Safety in numbers? Tackling domestic abuse in couples and network therapies.

Family, network or couples-based therapies have been helping to support people with substance problems for decades. Their value in supporting a person to change their alcohol or drug use is clear. However, as links between substance use and domestic abuse are increasingly recognised, these approaches need to reflect on the potential safety risks they present to people taking part. The prevalence of domestic abuse among people receiving drug and alcohol services is considerably higher than general population estimates, yet this does not appear to have been adequately addressed in network therap

A comparison of psychosocial adjustment among children of women who received behavioral couples therapy versus individually based therapy for substance use disorder.

BACKGROUND: Children of women with substance use disorder (SUD) exhibit elevated risk for psychosocial adjustment problems. However, little research has examined whether women's SUD treatment is beneficial for their children. In comparison to individually based therapy (IBT), behavioral couples therapy (BCT) is superior for reducing women's SUD problems and improving their intimate partner relationships. The current study sought to examine whether BCT is more efficacious than 12-step-oriented IBT for reducing psychosocial adjustment problems among the children of women with SUD. METHODS: The

Employment Stress and Couple Adjustment among Clients With Disorders of Gambling and Alcohol Use: Themes of Transfers in Congruence Couple Therapy.

BACKGROUND: Individuals with problematic gambling, alcohol and substance use commonly report lower employment rates and more employment-related problems such as job loss, work conflicts and poor performance. METHOD: A thematic qualitative analysis was conducted to extract employment-related themes from 21 sets of addiction counselors' case notes of couple therapy sessions (average 10 sessions per case) from a randomized controlled trial of Congruence Couple Therapy (CCT). Case notes were examined for the types of employment issues to answer the research question: What are the interconnections

Antioxidant therapy for infertile couples: a comprehensive review of the current status and consideration of future prospects.

Oxidative stress (OS) is established as a key factor in the etiology of both male and female infertility, arising from an imbalance between reactive oxygen species (ROS) production and the endogenous antioxidant (AOX) defenses. In men, OS adversely affects sperm function by inducing DNA damage, reducing motility, significantly impairing sperm vitality through plasma membrane peroxidation and loss of membrane integrity, and ultimately compromising overall sperm quality. In women, OS is implicated in various reproductive disorders, including polycystic ovary syndrome, endometriosis, and prematur

The Unaddressed Needs of Alcohol-Using Couples on Antiretroviral Therapy in Malawi: Formative Research on Multilevel Interventions.

Alcohol use among HIV-positive individuals in sub-Saharan Africa directly impacts adherence to antiretroviral therapy and HIV outcomes. Few studies have examined approaches to reduce alcohol use among HIV-affected couples, despite evidence that alcohol use is a couple-level concern. We conducted a qualitative study with 23 alcohol-using couples to identify multilevel barriers and facilitators of alcohol use, and potential intervention options with couples. Data were analyzed at individual and dyadic levels using framework analysis. All couples were married and had at least one partner on ART.

Do outcomes after behavioral couples therapy differ based on the gender of the alcohol use disorder patient?

OBJECTIVE: This naturalistic study (conducted from 1992 to 1998) of behavioral couples therapy (BCT) compared female and male alcohol use disorder (AUD) patients on improvement and on drinking and relationship outcomes after BCT. We also evaluated gender differences on presenting clinical problems and extent of BCT participation. METHOD: Participants were 103 female and 303 male AUD patients (98.5% alcohol dependence, 1.5% alcohol abuse) and their heterosexual partners, mostly White in their forties. Couples received 20-22 BCT sessions over 5-6 months. Drinking outcomes were percentage days a

Natural Conception is Safe for HIV-Serodiscordant Couples with Persistent Suppressive Antiretroviral Therapy for the Infected Partner.

Background: Sustained use of antiretroviral treatment to achieve a suppressed viral load in persons living with HIV is associated with zero or near-zero risk of sexual and vertical HIV transmission. This has led to an increasing number of HIV-serodiscordant couples (SDCs) who wish to have children. The aim of this study was to describe the most recent results of a protocol for reproductive counseling directed at HIV-SDCs who desire natural conception and to identify some of the factors influencing reproductive success. Methods: Two hundred fourteen couples were enrolled. Sociodemographic/behav

The application of behavioral couples therapy to the assessment and treatment of agoraphobia: implications of empirical research.

The present review was undertaken to examine the literature regarding the role of the marital relationship in treatment of agoraphobia from a behavioral couples therapy (BCT) perspective. Both qualitative and quantitative analyses were conducted to evaluate (a) whether marital quality predicts treatment responsiveness and (b) how best to include the spouse in treatment of agoraphobia. Regarding the assessment of agoraphobics' relationship quality, it is recommended that subjectively experienced "satisfaction" with the relationship be distinguished from more descriptive measures of general and

Left atrioventricular coupling index in patients undergoing cardiac resynchronization therapy.

