[Disorders of systemic hemodynamics in the acute period of severe craniocerebral trauma].

Disorders of systemic hemodynamics were studied in 68 patients in the acute period of severe cerebrocranial trauma. Three types of systemic hemodynamics were revealed: normodynamic--in corticohemispheric level of brain injury, hypodynamic--in injury to the stem, and hyperdynamic--in hemispheric-subcortical damage. The authors consider the hyperdynamic and hypodynamic types of the responses of systemic hemodynamics as a prognostically unfavourable sign.

[Electrographic reflection of vegetative protective and compensatory reactions in craniocerebral trauma].

The authors substantiate the assumption that the presence of high-amplitude superslow potential fluctuations in registration from the skin surface of the head is determined by destabilization of subcortical intercentral relationships which comprises inhibition of stem and activation of posterohypothalamic structures in craniocerebral trauma. The constant potential reacts only to the sharpest period of a severe craniocerebral trauma and reflects acute inflammatory and vascular phenomena on the cortical level in the first 24 hours; maintenance or appearance of increased positive values of the co

The potential of melatonin in reducing morbidity-mortality after craniocerebral trauma.

Craniocerebral trauma (CCT) is the most frequent cause of morbidity-mortality as a result of an accident. The probable origins and etiologies are multifactorial and include free radical formation and oxidative stress, the suppression of nonspecific resistance, lymphocytopenia (disorder in the adhesion and activation of cells), opportunistic infections, regional macro and microcirculatory alterations, disruptive sleep-wake cycles and toxicity caused by therapeutic agents. These pathogenic factors contribute to the unfavorable development of clinical symptoms as the disease progresses. Melatonin

Electrocardiographic changes in craniocerebral trauma--could they serve as prognostic indicators?

Electrocardiographic (ECG) changes were analysed in one hundred patients of craniocerebral injuries irrespective of age and sex. Males comprised 82% of patients and most vulnerable age group was 15-40 years (66%). Although the overall mortality was 17%, the maximum mortality (44.44%) was observed in the age group of 41-60 years. Computed tomographic scan proved lesions in intracerebral haemorrhage, subdural haematoma, mixed lesions and extradural haematoma were associated with mortality rates in 66.66, 33.33, 20 and 11.11 per cent respectively. ECG changes of some or the other type were presen

[Diagnostic and therapeutic measures in craniocerebral trauma in the intensive care unit].

Diagnostic and therapeutic measures after severe head-injury in the intensive care unit are discussed. The main goal of all efforts consists in minimizing secondary brain damage. Adequate shock therapy in the initial phase proves crucial for the later outcome. Methods and indications of intracranial pressure monitoring are described. Electrophysiological tests, transcranial doppler sonography, EEG and serial CCT controls provide valuable additional information. Frequent clinical assessments however remain the golden standard despite all technical advances. Surgical and conservative modalities

[Transcranial Doppler sonography with evaluation of CO2 reactivity in craniocerebral trauma].

17 patients with severe head injury were examined by transcranial Doppler sonography. Resting values and CO2 reactivity were determined. The resting measurements had no prognostic value. There was no significant side difference in patients with unilateral intracranial lesions. CO2 reactivity was 2.8 +/- 2.3%/mmHg pCO2 in patients with good outcome, whereas in patients with bad outcome CO2 reactivity was 0.6 +/- 5.1%/mmHg pCO2. However, in the acute situation after the trauma there was no difference between the two. In patients with a unilateral lesion we found a significantly lower CO2 reactiv

Results in patients with craniocerebral trauma and an operatively managed acetabular fracture.

