[Two cases of Turner's syndrome with spondyloepiphyseal dysplasia like bone appearance].

We report two cases of mosaic karyotype (45XO/46XiXq) Turner's syndrome with unique bone appearance. The cases were 44 and 34 year-old women and latter was complicated by Hashimoto's thyroiditis (hypothyroidism). Following the systemic bone surveys, we found the patients showed not only osteoporotic bone change and short stature, but also spondyloepiphyseal dysplasia (SED) like bone appearance (thinness of vertebral bodies, irregularity of vertebral end-plates, shortness of femoral necks, Coxa valga, Coxa magna and hypoplasia of acetabula). Those findings can not be explained by degenerative b

Characterization of hip morphology phenotypes in individuals with femoroacetabular impingement syndrome with cam morphology: Data from the PhysioFIRST trial.

OBJECTIVE: To characterize different hip morphology phenotypes in individuals with femoroacetabular impingement (FAI) syndrome with cam morphology. MATERIALS AND METHODS: Supine pelvic anterior-posterior radiographs from 119 hips from 119 individuals diagnosed with FAI syndrome with cam morphology from a RCT were analysed. The lateral center edge angle (LCEA), Wiberg center edge angle (W-CEA), global retroversion, protrusio acetabuli, and femoral neck shaft angle (FNSA) measures were assessed independently by two raters. Criteria from the Lisbon Agreement on FAI imaging were used to character

[Bilateral recurrent anterior dislocation of the hip (author's transl)].

The authors report a case of bilateral recurrent anterior dislocation of the hip in a child suffering from Seckel's disease (bird-headed dwarf). There were no neurological abnormalities. The dislocations appeared to relate to coxa valga and dysplastic acetabula. Femoral osteotomies together with a shelf procedure were performed. The dislocations had ceased when the child was reviewed in a two-year follow-up.

Intra-articular acetabular osteochondroma in patients with multiple hereditary exostoses.

We report three additional cases of intra-articular acetabular osteochondroma in multiple hereditary exostoses patients in order to raise the awareness of this rare location, to prompt early diagnosis, and to present various treatment options according to the patient's condition. A 2.5-year-old boy presenting with an out-toeing gait had a large acetabular osteochondroma causing lateral displacement of the femoral head and acetabular dysplasia. Mass excision through hip subluxation via anterior approach and concomitant Dega osteotomy resulted in a congruent, well-developed, and well-covered hip

Hereditary multiple exostoses of the hip.

PURPOSE: To assess the radiographic features of 36 hips with hereditary multiple exostoses (HME). METHODS: Hip parameters of 12 males and 6 females (36 hips) aged 2 to 28 years with HME were assessed using anteroposterior radiographs. The recorded features included the sites of osteochondromas, the femoral head/neck ratio, the Reimer's migration percentage, Sharp's acetabular angle, the centre edge angle, the femoral neck-shaft angle, and degenerative changes. RESULTS: 15 of the 18 patients were asymptomatic; 3 complained of pain (2 underwent excision or bone biopsy); no lesion was malignant

[A unique case of pseudohypoparathyroidism].

A case of pseudohypoparathyroidism is reported. The case presents with unusual features: primary hypogonadism (a rare feature) and several features undescribed until now with the syndrome--changes in the amino acid composition, coxae valgae, shallow fossa olecrani as well as normal values of the parathyroid hormones which is considered as a "phase state" due to the exhaustion of the secreting function of the parathyroid glands. The role of the G-unit in the etiology of the disease is discussed.

Bilateral varus osteotomies in hip deformities: are early interventions superior? A long-term follow-up.

Coxa valga (with or withour excessive femoral anteversion) combined with acetabular dysplasia is a well-known cause of early osteoarthritis. Many authors have stated that the best result of an osteotomy can be achieved at an early stage of these osteoarthritic changes. In this study, we present 26 patients with a symmetrical hip deformity for which we performed a therapeutic osteotomy on the symptomatic hip. The contralateral hip had the same anatomical predisposition to develop an OA, but there were only minor to no complaints. We advised and performed an early osteotomy on these hips. On rad

Mechanobiological predictions of growth front morphology in developmental hip dysplasia.

Developmental dysplasia of the hip (DDH) is the most common orthopedic problem of newborn children. Most clinicians and researchers agree that the primary cause of DDH is abnormal mechanical forces on the head of the femur due to limb position, pressure from the womb, or ligament laxity. The abnormal mechanical forces result in altered growth and bony deformities, in particular large neck-shaft and anteversion angles in the proximal femur and a shallow acetabulum. Previous studies have suggested that intermittent octahedral shear stress promotes growth and ossification, while intermittent hydr

Osteochondritis dissecans of the femoral head.

Seven patients with symptomatic osteochondritic lesions of the femoral head are presented. All were male with a mean age of 26 years (16 - 33 years). Two distinct morphologic appearances of the hip joint could be identified. Five patients presented with a coxa valga deformity, four of whom had signs of epiphyseal dysplasia. There were 2 patients whose hips appeared normal apart from the osteochondrontic lesions. In both cases an additional acetabular rim lesion due to a reproducible femoro-acetabular impingement was diagnosed at arthrotomy. This may have acted as the underlying cause of osteoc

[Biomechanical aspects of hip dislocation].

The stress distribution in the subluxated hip joint is analyzed by means of a two-dimensional computer model. Luxation becomes manifest if the action line does not intersect with the contour of the socket. Pathological stress with overloading of the bone and cartilage tissue appear if the articular resultant force is shifted towards the acetabular edge. The important parameter is the CE angle. The coxa valga per se has no pathological meaning as long as the femoral head is covered sufficiently by the acetabular roof.

