Establishment of the novel cystoscopic classification for bladder trabeculation of neurogenic bladder.

OBJECTIVE: To establish a cystoscopic grading system for trabeculation of neurogenic bladders. MATERIALS AND METHODS: A total of 140 neurogenic bladder patients who had undergone both fluoroscopic urodynamic study and cystoscopic examination were retrospectively reviewed. Cystoscopic images were categorized into 4 grades according to the formation of the muscle bundle layer and height-to-width ratio of the muscle bundle: 0 (none), 1 (mild), 2 (moderate), and 3 (severe). Test-retest reliability and interobserver reliability were assessed. Cystoscopic grade of the trabeculated bladder was corre

Laparoscopic repair of inguinal cystocele using cystoscopic light test.

OBJECTIVE: To describe how the cystoscopic light test aids in the repair of inguinal bladder herniation. METHODS: After the diagnosis of a bladder herniation into the right inguinal canal using intravenous pyelography, laparoscopic repair was planned. A cystoscope was manipulated into the defect while turning off the laparoscopic light intra-abdominally. The light of the cystoscope could be identified through the cystocele, and the neck of the hernia was easily recognized. Once the bladder was reduced, the cystoscope was replaced, and reduction of the herniation was confirmed. RESULTS: We we

Inability to pass a urethral catheter: the bedside role of the flexible cystoscope.

An all too common cause of urologic consultation is the inability to place a urethral catheter. Often other health care providers have unsuccessfully attempted catheter placement. Urethral false passages, perforations, and edema are common sequelae. Diseases such as urethral strictures, bladder neck contractures, and prostate cancer are often the underlying etiologies for failed catheterization. Traditionally, the use of filiforms and followers or the placement of a suprapubic tube is required to drain the lower urinary tract. Bedside flexible endoscopy was performed in this series not only to

Retrograde catheterization of the ureter without cystoscopic assistance: preliminary experience.

Standard angiographic techniques were applied to fluoroscopically directed retrograde guide wire or catheter placement into the distal ureter without cystoscopic assistance. The procedure was successful in five of the seven initial attempts, with no complications. Retrograde pyelography and stent placement were accomplished for a benign posttraumatic ureteral stricture, a proximal ureteral calculus, and three malignant ureteral strictures. Percutaneous nephrostomy was obviated in all successful cases. Fluoroscopy time averaged less than 3 minutes. Mild intravenous sedation was used, and the pr

A cystoscopically directed biopsy technique developed in canine pancreaticoduodenal transplantation.

Present methods of monitoring for allograft dysfunction in pancreaticoduodenal transplantation (urinary amylase, scintigraphy) give indirect evidence and do not consistently allow recognition of early, potentially reversible rejection from other causes of allograft dysfunction. For better diagnosis of allograft dysfunction, adequate tissue specimens must be obtained without trauma to the allograft. We devised a cystoscopically directed needle biopsy technique to obtain tissue from the canine pancreaticoduodenal allograft. This technique is made feasible by a duodenocystostomy similar to that i

Cystoscopic fluorescence detector for photodetection of bladder carcinoma with hematoporphyrin derivative.

Hematoporphyrin derivative, a fluorogenic substance, tends to accumulate at higher levels in most tumor tissues after intravenous injection. These properties provide a potential application for the detection of malignant tumors. We report the development and in vitro evaluation of an instrument designed to excite and detect low levels of hematoporphyrin derivative fluorescence emitted by tumor cells in the bladder after hematoporphyrin derivative administration. The light source of the instrument for specific hematoporphyrin derivative excitation is at a wavelength of 405 nanometers. Optical f

Percutaneous cystoscopically and fluoroscopically guided sclerotherapy for treatment of renal hematuria in a cat.

An 11-year-old male neutered domestic shorthair cat was evaluated for severe hematuria causing urethral obstruction and marked anemia. Percutaneous cystoscopy identified hemorrhagic jets of urine issuing from the right ureterovesical junction (UVJ), raising concern for renal hematuria. Focal bleeding was observed from the bladder wall and suspected to be iatrogenic. Under cystoscopic and fluoroscopic guidance, a 2.5 Fr nephrostomy tube was advanced retrograde over a guidewire to mid-ureter and used to infuse silver nitrate and povidone iodine into the renal pelvis. A urethral catheter was used

Artificial Intelligence-Based Classification and Segmentation of Bladder Cancer in Cystoscope Images.

