Spring-assisted cranial vault expansion in the setting of multisutural craniosynostosis and anomalous venous drainage: case report.

Patients with multisutural craniosynostosis can develop anomalous venous connections between the intracranial sinuses and cutaneous venous system through enlarged emissary veins. Cranial vault remodeling in this subset of patients carries the risk of massive intraoperative blood loss and/or occlusion of collateral draining veins leading to intracranial venous hypertension and raised intracranial pressure, increasing the morbidity of cranial expansion. The authors report the use of spring-mediated expansion as a technique for cranial reconstruction in which the collateral intracranial venous dr

[Cranial epidural abscesses: a case report].

OBJECTIVE AND IMPORTANCE: Cranial epidural abscess mostly follows sinusitis or mastoiditis, also open cranial injury or cranial surgery with iatrogenic infection. In our case the abscess possibly evolved from epidural haematoma and the process invaded frontal sinus and subgaleal space in the frontal region. CLINICAL PRESENTATION: A 22-year-old male after motorcycle accident 4 years ago, with signs of sinusitis frontalis and seizures, with CT confirmed epidural abscess. INTERVENTION: Craniotomy was performed and the abscess totally removed. Homogeneous bone was replaced at the operation. Good

Fenestrated Falx Cerebri and Additional Sinuses in the Tentorium Cerebelli.

Morphological variations of falx cerebri and tentorium cerebelli are extremely rare. The authors report an extremely rare type of combined variation of falx cerebri and dural venous sinuses in the tentorium cerebelli. During the removal of the brain from cranial cavity, it was noted that the anterior part of the falx cerebri had fenestrations and it looked like a mesh. There was no associated variations of medial surface of cerebrum. Further, there were unusual sinuses within the tentorium cerebelli. Two of them were present in the right half of the tentorium cerebelli and one in the left half

Comparative endocranial vascular changes due to craniosynostosis and artificial cranial deformation.

The processes of craniosynostosis (premature fusion of one or more of the calvarial sutures) and artificial cranial deformation are similar since both can alter the shape of the craniofacial complex. Most research exploring these processes has focused on the ectocranium, although it is obvious that these processes also modify the endocranium. Endocranial changes due to either craniosynostosis or artificial cranial deformation have not been as thoroughly examined. Silicone rubber endocasts were made from 11 craniosynostotic archaeologically derived specimens from North and South America. For co

Diplopia Rhinoceros.

Bilateral cranial nerve VI palsy is a rare neuro-ophthalmological presentation often associated with systemic or intracranial pathologies. In diabetic patients, cranial neuropathies can occur due to immune suppression, ischemia, or infectious complications. This case highlights the interplay of poorly controlled type 2 diabetes mellitus (T2DM), sinusitis, and skull base inflammation leading to bilateral lateral rectus palsy. A 56-year-old postmenopausal woman presented with a 10-day history of headache, an 8-day history of right-sided facial pain, and progressive binocular diplopia over 5 days

Controversy about the protective role of volume in the frontal sinus after severe head trauma: larger sinus equates with higher risk of death.

The "crumple zone" hypothesis suggests that the paranasal sinuses protect the brain as a zone to distribute and absorb energy after trauma to the head. We investigated the relation between the size of the frontal sinus and mortality in patients with cranial trauma. All patients with head trauma admitted to the ICU between 1 January 2016 and 20 December 2017 were reviewed retrospectively. They were divided into two groups (according to their outcome) : died and survived. The volumes of the frontal sinuses and other trauma-related variables were assessed on computed tomographs (CT) on admission.

Transglabellar resection of frontal sinus cholesterol granuloma extending cranially through cecum foramina: Technical note.

BACKGROUND: Cholesterol granuloma (CG) commonly occurs in the petrous apex; their occurrence in the anterior cranial fossa CGs is rare. Subfrontal approaches are the conventional surgical approaches for the resection of midline lesions of the anterior cranial fossa and frontal sinuses. In this article, we describe a successful minimally invasive approach for resection of a small midline anterior cranial fossa CG. METHODS: We report a rare case of frontal air sinus CG extending intradurally through the foramen caecum. The surgical management of this patient involved the use of a minimally inva

Head of Zebu cattle (Bos Taurus indicus): sectional anatomy and 3D computed tomography.