Progeria infantum (Hutchinson-Gilford syndrome) associated with scleroderma-like lesions and acro-osteolysis: a case report and brief review of the literature.

Progeria infantum (Hutchinson-Gilford syndrome) is a very rare syndrome of premature aging characterized by growth retardation and specific, progressive, premature senescent changes of the skin and other tissues. We report a 1.5-year-old girl with loss of scalp hair, eyebrows, and lashes, prominent scalp veins, micrognathia, abnormal ears, loss of subcutaneous tissue, and scleroderma-like areas over the trunk. Radiographic studies revealed coxa valga and acro-osteolysis of the terminal phalanges. The clinical and radiologic features corresponded well with progeria infantum.

Femoral head osteochondritis dissecans in a child.

This case report describes an early-adolescent boy with an osteochondritis dissecans (OCD) lesion of the left femoral head secondary to significant acetabular dysplasia and coxa valga of the proximal femur. Patient underwent left proximal femur varus osteotomy. Follow-up imaging demonstrates healing and resolution of the OCD lesion. Future plan for left hip is periacetabular osteotomy, following triradiate cartilage closure, to correct acetabular dysplasia. The aim of this case report is to support clinicians in the assessment and treatment of this rare condition.

Valgus slip of the capital femoral epiphysis.

Valgus slip of the capital femoral epiphysis, a rare entity which may be acute or chronic, occurred in a 12-year-old boy. The displacement may be associated with a pre-existing coxa valga. The patient also had a varus slip of the capital femoral epiphysis on the contralateral side. Both hips were successful treated by in situ pinning. A review of the English literature reveals only 8 previously reported cases of valgus slipping of the capital femoral epiphysis. Either valgus or lateral slips are unusual.

Fracture dislocation of sacroiliac joint associated with triradiate cartilage injury in a child: a case report.

We report a very rare injury of a 8-year-old girl with sacroiliac fracture dislocation and triradiate cartilage separation. After the restoration of the sacroiliac joint by open means, reduction of the separated cartilage was seen. At 20 months follow-up, an osseous bridging at the triradiate cartilage and mild coxa valga deformity developed. We think that every child with serious sacroiliac joint injury should be evaluated for associated triradiate cartilage injury and followed to skeletal maturity for late complications such as acetabular dysplasia, hip subluxation and pelvic asymmetry.

Complications of fracture of the neck of the femur in children. A long-term follow-up study.

This is a long-term study of 53 children with fracture of the proximal femur. The age of these children at the time of injury ranged from 3 to 16 years (average, 10.2 years). The follow-up period averaged 9.4 years (range, 5-20 years). There was a single transepiphyseal fracture, 28 transcervical fractures, 21 basicervical fractures and three intertrochanteric fractures. The overall results were good in 49, fair in 25 and poor in 26% of patients. Better results were obtained in undisplaced fractures and in those displaced fractures where anatomical reduction was achieved and maintained through

The radiological pattern associated with the trisomy of the short arm of chromosome No 4.

A fairly constant constellation of roentgen anomalies has been detected in the skeleton of 9 patients with the trisomy of the short arm of chromosome No 4. The more common changes involve the skull, spine and pelvis, while the limbs, as well as the internal organs, are less affected. Microcephaly, hypertelorism, small and closed sella, malocclusion, scoliosis, "square" vertebral bodies, narrow iliac wings, wide iliac angles and narrow acetabular ones, coxa valga, medial recumbency of the proximal tibial epiphysis and retarded bone age are usual findings.

Hip disease in Hutchinson-Gilford progeria syndrome.

Two cases of Hutchinson-Gilford progeria syndrome are presented with a focus on hip disease. A severe coxa valga is the first abnormality. The femoral head becomes increasingly uncovered as the acetabulum becomes more dysplastic. The center edge angle decreases, the acetabular index increases, and the medial wall of the acetabulum widens. Hip pain, subluxation, and eventually dislocation are the sequelae of these changes. Late osteotomy is unpredictable; thus the best therapeutic regimen is early osteotomy (age 4-6 years) and a regular exercise program to maintain muscle strength and range of

Distinctive metaphyseal chondrodysplasia with severe distal radius and ulna involvement (upper extremity mesomelia) and normal height.

Metaphyseal chondrodysplasias (MCD) are skeletal disorders characterized by metaphyseal irregularities and, usually, by short stature. In MCD, wide heterogeneity exists with regard to clinical and radiological changes. We report on a patient with clinical and radiological findings of MCD who had coxa valga and normal height with metaphyseal involvement of the long bones. The short radii and ulnae showed a very severe change in their distal metaphyses, leading to mesomelic shortening confined to the upper limbs. Hematological, ophthalmological, and hearing examinations were normal. This type of

Occipital horn syndrome: report of a patient and review of the literature.

We report an 18-year-old boy with occipital horn syndrome and we review the 20 cases previously published with this syndrome. The distinctive features common to all patients were unusual facial appearance, skeletal abnormalities, chronic diarrhea and genitourinary abnormalities. The skeletal abnormalities included occipital horns, short, broad clavicles, deformed radii, ulnae, and humeri, narrowing of the rib cage, undercalcified long bones with thin cortical walls and coxa valga. Occipital horn syndrome is inherited in an X-linked recessive fashion. Our analysis indicates that occipital horn
