Deep stromal opacity after corneal cross-linking.

PURPOSE: To describe 3 cases with deep corneal stromal opacity that occurred several months after corneal cross-linking. METHODS: A 36-year-old man, a 19-year-old man, and a 14-year-old girl underwent corneal cross-linking for their progressive keratoconus. Corneal cross-linking was performed according to the Dresden protocol. The corneal epithelium was ablated using an excimer laser in 2 cases and manually in 1 case. After 30 minutes of riboflavin presoaking, hypotonic riboflavin solution was applied until the corneal stroma swelled, after which the eyes were exposed to ultraviolet irradiati

Current status of accelerated corneal cross-linking.

Corneal cross-linking with riboflavin is a technique to stabilize or reduce corneal ectasia, in diseases such as keratoconus and post-laser-assisted in situ keratomileusis (LASIK) ectasia. There is an interest by patient as well as clinicians to reduce the overall treatment time. Especially, the introduction of corneal cross-linking in combination with corneal laser surgery demands a shorter treatment time to assure a sufficient patient flow. The principles and techniques of accelerated corneal cross-linking is discussed.

[Cross-linking and intrastromal corneal ring segment].

Corneal cross-linking is a procedure used for stabilizing the cornea in patients with progressive keratoconus by increasing corneal rigidity, and it is also used in corneal inflammatory melting process. The intrastromal corneal ring segments act by flattening the center of the cornea. Originally designed for the correction of mild myopia, the segments are now being used for reduction of keratoconus in order to improve the uncorrected visual acuity, the best spectacle corrected visual acuity, to allow good tolerance to the use of contact lenses and delay the need for corneal grafting procedures

Corneal cross-linking-induced stromal demarcation line.

PURPOSE: Corneal collagen cross-linking by UVA/riboflavin (X-linking) represents a new method for the treatment of progressive keratoconus and currently is under clinical study. To avoid UVA irradiation damage to the corneal endothelium, the parameters for X-linking are set in a way that effective treatment occurs only in the first 300 microm of the corneal stroma. Here, X-linking not only strengthens the biomechanical properties of the cornea but also induces keratocyte apoptosis. To date, the effectiveness of treatment could be monitored only indirectly by postoperative follow-up corneal top

Predicting the Effects of Customized Corneal Cross-Linking on Corneal Geometry.

PURPOSE: To validate an existing finite element model (FEM) for predicting the flattening effect of corneal cross-linking (CXL) in a clinical scenario and to use this model to investigate the parameters that most influence CXL-induced flattening effects. METHODS: Retrospective data were collected from two clinical cohorts, each with 20 patients receiving either standard or customized CXL. Data were collected before surgery and at the six-month follow-up. Both CXL treatments were simulated with a FEM calibrated on experimental data. Standard anterior corneal geometry indexes (e.g., sphere, cyl

Case Report: Acute Corneal Hydrops despite Accelerated Transepithelial Corneal Cross-linking.

SIGNIFICANCE: Corneal cross-linking is a well-established treatment to reduce or halt the progression of keratoconus. This case demonstrates that acute corneal hydrops may develop several years after corneal cross-linking even despite apparent treatment effect with corneal flattening, and patients should be monitored accordingly. PURPOSE: This study aimed to describe a case of continued progression in the form of corneal hydrops despite prior corneal cross-linking and evidence of treatment effect. CASE REPORT: A 19-year-old woman underwent bilateral accelerated transepithelial corneal cross-