BACKGROUND/OBJECTIVES: Cystoscopy is necessary for diagnosing bladder cancer, but it has limitations in identifying ambiguous lesions, such as carcinoma in situ (CIS), which leads to a high recurrence rate of bladder cancer. With the significant advancements in deep learning in the medical field, several studies have explored its application in cystoscopy. This study aimed to utilize the VGG19 and Deeplab v3+ deep learning models to classify and segment cystoscope images, respectively. METHODS: We classified cystoscope images obtained from 772 patients based on morphology (normal, papillary,

Bladder image stitching algorithm for navigation and referencing using a standard cystoscope.

To aid in the diagnosis, monitoring, and surveillance of bladder carcinoma, this study aimed to develop and test an algorithm that creates a referenceable bladder map rendered from standard cystoscopy videos without the need for specialized equipment. A vision-based algorithm was developed to generate 2D bladder maps from individual video frames, by sequentially stitching image frames based on matching surface features, and subsequently localize and track frames during reevaluation. The algorithm was developed and calibrated in a 2D model and 3D anthropomorphic bladder phantom. The performance

Excretory urographic, ultrasonographic, and cystoscopic characteristics of urinary bladder abscesses in a dog.

A 7-year-old male neutered, 6.5 kg, Maltese breed dog was referred with a history of dysuria after undergoing two cystotomies to remove calcium oxalate uroliths. Survey radiographs were inconclusive. Excretory urography allowed visualization of two filling defects in the cranioventral urinary bladder region. Ultrasonography showed urinary bladder sediments, a diffusely thickened wall, and two circumscribed intramural/intraluminal structures with an anechoic interior and a smooth and regular surface. The cystoscopic diagnosis was urinary bladder abscesses and urine cultures were positive for Es

Urethral metastasis from esophageal cancer: symptoms of dysuria and cystoscopic diagnosis.

Urethral malignant tumors are rare and can lead to stenosis, causing dysuria. We report a case of urethral metastasis secondary to esophageal cancer. At the time of diagnosis, a patient with esophageal squamous cell carcinoma presented with voiding difficulties, feeble stream, terminal dribbling and incomplete voiding. The urethral tumor was diagnosed using cystoscopy, and biopsy was thereafter performed. Histopathology of the urethral tumor microscopically resembled to that of esophageal cancer. On immunohistochemistry, the urothelium markers uroplakin 2 and GATA3 were negative in the carcino

Cystoscopic findings in women with minimal and maximal lower urinary tract symptoms.

BACKGROUND: Glomerulations are not specific for interstitial cystitis/bladder pain syndrome (IC/BPS). Controversy exists about whether cystoscopic findings differ between patients with and without lower urinary tract symptoms. We sought to compare the prevalence of cystoscopic findings in women with "no or minimal" urinary symptoms to those with a "high" symptom burden. METHODS: This is a secondary analysis of a prospective cohort study performed at a University Educational Facility. Participants in this study were part of a larger prospective study, in which female patients scheduled to unde

A healthy 37 - year - old woman referred to our clinic with one - year history of recurrent urinary tract infection, dysuria and frequency. Her past medical history informed us that an IUD (Copper TCu380A) had been inserted 11 years ago. Eleven months after the IUD insertion she had become pregnant, unexpectedly. At that time, she had undergone gynecological examination and abdominal ultrasound study. However, the IUD had not been found, and the gynecologist had made the diagnosis of spontaneous fall out of the IUD. She had experienced normal pregnancy and caesarian section with no complicatio

Suprapubic-assisted cystoscopic excision of intravesical tension-free vaginal tape.

We report on a 56-year-old woman with irritative voiding symptoms, which started 4 months after undergoing a TVT procedure for stress incontinence (SI). Evidence of the mesh coming through the left bladder wall did not emerge until cysto-urethroscopic examination 2 years after surgery. Excision was done via transurethral cystoscopy with the aid of endoscopic scissors placed through a 5-mm suprapubic port inserted into the dome of the bladder, thus avoiding laparotomy. A Foley catheter was left in for 3 days, and after removal, the patient's symptoms disappeared almost completely.
