Crush syndrome: a review for prehospital providers and emergency clinicians.

INTRODUCTION: Disasters and accidents have occurred with increasing frequency in recent years. Primary disasters have the potential to result in mass casualty events involving crush syndrome (CS) and other serious injuries. Prehospital providers and emergency clinicians stand on the front lines of these patients' evaluation and treatment. However, the bulk of our current knowledge, derived from historical data, has remained unchanged for over ten years. In addition, no evidence-based treatment has been established to date. OBJECTIVE: This narrative review aims to provide a focused overview of

A case of crush syndrome induced by the kneeling seiza position.

Crush syndrome results in a characteristic syndrome of rhabdomyolysis with myoglobinuric acute renal failure. The most commonly described crush injury is that which affects victims of natural disasters such as earthquakes. Here, we report a rare case of crush syndrome that was induced by the kneeling seiza position.

Crush syndrome and acute kidney injury in the Wenchuan Earthquake.

BACKGROUND: The Wenchuan Earthquake resulted in calamitous destruction and massive death. We report the characteristics of crush syndrome (CS) and acute kidney injury (AKI) brought by the earthquake, which took place in a mountainous area. METHODS: We conducted a cross-section survey of total 2,316 consecutive admissions because of seismic trauma, of which 1,827 had complete data available after we excluded those victims with mild injuries. The characteristics of CS and AKI in the mountainous earthquake were analyzed. RESULTS: A total of 149 patients (8.2%) were diagnosed with CS. They had v

Crush syndrome and acute renal failure in the Marmara earthquake.

In this study, patients with acute renal failure as a result of crush syndrome after the Marmara earthquake were evaluated retrospectively. Six hundred thirty injured patients were evaluated after the Marmara earthquake at Gülhane Military Medical Academy. Acute renal failure in association with crush syndrome developed in 31 patients. Twenty-nine of them required hemodialysis. Duration of stay under the ruins was 6 to 135 hours. Seventy-seven patients underwent fasciotomy, and 6 underwent extremity amputation. The total number of hemodialysis sessions was 173. Six patients died as a result of

Temporary hypothyroidism in severe crush syndrome: a potential novel entity.

BACKGROUND: Thyroid hormone synthesis is a complex process in the human body. Although the thyroid gland is essential for thyroid hormone synthesis, skeletal muscles also have crucial roles in thyroid hormone metabolism due to the deiodinase activities of the muscle cells. Hypothyroidism-related myopathy is a well-known entity. However, systemic effects of acute myopathies, such as rhabdomyolysis, on thyroid hormone metabolism have not to date been fully clarified. METHODS: Fifty-three earthquake victims were evaluated retrospectively. We investigated the thyroid function tests (TFTs) among p

Double crush syndrome due to plating of humeral shaft fracture.

Median nerve injury is rarely associated with the humeral shaft fracture. A Sixty two year old woman with a displaced humeral shaft fracture, developed a symptomatic carpal tunnel syndrome after plating with a screw protruding medially. 16 months later, the implants were removed and the symptoms gradually improved without carpal tunnel release surgery. A double crush syndrome resulted due to the proximal compression by the medially protruding screw and the distal compression by carpal tunnel. The proximal decompression produced by removal of the screw led to relief of the symptoms.

[Bilateral tibial compartment syndrome with crush symptoms after cannabis abuse].

Acute compartmental syndrome (CS) is a surgical emergency. Different conditions in which high non-physiological pressure appears within a closed fascial space reduce the necessary blood perfusion. CS is caused by trauma, burns, bleeding in patients with coagulopathies arterial injuries, nephrotic syndrome or unusual physical exercise with secondary compartmental swelling. When decompression occurs too late, permanent loss of function and limb contracture may result. In the following paper we report on a case of four-compartmental syndrome in the lower legs of a patient with drug intoxication.

The Incidence of Double Crush Syndrome in Surgically Treated Patients.

