Clinical, radiographic, diagnostic and cephalometric features of pycnodysostosis in comparison with Turkish cephalometric norms: A case report.

Pycnodysostosis is a rare genetic disorder and was first described in 1962 by Maroteaux and Lamy. The incidence of this anomaly is estimated to be 1.7 per 1 million births. The principal characteristics of this disorder are short stature, prominent eyes with blue sclera, beaked nose, cranial dysplasia, exposed fontanelles and cranial sutures, clavicular dysplasia, total/partial dysplasia of the terminal phalanges, obtused mandibular gonial angle, and generally increased bone density. Some features of pycnodysostosis are similar to osteopetrosis and cleidocranial dysostosis. Therefore, it must

Physiology and Clinical Manifestations of Pathologic Cranial Suture Widening.

Cranial sutures are complex structures integrating mechanical forces with osteogenesis which are often affected in craniofacial syndromes. While premature fusion is frequently described, rare pathological widening of cranial sutures is a comparatively understudied phenomenon. This narrative review aims to bring to light the biologically variable underlying causes of widened sutures and persistent fontanelles leading to a common outcome. The authors herein present four syndromes, selected from a literature review, and their identified biological mechanisms in the context of altered suture physi

Cranial ultrasound as a diagnostic tool for infant meningitis in a low-or middle income setting: "sensitivity and specificity".

BACKGROUND: Cerebrospinal fluid (CSF) analysis remains the gold standard for diagnosing meningitis but it requires technique and time. In infants, open fontanels allow ultrasound visualization, making cranial ultrasound a low-cost, point-of-care diagnostic tool for suspected meningitis. This study aimed to develop a standardized cranial ultrasound checklist, and to assess the sensitivity and specificity of cranial ultrasound as a diagnostic tool for infants with meningitis compared with the gold standard cerebrospinal fluid (CSF) analysis. MATERIALS AND METHODS: This diagnostic tool validatio

Choroid plexus papilloma in a 4-month-old child: a case report.

We report a case of choroid plexus papilloma arising from the right lateral ventricle in a 4-month-old infant. Because of intermittent vomiting, abdominal pain, and diarrhea, the child was initially treated with intravenous rehydration, which aggravated the symptoms, and a bulging fontanel developed. Before a planned lumbar puncture, cranial ultrasound was performed, which disclosed a choroid plexus papilloma. The tumor was removed surgically, and the child made an uneventful recovery. We discuss the diagnostic value of cranial ultrasound compared to computed tomographic (CT) scan and angiogra

Peritrigonal echogenic "blush" on cranial sonography: pathologic correlates.

Cranial sonography in neonates almost always reveals a hyperechoic "blush" just posterior and superior to the ventricular trigones on parasagittal views. This normal increased echogenicity resembles fine brush strokes. It is probably caused by the interface of numerous parallel fibers that are nearly perpendicular to the longitudinal axis of a sonographic beam passing through the anterior fontanelle. The same echogenicity is not seen on sonograms obtained through the posterior fontanelle because with that angulation the long axis of the sonographic beam and the fiber tracts are nearly parallel

Association of Extreme Brachycephaly With Persistent Fontanelles in Adult Chihuahuas.

BACKGROUND: Although persistent fontanelles (PFs) are common in adult Chihuahuas, their association with cranial morphology remains unknown. OBJECTIVES: To identify whether cranial morphology is associated with PFs in Chihuahuas and if bodyweight is associated with cranial morphology in this breed. ANIMALS: Fifty client-owned Chihuahuas. METHODS: In this retrospective cross-sectional study using computed tomography images, we measured two different cranial base lengths (1 and 2), cranial length, height, and width, and two craniofacial angles. We calculated the ratios of cranial height to cr

Cerebellar hemorrhage in extremely low birth weight siblings: is there a familial disposition?

Cerebellar hemorrhage is an underrecognized complication in the preterm neonate. It is multifactorial including combined maternal, intrapartum, and early postnatal factors. We present the case of 2 preterm brothers, 24 + 1 and 24 + 3 weeks of gestation, who both died because of cerebellar hemorrhage. We sought familial pathogenic factors predisposing to cerebellar hemorrhage. Cerebral imaging performed by ultrasonography through the anterior fontanel was normal and showed no signs of bleeding or brain edema. Postmortem neuropathologic findings confirmed cerebellar hemorrhagic lesions in both i

Massive Wormian bone at the cranial apex: identification, correction and outcome.

