Inherited causes of combined vision and hearing loss: clinical features and molecular genetics.

Combined vision and hearing loss, also known as dual sensory impairment, can occur in several genetic conditions, including ciliopathies such as Usher and Bardet-Biedl syndrome, mitochondrial DNA disorders and systemic diseases, such as CHARGE, Stickler, Waardenburg, Alport and Alstrom syndrome. The retinal phenotype may point to the diagnosis of such disorders. Herein, we aim to provide a comprehensive review of the molecular genetics and clinical features of the most common non-chromosomal inherited disorders to cause dual sensory impairment.

GoCC4All a pervasive technology to provide access to TV to the deafblind community.

Considering the importance of communication and independence for the deafblind community, this work presents findings of the use of technology to address the lack of information due to communication challenges among the deafblind community. Over time, many investigations have been carried out regarding this matter, but very few providing solution, which is why this study emerged, looking to making all the information broadcasted through television accessible for this community. The work team designed a technology (GoCC4All) to address the needs of the deafblind community. GoCC4All provides acc

The relative influence of intellectual disabilities and autism on sensory impairments and physical disability: A whole-country cohort of 5.3 million children and adults.

BACKGROUND: Intellectual disabilities and autism are lifelong and often co-occur. Little is known on their extent of independent association with sensory impairments and physical disability. METHODS: For Scotland's population, logistic regressions investigated age-gender-adjusted odds ratios (OR) of associations, independently, of intellectual disabilities and autism with sensory impairments and physical disability. RESULTS: 1,548,819 children/youth, and 3,746,584 adults. In children/youth, the effect size of intellectual disabilities and autism, respectively, was as follows: blindness (OR =

[Focal brain lesions and language dysfunction].

To be a neurologist, training in cognitive/behavioral neuroscience is essential. In this review article, the author tries to emphasize the localization relationship between focal brain lesions and language dysfunction, by discussing from split-brain syndromes, hemispheric asymmetry, to pure word dumbness, pure word deafness (verbal auditory agnosia), pure agraphia, and pure word blindness (pure alexia; visual word agnosia), and finally to aphasia and aprosodia in general. Hopefully, after getting familiar with all the terms, the readers will feel free exploring language disturbances and behavi

Crossmodal plasticity in sensory loss.

In this review, we describe crossmodal plasticity following sensory loss in three parts, with each section focusing on one sensory system. We summarize a wide range of studies showing that sensory loss may lead, depending of the affected sensory system, to functional changes in other, primarily not affected senses, which range from heightened to lowered abilities. In the first part, the effects of blindness on mainly audition and touch are described. The latest findings on brain reorganization in blindness are reported, with a particular emphasis on imaging studies illustrating how nonvisual i

Conditions associated with Alzheimer's disease at death: case-control study.

To determine which conditions may be associated with reduced survival in patients with Alzheimer's disease, we studied all death certificates in the United States for 1978 on which senile and presenile dementia (ICDA 290, N = 7,195) was mentioned. Each case was compared with two control deaths. Differences in the frequency of listing on the death certificates for the following conditions reached statistical significance: infections, trauma, nutritional deficiency, chronic ulcer of skin, foreign body in pharynx, cataract, glaucoma, blindness, deafness, Parkinson's disease, and epilepsy. There s

A novel missense mutation in the NDP gene in a child with Norrie disease and severe neurological involvement including infantile spasms.

Norrie disease (ND) is a rare X-linked recessive disorder characterized by congenital blindness and in some cases, mental retardation and deafness. Other neurological complications, particularly epilepsy, are rare. We report on a novel mutation identified in a patient with ND and profound mental retardation. The patient was diagnosed at the age of 6 months due to congenital blindness. At the age of 8 months he developed infantile spasms, which were diagnosed at 11 months as his EEG demonstrated hypsarrhythmia. Mutation analysis of the ND gene (NDP) of the affected child and his mother revealed

Legg-Calve-Perthes' disease: an opportunity to prevent blindness?

Legg-Calve-Perthes' disease (LCP) is defined as avascular necrosis of the femoral head in a child and may present to a variety of disciplines from general practice to orthopaedics, paediatrics, rheumatology and more. The Stickler syndromes are a group of disorders of type II, IX and XI collagen associated with hip dysplasia, retinal detachment, deafness and cleft palate. The pathogenesis of LCP disease remains an enigma but there have been a small number of cases reporting variants in the gene encoding the α1 chain of type II collagen (COL2A1). Variants in COL2A1 are known to cause type 1 Stic

Edges of perception: balancing sensory loss and potential in assistive technology.

Being deafblind means my perception differs profoundly from those who are conventionally sighted and have non-impaired hearing. A lot of hidden knowledge is to be found in the disparity between these differing experiences that could be of great value in developing assistive technologies that have a broad scope to engage with both disabled and non-disabled users. This article explores the balancing act between sensory loss and the potential inherent in all of us and how this should be part of the design process of haptic assistive technology.Facing the true impact of my sensory loss, I realised

Sleep disturbances in CHARGE syndrome: types and relationships with behavior and caregiver well-being.

Children with CHARGE syndrome frequently develop moderate to severe behavior difficulties and are often diagnosed with obsessive-compulsive disorder, attention deficit disorder, Tourette syndrome, and autism. Anecdotal reports have indicated that sleep is also affected. However, the prevalence and types of sleep disturbance have not been identified. This study investigated sleep disturbances in 87 children with CHARGE syndrome, aged 6 to 18 years (mean 11y, SD 3y 8mo). There were 52 males and 35 females represented. Instruments included measures of sleep (Sleep Disturbances Scale for Children

Juvenile onset diabetes mellitus, central diabetes insipidus and optic atrophy (Wolfram syndrome)--neurological findings and prognostic implications.

