryngeal nerve injury after thyroid gland surgery is one of the leading causes of acquired vocal cord paralysis.
REPORT: A 46-year-old woman with OSAS due to bilateral abductor vocal cord paralysis was presented. She had thyroidectomy 30 years ago and had a weak, breathy voice. She had been referred with a history of high-pitched snoring, apnea witnessed by her spouse, and excessive daytime sleepiness for the last 5 years. Full-night polysomnography revealed that her apnea-hypopnea index wa

BACKGROUND: Cancer-related pain remains one of the most debilitating symptoms in patients with advanced malignancy, with a significant proportion experiencing pain refractory to standard medical therapy. Percutaneous cordotomy is an established intervention targeting the spinothalamic pathway and can provide meaningful relief in appropriately selected patients; however, its use has declined and is typically performed in awake patients.
CASE PRESENTATION: A 54-year-old woman with metastatic gallbladder carcinoma presented with severe, unilateral, nociceptive abdominal pain refractory to medica

Objective:To analyze the efficacy of endoscopic CO₂ laser arytenoidectomy in treating bilateral vocal cord paralysis. Methods:Ninety-five patients who underwent endoscopic CO₂ laser arytenoidectomy for bilateral vocal cord paralysis at the First Affiliated Hospital of Naval Medical University(Changhai Hospital) of Shanghai from January 2009 to December 2022 were included in this study. Among them, 59 patients underwent endoscopic CO₂ laser arytenoidectomy as their first glottic enlargement surgery(Group A), while 36 patients, who had previously undergone two unsuccessful CO₂ laser-assisted pos

PURPOSE: To describe the surgical and functional outcomes of modified posterior cordotomy in patients with bilateral vocal fold paralysis (BVFP), with particular attention to airway function, swallowing, and voice outcomes.
METHODS: This retrospective study included 30 patients with BVFP who underwent modified posterior cordotomy between June 2017 and August 2024. Preoperative and postoperative evaluation included videolaryngoscopy and patient-reported outcome measures, including the Voice Handicap Index (VHI-30), Eating Assessment Tool-10 (EAT-10), and Airway-Dysphonia-Voice-Swallowing (ADVS

The aim of this prospective study was to analyse airway improvement and acoustic and aerodynamic parameters after CO2 laser posterior transverse cordotomy (LPTC) in bilateral vocal fold paralysis (BVFP). Four patients (2 males, 2 females) were recorded pre- and post-operatively at 1, 3, 6, 12 and 24 months. Forced inspiratory volume during the first second (FIV), vital capacity, peakflow, and forced expiratory volume during the first second (FEV) were measured with the Gould II spirometer. Acoustic frequency features (average fundamental frequency, standard deviation, jitter, shimmer and harmo