STUDY DESIGN: Retrospective Cohort Study. OBJECTIVES: Cervical radiculopathy and peripheral entrapment neuropathies often have overlapping symptoms that are difficult to distinguish on physical examination. Small-scale studies have attempted to report the incidence of this phenomenon, often called double crush syndrome (DCS), with varying results. The present study aims to determine the incidence of concomitant cervical radiculopathy and peripheral nerve compression and to determine if the DCS hypothesis, which states that compression of a nerve at one site leaves it more susceptible to compr

Double crush syndrome in the lower extremity: a case report.

Upton and McComas first described double crush syndrome in 1973. The theory behind double crush syndrome postulated that a proximal lesion in a nerve would make that same nerve more vulnerable to additional distal lesions. Many of the studies investigating the possibility of the double crush syndrome involve lesions in the upper extremity with very few articles written specifically about double crush syndrome in the lower extremity. We present the case of a 33-year-old massage therapist who uses her feet to provide therapy to clients who presented to our clinic with symptoms consistent with ta

Common complication of crush injury, but a rare compartment syndrome.

Compartment syndrome (CS) is a common complication of crush injury but it is rare to find bilateral gluteal compartment syndrome (BGCS). Only six cases of BGCS have been reported in the literature. This syndrome has been reported after crush injury, drug overdose, surgical positioning, and vascular surgery. Apart from CS, crush injury is associated with multi-system adverse effects and these patients are at high risk for renal failure and sepsis. CS patients may present with dehydration; coagulation disorders; elevated creatine phosphokinase and myoglobin levels; hyperkalemia and hypocalcaemia

Case report: Myofascial pain syndrome: a double crush-like appearance.

OBJECTIVE: To discuss the case of a patient with myofascial pain syndrome that appeared similar to double crush syndrome. CLINICAL FEATURES: This is a case report of a 45-year old female who presents to the private practice clinic with the same signs and symptoms of carpal tunnel syndrome, 1 year post-carpaltunnel surgery. The history is consistent with injuries that result in double crush syndrome and thoracic outlet syndrome. The patient history includes a motor vehicle accident (MVA) in 1963 that resulted in a prolonged hospital stay and fracture of a cervical vertebral (C4) body. She frac

An isolated double-crush-syndrome in posttraumatic thoracic outlet syndrome - A case report.

Thoracic outlet Syndrome (TOS) includes all disorders caused by compression of all neurovascular Structures in the upper thoracic outlet (Ross and Owners, 1966, Bürger and Arterien, 2014, Sanders and Annest, 2017). The Double-Crush-Syndrome (DBS) defines multilevel lesions along a neurovascular trunk caused by mechanical compression in different areas. Pectoralis-minor-syndrome (PMS) is also classified in the disorders of he upper thoracic outlet and was also known as hyperabductionsyndrome or subcoracoidal-syndrome. Between 2015-2019 our department treatet 488 patients suffering from neurolog

Cubital tunnel surgery in patients with cervical radiculopathy: double crush syndrome?

To determine differences in clinical outcomes in patients harboring both cubital tunnel syndrome (CuTS) and cervical radiculopathy and the influence of the so-called double crush syndrome. Both procedures were performed in 24 patients, mean age 55 years; first group of 14 patients underwent CuTS surgery as a first procedure. Second group of 10 patients underwent anterior cervical discectomy and fusion (ACDF) then ulnar nerve release (UNR). Two patients underwent bilateral nerve surgery and six multiple cervical discectomies. Surgeries consisted in 26 nerve releases with associated external neu

[Crush Syndrome - What Should We Know in the Pre-hospital Setting?].

A crush injury (CI) is damage to bone, muscle, subcutaneous tissue, and skin caused by prolonged external pressure applied to soft tissues. In contrast, a crush syndrome (CS) is a multisystem syndrome caused by the release of toxins from injured muscle. The syndrome is also sometimes referred to as traumatic rhabdomyolysis. The pathophysiology of the syndrome involves prolonged ischemia and reperfusion injury. These processes lead to the release of toxic metabolites from the damaged muscle, resulting in hypovolemic shock, acidosis, electrolyte disturbances, acute kidney injury, and even multip