The research was designed to use computed tomography (CT) with 3D-CT reconstruction imaging techniques and the various anatomical sections-plana transversalia, frontalis, and dorsalia-to describe the anatomical architecture of the Zebu cattle head. Our study used nine mature heads. The CT bone window created detailed images of cranial bones, mandibles, teeth, and hyoid bones. All of the head cavities were evaluated, including the cranial, orbital, oral, auricular, and nasal cavities with their paranasal and conchal sinuses. The septum nasi, attached to the vomer and maxillary bones, did not re

Antibiotic responsive exophthalmos and lateral rectus paralysis.

Sinusitis is an unusual cause of exophthalmos in adults since the advent of antibiotics. Many periorbital complications of sinusitis have been described; however exophthalmos associated with apparent sixth cranial nerve palsy in the absence of ipsilateral sphenoidal sinusitis is a rare and, to our knowledge, unreported complication of sinusitis. We report such a case that was treated with oral antibiotics. We believe that sinusitis must always be considered in adults as a treatable cause of exophthalmos and various orbital complications from isolated lateral rectus paralysis to acute-onset, ac

Isolated oculomotor nerve palsy secondary to acute sinusitis.

Sinusitis is a common condition, but only very rarely accompanied by isolated cranial nerve palsies. We describe a case of a 64-year-old male with a two-day history of left-sided ptosis associated with one week of nasal congestion and frontal sinus pain. Examination revealed ptosis with left pupil mydriasis. Uncontrasted computed tomography and angiography of the head demonstrated neither intracranial vascular abnormalities nor acute lesions; however, it did show mucosal thickening in the left frontal sinus, ethmoid air cells and left maxillary sinus, indicating potential obstruction of the le

Allergic fungal sinusitis with extensive bone erosion of the clivus presenting with diplopia.

OBJECTIVE: We report a case of allergic fungal sinusitis causing bone erosion and diplopia. CASE REPORT: A 43-year-old man presented with a four-month history of increased nasal congestion and progressive diplopia. Clinical examination revealed bilateral nasal polyposis and a right lateral gaze deficit, consistent with a VIth cranial nerve palsy. Computed tomography of the paranasal sinuses demonstrated a large sellar mass with extensive bony erosion and both supra- and infra-sellar extension. An endoscopic approach to the sphenoid sinus, clivus and posterior cranial fossa with image guidance

Roentgenological investigation of cavernous sinus structure with special reference to paracavernous cranial nerves.

We have evaluated the anatomical structure of cavernous sinuses in parasellar blocks obtained from adult cadavers to gain clinically useful information. Ten microdissectional studies of parasellar blocks (20 sides of cavernous sinus) were carried out with special emphasis on the course of paracavernous cranial nerves (3rd to 6th). These were also examined on plain radiographs. Secondly, CT investigations of the cavernous sinuses (notably as to paracavernous cranial nerves) were undertaken in twenty clinical cases having presumed parasellar lesions and in three parasellar blocks from cadavers.

Low-Dose Dabigatran for Venous Sinus Thromboembolism Associated with Hereditary Dysfibrinogenemia: A Case Report.

Dabigatran, an anticoagulant, may increase the risk of bleeding in patients with dysfibrinogenemia because of coagulation irregularities, especially at high doses. Cranial Magnetic Resonance Imaging (MRI) and Magnetic Resonance Venography (MRV) were used in the diagnosis of venous sinus thromboembolism in a 42-year-old woman with hereditary dysfibrinogenemia, as documented in our case report. Cranial MRI suggested thrombosis in the venous sinuses, which was confirmed by MRV as thromboses in the superior sagittal, straight, left transverse, and sigmoid sinuses. Instead of the usual fixed-dose,

Beyond the sniffer: frontal sinuses in Carnivora.

Paranasal sinuses are some of the most poorly understood features of mammalian cranial anatomy. They are highly variable in presence and form among species, but their function is not well understood. The best-supported explanations for the function of sinuses is that they opportunistically fill mechanically unnecessary space, but that in some cases, sinuses in combination with the configuration of the frontal bone may improve skull performance by increasing skull strength and dissipating stresses more evenly. We used CT technology to investigate patterns in frontal sinus size and shape dispari