Results in 23 patients with significant craniocerebral trauma (Glasgow Coma Scale less than or equal to 10) and displaced acetabular fracture requiring surgery were reviewed after a minimum follow-up of 1 year. Despite a postoperative anatomic reduction in all but one case, clinical outcome for these hips was poor, with an average Harris hip rating of 59. Patients older than 40 years had a significantly poorer outcome than did younger patients (p = 0.004). Postoperative problems occurred in 70% of patients (n = 16); the largest portion of these represented symptomatic heterotopic bone, which o

The aim of the present work was to assess the influences of age, gender, and the severity of brain trauma on recovery of motor function using kinetotherapy. The study included 131 patients (99 men and 32 women) investigated during the acute phase of trauma in the Department of Brain Trauma, Clinical Hospital, Kaunas Medical University. After stabilization, 80 patients were transferred to the neurorehabilitation clinic and 51 were transferred to other rehabilitation centers. Motor function in the patients was assessed using the Clinical Outcomes Variable Scale (COVS) at the beginning and end of

[Characteristics of higher nervous activity in patients after mild craniocerebral trauma].

A three-four year dynamic study of 211 patients (age: 15-60 years) with a history of mild head injury revealed that the younger group of patients (from 20 to 30 years) showed within 3-4 years after the injury a more complete restoration of the higher nervous activity processes than patients of other age groups. In patients suffering of cerebrovascular diseases before mild head injury the dynamics of this restoration depended on the treatment carried out before the head injury.

[Decompressive craniotomy in severe craniocerebral trauma following unsuccessful treatment with barbiturates].

During the period from September 1983 to October 1984 the intracranial pressure of 51 patients who had suffered cranio-cerebral trauma was continuously recorded. In 21 patients (41%) the intracranial pressure remained within normal limits (10-25 mm Hg) and apart from hyperventilation and corticosteroids no other treatment was required. All these patients recovered satisfactorily. In 28 patients the raised intracranial pressure could not be reduced by means of hyperventilation, corticosteroids and osmotherapy so that barbiturates had to be administered. Seven patients did not respond to this ad

[Bilateral hypoglossal nerve paralysis as isolated neurological symptom after craniocerebral trauma].

We report a 72-year-old patient who developed an isolated bilateral hypoglossal nerve paralysis following head trauma with complete recovery after three months. Since the CT scan did not show any fractures of the posterior skull base, we discuss a traction nerve injury as a possible mechanism.

[Computer tomographic measuring procedure in generalized brain edema after craniocerebral trauma].

Computer tomography enables for the first time the direct demonstration of the consequences of a craniocerebral trauma. In the computer tomogram, the cerebral oedema is dominating as an area of reduced density (hypodense region). Three different measuring techniques are compared and their application in 43 craniocerebral traumata discussed. Among the measuring methods employed, the area index shows the greatest diagnostic reliability in the computer-tomographic findings of a posttraumatic cerebral oedema. The partial volume analysis without application of a contrast medium is of little use for

Pneumococcal meningitis in community is frequent after craniocerebral trauma and in alcohol abusers.

Aim of this short communication was to assess risk factors and outcome of community acquired pneumococcal meningitis and compare it to all cases of community acquired meningitis. Univariate analysis was used for comparison of 68 pneumococcal to 201 CBM within a Slovak nationwide database of CBM. Significant risk factors for pneumococcal meningitis were previous craniocerebral trauma within 7 days (39.7% vs. 14.9%, p=0.00002), splenectomy (10.3% vs. 3.5%, p=0.03) and alcohol abuse (36.8% vs. 15.4%, p=0.0001). Concerning outcome, mortality was similar (8,8% and 12,4%, NS), proportion of those wi

[Effect of hyperventilation on the cerebral blood flow and metabolism in patients with craniocerebral trauma].

Cerebral blood flow and metabolism were studied in 44 patients with acute severe craniocerebral injuries (contusions of the brain and removal of intracranial hematomas) with consciousness depression (moderate coma), hospitalized in intensive neurological care wards. Oxygen supply to the brain was repeatedly evaluated (acid-base status of the jugular vein blood, oxygen arterio-venous difference, oxygen extraction coefficient), oxymetry of the brain was repeatedly carried out, and vital functions were monitored over the entire period of disease. Cerebral blood flow was monitored by rheoencephalo
