Advances in the treatment of interstitial cystitis.

Recent years have brought dramatic advances in the clinician's ability to offer effective pharmacotherapy to patients who have interstitial cystitis. Medical treatments have been developed and applied to reduce the interstitial cystitis symptoms of pelvic pain and urinary urgency/frequency, and to address underlying causes of the disorder. In addition, advances in the understanding of the natural history of interstitial cystitis have revealed that it is insidiously progressive and the classical definition--rare, severe and difficult to treat--is in fact the relatively uncommon, advanced stage

The management of interstitial cystitis: an update.

Treating interstitial cystitis (IC) is one of the greatest challenges facing physicians and other health care providers who manage patients with this condition. The symptoms of urinary frequency and urgency, dysuria, and chronic pelvic pain characterize IC, but it is the debilitating pelvic pain associated with IC that is most difficult to control. The pathophysiology of IC pain is poorly understood, but is thought to be a complex entity including nociceptive, visceral, and neuropathic components. There are currently no universally effective therapies available. Oral treatments, however, are f

The investigation and management of interstitial cystitis.

Interstitial cystitis (IC) is a chronic inflammatory disorder of the bladder that is notoriously difficult to manage and can result in considerable morbidity. It very likely overlaps with painful bladder syndrome, but they are different conditions. The aetiology remains obscure, and the definition and diagnostic criteria are debated. The diagnosis of IC is one of exclusion, frequently based on symptoms and cystoscopy findings. Typical symptoms include frequency, urgency, dysuria and lower abdominal, bladder, vaginal, urethral or perineal pain, in the absence of bacterial cystitis. Voiding ofte

The historical origins of interstitial cystitis.

PURPOSE: We identify early descriptions of interstitial cystitis and trace its evolution as a clinical entity during the 19th century. MATERIALS AND METHODS: Primary and secondary source documents relating to interstitial cystitis, bladder inflammation and bladder stones were reviewed. RESULTS: What is believed to be the earliest published record of interstitial cystitis appeared in an 1836 textbook by the Philadelphia surgeon Joseph Parrish, who documented a syndrome of chronic frequency, urgency, dysuria and pelvic pain he called "tic doloureux of the bladder." Tic doloureux was a contempo

Interstitial cystitis: review of the literature.

Interstitial cystitis is a chronic bladder disease of unknown frequency and unclarified etiology. The condition is often missed and the patients incorrectly treated. The problematic patient is often a woman with a long-lasting urological history, sterile urine, urinary incontinence, dyspareunia or chronic pelvic pain. Here characteristics of 5 patients are described and the literature is reviewed to draw attention to this condition.

Development of the Interstitial Cystitis Self-Help and Medical Resources Scale (ICSR) for Women with Interstitial Cystitis.

Background and Objectives: Women with interstitial cystitis (IC) suffer from spontaneous serious bladder pain symptoms without immediate resolution. Women with IC may lack knowledge of how to help themselves. Therefore, a measurement of IC self-help and medical-resource-seeking for women with IC is needed. Materials and Methods: This study recruited 100 women with IC from a teaching hospital in Northern Taiwan. The reliability and validity of the Interstitial Cystitis Self-Help and Medical Resources Scale (ICSR) were assessed using expert validity, confirmatory factor analysis (CFA) to test th

Reliability and validity of Turkish versions of the interstitial cystitis symptom index and interstitial cystitis problem index.

AIM: To validate the Turkish versions of the interstitial cystitis symptom index (ICSI) and interstitial cystitis problem index (ICPI) for use in Turkish speaking patients with bladder pain syndrome/interstitial cystitis (BPS/IC). METHODS: After translation of the original ICSI and ICPI into the Turkish language, Turkish versions of ICSI and ICPI were self-administered to all participants. Test-retest reliability (intraclass correlation coefficient) was evaluated at 2 weeks intervals in the BPS/IC group. Internal consistency was evaluated using Cronbach's alpha. Scores of ICSI and ICPI was co

Intravesical liposome therapy for interstitial cystitis.

