An Unusual and Late Presentation of 46, XY Ovotesticular Difference of Sex Development.

Ovotesticular difference of sex development (OT-DSD) is a rare condition encompassing a broad spectrum of phenotypic variations. We describe a 67-year-old man who presented with complaints of poor urinary stream and hesitancy for the past 6 months. He had normal pubertal development, libido, and erection, but has been experiencing sexual dysfunction and reduced energy over the last 2 years. Physical examination revealed bilateral cryptorchidism with no other genital ambiguities. Laboratory evaluation showed a 46, XY karyotype with hypergonadotropic hypogonadism, and imaging revealed müllerian

Innovative laparoscopic technique for the resection of a giant abdominal cyst in a young female: A case report.

INTRODUCTION: Abdominal cystic lesions are a diverse spectrum of pathologies, predominantly ovarian in origin, including benign functional cysts to malignant neoplasms. This report presents a novel laparoscopic technique for resection of large abdominal cysts with oncological safety. CASE PRESENTATION: A 23-year-old female had a 2-year history of progressive abdominal distension. A 30 × 25.6 × 16.8 cm midline intraperitoneal cystic lesion was seen on imaging. A laparoscopic approach was adopted with protected cyst aspiration, exploration, and cystectomy of a right ovarian cyst, preserving the

Ovarian benign clear cell adenofibroma in a patient with premature ovarian failure after 8-year hormone replacement therapy.

Ovary benign clear cell adenofibroma (CCAF) is extremely rare. Here, we reported a 38-year-old woman with CCAF who was given sequential cyclic hormone replacement therapy for eight years due to premature ovarian failure (POF). At operation, we found adenofibromas in both ovaries and well encapsulated. The cut surface was cystic and solid in appearance, and multilocular cysts embedded in hard stroma. Determination of sex hormone levels in the fluid of cysts of the ovary showed normal level. Clear cell cystadenofibroma proliferation was found in ovarian stroma while atrophy in ovarian cortex. Hi

Sonographic features of parovarian cysts and their complications.

Parovarian cysts have received virtually no attention in the sonography literature, despite a common occurrence, constituting 10%-20% of adnexal tumorlike conditions in pathologic series. The sonographic features of 11 parovarian cysts, including a number with complications, are reported. Six uncomplicated lesions had the appearance of simple cysts found in other organs. One cyst had ruptured, and one lesion was associated with ipsilateral tubal torsion. Two cysts had echogenic areas representing blood clot. One cyst had a triangular papillation representing the development of a serous cystade

Unusual case of anti-N-methyl-D-aspartic acid-receptor (NMDA-R) encephalitis and autoimmune polyglandular syndrome (APS).

Anti-N-methyl-D-aspartic acid-receptor (NMDA-R) encephalitis is a novel disease discovered within the past 10 years. It is an autoimmune disease (AD) that has been associated with other ADs, such as Graves' disease. However, association with autoimmune polyglandular syndromes (APS) has not been previously described. A 58-year-old woman presented with altered mental status and an 8-month history of weight loss, apathy and somnolence. Laboratory evaluation confirmed Graves' disease with thyrotoxicosis and type 1 diabetes mellitus. Despite treatment, she continued to have a fluctuating mental sta

Benign Lesions Scored as O-RADS US v2022 4 and 5 Categories: Prevalence and Morphologic Analysis.

Background Lesions scored as Ovarian-Adnexal Reporting and Data System (O-RADS) US 4 have an expected risk of malignancy of 10% to less than 50%, and those scored as O-RADS US 5 have an expected risk of malignancy of 50% or greater. However, studies of unselected populations demonstrated lower risk-of-malignancy ranges due to benign lesions being assigned these scores. Purpose To determine the frequency of benign lesions scored as O-RADS US 4 and 5 and to assess their pathologic and lexicon imaging features. Materials and Methods Consecutive patients who underwent pelvic US between January 201

Rare and uncommon tumours of the female pelvis: what the radiologist should know.

Gynaecological tumours present a broad spectrum of histological subtypes due to the diverse anatomical and tissue origin of the reproductive organs. Rare tumours affect less than 6 per 100 000 individuals annually, posing significant challenges in diagnosis and management due to limited clinical awareness. Indeed, treatment protocols rely on options developed for more common histotypes, which may have limited efficacy on these rare tumours. In recent years, collaborative international efforts have started to address these gaps, improving standards of care. A comprehensive understanding of rare

Atypical papillary proliferation in gynecologic patients: a study of 32 pelvic washes.

Papillary clusters in gynecologic pelvic washes frequently cause diagnostic challenges because they can be associated with borderline or malignant ovarian tumors, as well as benign pelvic diseases. The objective of our study was to review all pelvic washes with atypical papillary proliferation (APP) and investigate whether cytomorphology and/or immunohistochemistry on cell block could determine their origin. Thirty-two pelvic washes from 31 patients containing APP were reviewed and correlated with their corresponding gynecologic or pelvic disease. Previously obtained cell blocks with immunohis

Serous adenocarcinoma of the sigmoid mesentery arising in cystic endosalpingiosis.

This case report describes a Mullerian serous adenocarcinoma arising within a multoloculated cyst lined by ciliated serous-type epithelium located in the sigmoid mesentery. Twenty years previously the patient underwent a hysterectomy, bilateral salpingo-oophorectomy, and omentectomy. The ovaries contained bilateral serous cystadenofibromas, and multiple cysts lined by ciliated serous-type epithelium were present in the omentum. The resection specimen 20 years later contained a 14 cm multiloculated cyst located in the sigmoid mesentery. This was lined largely by benign ciliated serous-type epit

TFF3 Expression as Stratification Marker in Borderline Epithelial Tumors of the Ovary.