The authors report on one case of Wolfram syndrome, a rare condition, which is characterized by juvenile onset diabetes mellitus, diabetes insipidus, optic atrophy and sensorineural deafness. The findings of this 13-year follow-up show that this patient developed typical neurological complications of long-standing diabetes mellitus as in the common type 1 variant. Moreover, some peculiar signs occurred such as anosmia, ophthalmoplegia interna, and central nystagmus. Since Wolfram syndrome is probably part of a more generalized neurodegenerative disorder, long-term prognosis will depend both up

Review of Genotype-Phenotype Correlations in Usher Syndrome.

Usher syndrome (USH) encompasses a group of clinically and genetically heterogenous disorders defined by the triad of sensorineural hearing loss (SNHL), vestibular dysfunction, and vision loss. USH is the most common cause of deaf blindness. USH is divided clinically into three subtypes-USH1, USH2, and USH3-based on symptom severity, progression, and age of onset. The underlying genetics of these USH forms are, however, significantly more complex, with over a dozen genes linked to the three primary clinical subtypes and other atypical USH phenotypes. Several of these genes are associated with

Proven Neonatal Neuroprotection Strategies: Magnesium Sulfate and Therapeutic Hypothermia.

Neurodevelopmental impairments follow both term and preterm birth, and include cerebral palsy (CP), cognitive dysfunction, and sensory impairment (blindness and deafness). Proven neuroprotective strategies include antepartum magnesium sulfate (MgSO4) administration prior to anticipated preterm delivery, delayed cord clamping, and therapeutic hypothermia for term and late preterm infants. MgSO4 administration prior to anticipated preterm delivery has been shown to reduce the risk of death, cerebral palsy and major neurodevelopmental disorders, whereas therapeutic hypothermia in term and later p

Assessing the pool activity level (PAL) checklist for use with people with hearing and vision loss.

BACKGROUND: The PAL is a career-completed assessment that indexes cognitive functional ability to inform individualised support. As hearing and vision loss are prevalent, we assessed the PAL for potential bias with hearing or vision impairment. METHODS: We collected PAL responses for 333 adults aged over 60 years in the UK, France, Canada, Greece and Cyprus. All participants had normal cognition based on self-reported status and normal range scores on a cognitive screening test. Using a Kruskal-Wallis test, we compared PAL item response distributions for people with assessed hearing or vision

Rehabilitation healthcare professionals' competence and confidence in differentially diagnosing deafblindness from autism spectrum disorders: a cross-sectional survey in South Africa.

BACKGROUND: Early diagnosis and management of children who are deafblind is important to alleviate the effects of deafblindness on the development of the child who is deafblind and their families. However, children who are deafblind are often misdiagnosed or diagnosed late. The misdiagnosis or late diagnosis has been attributed to many factors, one of which is the competence and confidence of healthcare professionals in differentially diagnosing deafblindness from other conditions, in most cases, autism spectrum disorder (ASD). The study therefore aimed to establish the competence and confiden

Common psychiatric disorders among the elderly attending a general psychiatric out patient clinic in Accra, Ghana: a five year retrospective study (1989-1993).

Details of thirty-five (35) elderly patients aged from sixty (60) years and upwards, who had attended a general psychiatric outpatient clinic, and duly registered as new patients between 1989-1993 were analysed. Depressive illness using the ICD-10 diagnostic criteria was found to be the predominant diagnostic category. This was followed by Dementia and Paranoid delusional disorder. No significant psychosocial contributory factors were elicited among the depressives. However, among the demerits there was a strong association with a previous history of excessive alcohol intake, especially the lo

Sox10 and Pax3 physically interact to mediate activation of a conserved c-RET enhancer.

The neurocristopathies encompass a spectrum of developmental disorders characterized by abnormalities of neural crest-derived structures. Neural crest cells are pluripotent progenitors and the mechanisms by which specific cell-fate decisions are regulated have emerged as an important field of study. Many neurocristopathies are characterized by defects in melanocyte differentiation that can result in pigmentation abnormalities and deafness. One example is Waardenburg syndrome that can be caused by mutations in the PAX3, SOX10 or MITF genes. Other neural crest-related disorders are associated wi

Osteopetrosis and its relevance for the discovery of new functions associated with the skeleton.

Osteopetrosis is a rare genetic disorder characterized by an increase of bone mass due to defective osteoclast function. Patients typically displayed spontaneous fractures, anemia, and in the most severe forms hepatosplenomegaly and compression of cranial facial nerves leading to deafness and blindness. Osteopetrosis comprises a heterogeneous group of diseases as several forms are known with different models of inheritance and severity from asymptomatic to lethal. This review summarizes the genetic and clinical features of osteopetrosis, emphasizing how recent studies of this disease have cont

The International Classification of Functioning, Disability and Health (ICF) core sets for deafblindness, part II of the systematic review: linking data to the ICF categories.

INTRODUCTION: Deafblindness, a health condition with varying combinations of hearing and vision impairment, affects functioning and social participation. In 2001, the World Health Organization (WHO) introduced the International Classification of Functioning, Disability, and Health (ICF) to examine human health and functioning. To use the ICF in clinical practice, smaller categories of ICF codes, referred to as Core Sets, were developed for specific health conditions. However, no ICF Core Set exists for deafblindness. As part of an ICF Core Set development, this paper examines the existing lite