Over the past two decades, there has been lot of interest in the use of liposomes as lipid-based biocompatible carriers for drugs administered by the intravesical route. The lipidic bilayer structure of liposomes facilitates their adherence to the apical membrane surface of luminal cells in the bladder, and their vesicular shape allows them to co-opt the endocytosis machinery for bladder uptake after instillation. Liposomes have been shown to enhance the penetration of both water-soluble and insoluble drugs, toxins, and oligonucleotides across the bladder epithelium. Empty liposomes composed e

Contemporary clinical presentation of interstitial cystitis.

For the purpose of presenting a comprehensive review of current information, the medical literature was reviewed to extract data related to the clinical presentation of interstitial cystitis (IC). The most common symptoms at diagnosis are urinary urgency, daytime frequency, dysuria, pain, and nocturia. The most common sites where pain is localized are the suprapubic, pubic, vaginal, and genital areas. Pain is commonly triggered or exacerbated by vaginal intercourse in women or ejaculation in men. The most common findings on physical examination are dysphoric mood, suprapubic tenderness, and an

Interstitial cystitis and systemic autoimmune diseases.

The cause of interstitial cystitis, a chronic disease that affects the bladder, is unknown. Autoantibodies, such as those against nuclear and bladder epithelium antigens, have been found in patients with interstitial cystitis, but these are likely to be secondary to the disease. No data support a direct causal role of autoimmune reactivity in the pathogenesis of interstitial cystitis. Indirect evidence, however, does support a possible autoimmune nature of interstitial cystitis, such as the strong female preponderance and the clinical association between interstitial cystitis and other known a

[Interstitial cystitis--its diagnostics and treatment].

Interstitial cystitis is a chronic, non-bacterial inflammatory disorder of the wall of the urinary bladder and it manifests with very painful symptoms. Initially it can resemble a bacterial inflammation of the urinary bladder. This syndrome is characterised with motor and sensory dysfunction of the urinary bladder. The difficulties can be classified into so-called syndrome of the chronic pelvic pain. Increasing incidence, chronicity of this disorder and specific treatment leads to the reinterpretation of the chronic inflammation of the urinary tract diagnosed in the past and to the modificatio

Interstitial cystitis: urgency and frequency syndrome.

Interstitial cystitis is a chronic, severely debilitating disease of the urinary bladder. Excessive urgency and frequency of urination, suprapubic pain, dyspareunia, chronic pelvic pain and negative urine cultures are characteristic of interstitial cystitis. The course of the disease is usually marked by flare-ups and remissions. Other conditions that should be ruled out include bacterial cystitis, urethritis, neoplasia, vaginitis and vulvar vestibulitis. Interstitial cystitis is diagnosed by cystoscopy and hydrodistention of the bladder. Glomerulations or Hunner's ulcers found at cystoscopy a

[Open surgical therapy of interstitial cystitis].

Treatment of IC is one of the most difficult therapeutic challenges in urology, frequently resulting in frustration for both patient and therapist. Many urologists believe that conservative treatment should not be unnecessarily prolonged in severe cases with low bladder capacity, since cystectomy may provide immediate and permanent relief for the suffering patient. However, it remains unclear which surgical approach and technique is the most suitable. Generally three different techniques are performed: supratrigonal cystectomy; radical cystectomy, saving only the bladder neck; and, finally, ra

Interstitial cystitis. Etiology, diagnosis, and treatment.

OBJECTIVE: To review current knowledge about the epidemiology, etiology, diagnosis, and treatment of interstitial cystitis, with special emphasis on management of this condition by family physicians. QUALITY OF EVIDENCE: Articles were identified through MEDLINE and review of abstracts presented at Urology and Interstitial Cystitis meetings during the last decade. Recent reviews were further searched for additional studies and trials. Data were summarized from large epidemiologic studies. Etiologic theories were extracted from current concepts and reviews of scientific studies. Diagnostic crit

Nephrogenic adenoma associated with interstitial cystitis.

A nephrogenic adenoma occurring in a forty-five-year-old male with a past history of urinary tract infection and interstitial cystitis is described. Four years after the diagnosis of interstitial cystitis, nephrogenic adenoma developed. The patient underwent a total cystoprostatectomy and urinary diversion for bilateral vesicoureteral reflux and progressive and incapacitating symptoms that persisted in spite of accepted standards of management for this disease. This case further supports the role of chronic inflammatory processes of the bladder in the subsequent development of nephrogenic aden