The left atrioventricular coupling index (LACI), defined as the ratio of left atrial (LA) to left ventricular (LV) end-diastolic volumes, serves as a surrogate for atrioventricular coupling. Cardiac resynchronization therapy (CRT) promotes reverse remodeling of both chambers and may enhance atrioventricular coupling, yet the clinical utility of LACI in CRT patients remains unclear. This study evaluated the effect of CRT on LACI and assessed its prognostic value for CRT response. In 123 patients enrolled in the ADVISE-CRT III trial (mean age 66 ± 10 years; 63% male; and 81% with a Class I CRT i

Perspectives on photothermal therapy to enhance osteogenesis-angiogenesis coupling in bone repair.

Recent attention has focused on bone's highly vascularized characteristics to improve bone defect repair, an interplay referred to as osteogenesis-angiogenesis coupling. Regarding this point, it is promising to utilize photothermal therapy (PTT) to enhance the coupling, with increasing research in the field. However, the application models, mechanisms, and therapeutic efficacy remain underexplored. Previous reviews have mainly addressed the general osteogenic applications of PTT. This review instead focuses on how PTT regulates the spatiotemporal coordination of osteogenesis and angiogenesis d

Gonadotropins in the Management of Couple Infertility: Toward the Rational Use of an Empirical Therapy.

Although epidemiology shows that both men and woman can experience infertility, the female partner usually experiences most of the diagnostic and therapeutic burden. Thus, management of couple infertility is a unique example of gender inequality. The use of exogenous gonadotropins in assisted reproductive technology (ART) to induce multifollicular growth is well consolidated in women, but the same is not done with the same level of confidence and purpose in infertile men. Indeed, the treatment of idiopathic male infertility is based on an empirical approach that involves administration of the

Endothelial TRPV4-eNOS coupling as a vital therapy target for treatment of hypertension.

BACKGROUND AND PURPOSE: Reduced NO levels and activity are signs of endothelial dysfunction, which is important in mediating BP changes. Previously, we demonstrated that transient receptor potential channel V4 (TRPV4) could form a functional complex with other proteins to mediate vasodilation in endothelial cells (ECs). But how TRPV4 interacts with the NO pathway in larger arteries requires further exploration. EXPERIMENTAL APPROACH: We used single-cell RNA-sequencing to find the CD106+ TRPV4high NOS3high ECs. The TRPV4-eNOS interaction was verified by co-immunoprecipitation and immuno-FRET,

A randomized clinical trial of a brief, problem-focused couple therapy for depression.

The aim of this study was to evaluate a brief couple therapy for depression targeted for mildly discordant or nondiscordant couples struggling with the negative impact of depression. Subjects included women with major depression or dysthymia who had husbands without clinical depression. Thirty-five couples were randomly assigned to the 5-week intervention (n=18) or a waitlist control group (n=17), and followed up 1 and 3 months later. Results showed a significant effect of treatment in reducing women's depressive symptoms, with 67% of women improved and 40% to 47% recovered at follow-up, compa

A pilot and feasibility study of Sage: A couple therapy for borderline personality disorder.

Objective: Borderline personality disorder (BPD) is characterized by emotion dysregulation and relationship dysfunction. However, the majority of BPD treatments provide individually-focused treatment and do not target relationship functioning or include significant others. The current paper presents the primary and secondary outcomes from an uncontrolled trial of Sage, a manualized conjoint treatment for people with BPD and their intimate partners that targets BPD, relationship conflict, and partner mental health simultaneously. Method: Sixteen couples wherein one member has BPD and elevated s

Reversible infertility associated with testosterone therapy for symptomatic hypogonadism in infertile couple.

PURPOSE: Androgen replacement therapy has been shown to be safe and effective for most patients with testosterone deficiency. Male partners of infertile couples often report significantly poorer sexual activity and complain androgen deficiency symptoms. We report herein an adverse effect on fertility caused by misusage of androgen replacement therapy in infertile men with hypogonadal symptoms. MATERIALS AND METHODS: The study population consisted of 8 male patients referred from a local clinic for azoospermia or severe oligozoospermia between January 2008 and July 2011. Screening for physical violence in couple therapy: methodological, practical, and ethical considerations.

Over two-thirds of clients in family therapy clinics engage in some form of physical violence against their partners within the year prior to the initiation of therapy. However, family therapists are aware of only a small proportion of these cases. The purpose of this article is to enhance the ability of family therapists to detect marital violence. We examine client and therapist reasons for why physical violence is not detected. We then review various methods to screen for the presence of physical violence, especially the Conflict Tactics Scales. Finally, indicators of life-threatening viole

Childhood sexual abuse as a predictor of adult female sexual dysfunction: a study of couples seeking sex therapy.

The present study investigated 359 married adult women who sought sex therapy with their spouses. Discriminant function analyses indicated that childhood sexual abuse plus a college education significantly discriminated between women with and without a diagnosed sexual dysfunction. Among abused women, abuse involving sexual penetration significantly discriminated between dysfunctional and nondysfunctional women. Current findings confirm previous theory and research regarding a connection between childhood sexual abuse and adult female dysfunction. Furthermore, the findings suggest that abuse i
