De Quervain's disease: surgical or nonsurgical treatment.

Ninety-one wrists in 82 consecutive patients with Quervain's disease were treated by the one surgeon between 1978 and 1987. The treatment of 79 wrists in 71 patients who had received their entire treatment from this surgeon is analyzed. Uniformity of injection technique is thus ensured. Initial treatment in 63 wrists was an injection of steroids and local anesthetic into the tendon sheath, which gave complete relief in 45 cases. Seven wrists received two injections before the pain abated. Only 11 of the 63 injected wrists had an operation. In 10 of these the extensor pollicis brevis tendon was

Impact of Septated First Dorsal Compartments on Symptomatic de Quervain Disease.

BACKGROUND: The authors conducted this study to determine whether septation of the first dorsal compartment is more prevalent in de Quervain … ion, 48 matched cadaver upper extremities (96 wrists) that had not previously undergone surgery for de Quervain disease were evaluated for the presence of first dorsal compartment septation. The prevalence of septation was compared between matched wrists and against the surgically treated clinical cohort. RESULTS: In the clinical cohort, 61.2 percent of wrists conta …

De quervain disease: Ibri technique to avoid superficial radial nerve injury.

Tenosynovitis of the first dorsal compartment of the wrist, commonly known as de Quervain disease, is most of the time amenable to conservative treatment in form of splinting and injection of steroids into the compartment. Resistant cases need surgical release of the compartment but with high incidence of incomplete release owing to tendon anomalies and damage to the superficial branch of the radial nerve (SBRN). Many techniques evolved for a period of years, techniques using longitudinal, transverse, and oblique incisions, but damage to SBRN still remains unsolved. The authors describe a tech…

Sonographic Identification of the intracompartmental septum in de Quervain's disease.

BACKGROUND: The intracompartmental septum in the first extensor compartment in patients with de Quervain's disease has been associated with disease development and prognosis. However, with the exception of surgical exploration, there is no way of detecting the septum. QUESTIONS/PURPOSES: We evaluated the accuracy of sonography for identifying the intracompartmental septum in the first extensor compartment in patients with de Quervain's disease using surgical findings as the reference standard. PATIENTS AND METHODS: We performed surgical release of the first extensor compartment in 43 wrists

Histological appearances in post-partum de Quervain's disease.

Six patients who developed de Quervain's disease during pregnancy or within 12 months of childbirth were treated surgically. Histopathological examination of the tendon sheaths using alcian blue stain showed that myxoid degeneration was responsible for the thickening observed in the sheath. Characteristic intramural deposits of mucopolysaccharides were present, predominantly in the subsynovial region. Acute or chronic inflammatory changes were not seen. These findings were the same as those described in patients with de Quervain's disease unrelated to pregnancy or childbirth.

The anatomy of de Quervain's disease. A study of operative findings.

We have studied the anatomical findings at operation on 67 patients with De Quervain's disease, 5% of whom had bilateral lesions, compared with published series of cadaveric dissections. There was division with a septum in the extensor compartment in 60%. The abductor pollicis longus had more than one tendon in 76% and extensor pollicis brevis was represented by a single tendon in 96%. If there was an anatomical predisposition to the condition, more cases would have been bilateral. A septum and a crowded compartment are mainly responsible for the failure of steroid injections.

Tuberculous tenosynovitis of the wrist mimicking de Quervain's disease.

We describe a rare case of tuberculous tenosynovitis involving the first dorsal compartment at the wrist mimicking de Quervain's disease. Differentiation between tuberculous infection and de Quervain's disease is important since corticosteroid injection, appropriate in the latter, is detrimental in tuberculous tenosynovitis. Early culture and biopsy in suspicious cases provide accurate diagnosis.

De Quervain's Disease: A Discourse on Etiology, Diagnosis, and Treatment.

Since Fritz De Quervain first postulated stenosing tenosynovitis within the radial dorsum of the wrist, much research has been conducted to provide further insights. De Quervain's Disease (DQD) is a condition that affects the tendons that control the movement of the thumb, specifically the abductor pollicis longus and extensor pollicis brevis. Numerous studies have shown that structural divergence from normal anatomy is partly related to contingency for developing DQD. Even though this condition was discovered many years ago, its exact etiology remains a subject of debate. Two schools of thoug

The Estrogen Receptor-β Expression in De Quervain's Disease.

Stenosing tenosynovitis of the first dorsal compartment of the wrist (a.k.a. de Quervain's disease) is common but how estrogen is involved is still unknown. We previously reported that inflammation was involved in the pathogenesis of this ailment. In the present study, we extended our investigation of estrogen receptor (ER)-β expression to determine whether estrogen is involved in the pathogenesis of de Quervain's. Intraoperative retinaculum samples were collected from 16 patients with the ailment. Specimens were histologically graded by collagen structure and immunohistochemically evaluated b

De Quervain's disease. Changing pathology or changing perception?

The basic anatomy of the first extensor compartment is presented with a review of the pathology of de Quervain's stenosing tenovaginitis. The results in 100 medicolegal cases of de Quervain's disease are analysed and reasons are sought for the poor results. A review of the known factors associated with the causation of de Quervain's disease is presented with recommendations about the management of this condition.

Complications of a simple procedure: de Quervain's disease revisited.

We reviewed the results of 22 operations performed on 21 patients for the relief of de Quervain's disease over the last six years. At a mean follow-up of 34 months (range 4-78 months), 18 of 22 wrists had complete relief of their original symptoms. One patient required reoperation because of inadequate decompression, and two others await further surgery. Most operations (14 out of 22) were performed under local anaesthesia, and 17 out of 22 used a longitudinal incision. The use of a longitudinal incision was associated with a significant risk of complications: four patients had a poor cosmetic

de Quervain thyroiditis in a young boy following hand-foot-mouth disease.

de Quervain thyroiditis, also known as subacute thyroiditis, is a self-limited inflammatory disease of the thyroid gland. It is extremely rare in children. The hallmarks for diagnosis are painful thyroid enlargement, elevated inflammatory markers, and decreased uptake of the thyroid gland on thyroid scintigraphy. Viral infection has been proposed to be associated with de Quervain thyroiditis. Coxsackie virus has been reported to be one of the viruses associated with the disease. To our knowledge, childhood de Quervain thyroiditis associated with hand-foot-mouth disease caused by coxsackie infe