We present the case of a three-month old African American female who presented with abnormal head shape and alopecia areata in the overlying skin. The patient underwent a subtotal cranial vault reconstruction at ten months of age. At surgery, a 7cm by 8cm Wormian bone was found at the cranial apex involving the coronal and sagittal sutures, obliterating the anterior fontanelle. In its center was an epidermoid cyst projecting intracranially between the cerebral hemispheres, which was also safely removed. At this time she has reached her developmental milestones without deficiency at three years

Evaluation of fontanel size variation and closure time in children followed up from birth to 24 months.

OBJECTIVE Anterior fontanel size and closure time can be useful clinical signs for the early diagnosis of diseases. Knowing age-related normal variations is important to decide on further investigations. The authors of this study aimed to describe fontanel size variation according to age, determine fontanel closure time, and identify the optimal method for assessing fontanel size. METHODS For this prospective cohort study, 321 healthy term babies who fulfilled the inclusion criteria were consecutively recruited. Percentile charts for the mean anterior fontanel size and oblique measurements fro

Persistent Metopic Suture: Incidence and Clinical Correlates in a Large Southeast Asian Population.

The metopic suture usually fuses by age 2, but persistence beyond this age (metopism) is uncommon and may mimic a fracture or signal disordered craniofacial development. The authors assessed the incidence of persistent metopic sutures and associated demographic and clinical factors. A retrospective study was organised of cranial CT scans of patients aged 24 months or older at Siriraj Hospital, Bangkok, Thailand, classifying metopic patency with the Lottering system and defining persistence as any patency beyond 24 months. The authors collected data on age, sex, cephalic index, imaging indicati

Case series of infants presenting to the emergency department with fever, bulging anterior fontanelle and COVID-19 infection.

We present a three patient case series of infants who presented to the pediatric emergency department with fever, bulging anterior fontanelle (BAF), and an omicron variant COVID-19 infection. All patients had a benign course, none developed meningitis, and all had symptom resolution after two days. Considerations for neuroimaging and lumbar puncture are discussed. This case series adds to the previously published case reports of infants with COVID-19, fever and BAF and further describes a variant in the presenting symptomology of COVID-19 infection in infants under 12 months. Acute and primary

Anterior fontanelle closure and diagnosis of non-syndromic craniosynostosis: a comparative study using computed tomography.

OBJECTIVE: Suspicion of early anterior fontanel (AF) closure is a common reason for referral to a pediatric neurosurgeon because of the suspected increased risk of developing craniosynostosis (CS) in spite of the absence of evidence in the literature. The aim of this study was to analyze the association between AF closure and the diagnosis of non-syndromic CS in Brazilian children. METHODS: An observational and case-cohort study was conducted to compare the incidence of closed AF between healthy children (group 1) and children diagnosed with non-syndromic CS (group 2) at a pediatric neurosurg

HPGD mutations cause cranioosteoarthropathy but not autosomal dominant digital clubbing.

Cranio-osteoarthropathy, clinically classified as a variant of primary hypertrophic osteoarthropathy, is a very rare autosomal-recessive condition characterized by delayed closure of the cranial sutures and fontanels, digital clubbing, arthropathy, and periostosis. Recently, mutations in the gene HPGD, which encodes the NAD(+)-dependent 15-hydroxyprostaglandin dehydrogenase, were reported in four families affected with primary hypertrophic osteoarthropathy and one family with autosomal-recessive isolated nail clubbing. We report the clinical and molecular findings in four patients from two fam

The open fontanelle: a window to less radiation.

INTRODUCTION: Over the years, there has been increasing awareness of the radiation doses from medical investigation and treatment modalities and the long-term effect of this radiation. In no other patient population is this more of an issue than in the paediatric population who are more radiosensitive and have a longer life span in which to express any negative effects of radiation. In children under the age of one, the anterior fontanelle is an acoustic window to intracranial structures allowing for the use of cranial ultrasound (CRUSS) instead of CT. AIM: To determine the pattern of CT and

Unusual presence of Wormian (sutural) bones in human skulls.

Wormian bones are a subset of the small intrasutural bones that lie between the cranial sutures formed by the bones of the skull vault. They are formed due to additional ossification centres in or near sutures. They are usually considered as normal variants and seem to be determined genetically in certain populations. They have been linked with rapid cranial expansion as they appear in great number in hydrocephalic skulls. They are commonly found in the lambdoid suture and fontanelles but are occasionally seen in other sutures especially the coronal, squamosal, and sagittal sutures. We examine

Craniomicromelic syndrome: first report in a male.

We report on craniomicromelic syndrome in a male fetus. This case had the previously reported features of prenatal onset growth retardation, underossified cranial bones, wide sutures and fontanels, small face as compared to head, small palpebral fissures, pinched nose, microstomia, micrognathia, and narrow thorax. The consistent combination of these features with short appearing limbs as observed in this case establishes this syndrome as a distinct entity.