Borderline tumors (BOT) of the ovary account for 10% to 20% of ovarian neoplasms. Like ovarian cancer, BOT encompass several different histological subtypes (serous, mucinous, endometrioid, clear cell, transitional cell and mixed) with serous (SBOT) and mucinous (MBOT) the most common. Current hypotheses suggest low-grade serous carcinoma may develop in a stepwise fashion from SBOT whereas the majority of high grade serous carcinomas develop rapidly presumably from inclusion cysts or ovarian surface epithelium. The pathogenesis of mucinous ovarian tumors is still puzzling. Molecular markers co

[Serous ovarian tumors].

Because of different patterns of molecular changes, a dualistic model of serous tumors is now assumed with serous borderline tumors (SBT) and low-grade serous carcinomas (LGSC) on one side and high-grade serous carcinomas (HGSC) on the other. The clinical course and the type of treatment of SBT and LGSC depend crucially on whether they are associated with extraovarian manifestations. So-called invasive implants of SBT correspond morphologically to LGSC. The MD Anderson grading system has become established for the distinction between LGSC and HGSC, HGSC shows a wide range of growth patterns, i

Accuracy of serum human epididymis protein 4 in ovarian cancer diagnosis: a systematic review and meta-analysis.

OBJECTIVE: A systematic review was performed to estimate the accuracy of human epididymis protein 4 (HE4) assay in the diagnosis of ovarian tumors. METHODS: A comprehensive search of the MEDLINE (PubMed), EMBASE, Cochrane Central Register of Controlled Trials, IBECS, BIOSIS, Web of Science, SCOPUS, congress abstracts, and Grey literature (Google scholar; British Library) from January 1990 to April 2013 was conducted. Studies that evaluated HE4 levels for the diagnosis of ovarian tumors and compared them with paraffin-embedded sections as the diagnostic standard were included. RESULTS: Forty-

Atypical Proliferative (Borderline) Serous Tumor in the Brain: A Case Report.

A 59-year-old woman with a remote history of invasive ductal carcinoma of the breast was found on a follow-up computed tomography scan of her brain to have a 1-cm lesion in the right frontal lobe in 2008. In the ensuing years, before her current admission, multiple imaging studies of the brain revealed that the lesion was stable and it was, therefore, interpreted as a small area of encephalomalacia related to a thrombosed cortical vein, a cavernoma, or treated metastatic breast cancer. In 2013, the patient underwent a bilateral salpingo-oophorectomy for ovarian tumors that were diagnosed as bi

Brenner tumors.

Brenner tumors are rare ovarian neoplasms composed of ovarian transition cells surrounded by dense fibrous tissue. Most of them are small tumors (<2 cm), detected incidentally in asymptomatic women. Its predominantly fibrous content results in relatively low signal on T2 weighted images, establishing differential diagnosis with ovarian fibroma and thecoma. Their imaging features are very similar, the differentiation is based on secondary characteristics, such as signs or symptoms of estrogen excess and the presence of a second ovarian neoplasm, which has been reported in up to 30% of patients

[Ovarian cancer after in vitro fertilization].

There is serious concern about cancer risk in women undergoing ovarian stimulation treatment for infertility and longterm safety of these procedures. Association between fertility drugs and ovarian cancer is still controversial. A 30-year-old woman was referred to our institution with the initial diagnosis of an adnexal tumor after in vitro fertilization. Her history revealed adnexectomy for mucinous cystadenofibroma of the left ovary eight years ago, and cystectomy due to cystadenoma of the right ovary three years ago. At admission, the most remarkable findings were high temperature and eleva

[Surgical laparoscopy in tumors of the adnexa].

OBJECTIVES: The laparoscopic management of adnexal masses is still controversial. The main problems are preoperative diagnosis, operative technique and the possibility of compromising the prognosis by spillage of cancer cells when operating on a hitherto unrecognized early stage ovarian cancer. We report on our experience with minimal invasive surgery of adnexal masses. METHOD: From June 1989 to July 1992, 375 patients underwent surgery for adnexal tumors at the University Hospital, Zurich. In 112 cases a laparoscopic operation was performed. The indications were unilateral or bilateral masse

[Mucinous ovarian neoplasms. Prognostically mostly excellent, infrequently a wolf in sheep's clothing].

Mucinous ovarian neoplasms represent the second largest group of epithelial ovarian tumors after serous neoplasms, of which benign cystadenomas constitute more than 80 %. Mucinous cystadenomas and carcinomas cannot be distinguished by the clinical features or the mean age of onset of the disease. They typically occur unilaterally, are confined to the adnexae (FIGO stage I) and clinically present with non-specific abdominal symptoms or are diagnosed by chance. The mean age of disease onset is around 50 years old. The prognosis is excellent. Implants, peritoneal metastases and bilateral occurren

Endocrine cell micronests in an ovarian mucinous borderline tumor: a potential diagnostic pitfall for microinvasion.

The occurrence of endocrine cell micronests in ovarian tumors is rarely reported. To our knowledge, there are only three prior cases reported to date: one occurring in an ovarian mucinous cystadenoma, one in an ovarian mucinous cystadenofibroma, and another in an ovarian mucinous carcinoma with a predominant borderline component. This is a 27-year-old woman that presented with a one-month history of abdominal pain and fullness. Imaging studies revealed a large multiloculated cystic and solid mass measuring 23 cm occupying the majority of the pelvis and abdomen concerning for a primary ovarian
