Arthroscopic debridement of the knee for septic arthritis.

Sixteen knees with hematogenous septic arthritis in 12 adult patients were treated by arthroscopic decompression, debridement, and irrigation with motorized instruments plus suction drainage. Infectious disease consultants supervised the administration of intravenous antibiotics, and range-of-motion exercises were instituted when the drains were removed 48 hours after surgery. Patients were protected from bearing weight for six weeks. There were no treatment complications, and no patient required a repeated drainage procedure. Two patients died of diseases unrelated to knee infection. The 11 i

A guide to debridement, antibiotics, and implant retention.

Debridement, antibiotics, and implant retention (DAIR) is an alternative management strategy for the treatment of periprosthetic joint infection (PJI). While infection eradication rates are lower with DAIR, the benefits including decreased morbidity, improved functional outcomes, and decreased cost may justify the risks when considering this form of treatment compared to traditional one or two stage exchange arthroplasty. Implant longevity in the setting of a successful DAIR is similar to matched patients who have not experienced a PJI. An experienced arthroplasty surgeon well versed in extens

Ether for Wound Debridement of Wild Maggots.

Myiasis is defined as an infestation of the organs or tissues of live vertebrates (humans or animals) by dipterous larvae, commonly referred to as maggots. Wound myiasis is a common presentation in the emergency department. The presence of wild maggots and larvae in wounds is concerning for subsequent myiasis-induced infections and complications. Proper extraction of wild maggots and larvae is required to avoid complications that may occur with wound myiasis. Surgical debridement is often utilized to extract wild maggots and larvae, but can be painful and time-consuming. There is limited liter

Role of Arthroscopic Debridement of Hemophilic Ankles.

BACKGROUND: Arthroscopic debridement has been used in the treatment of early stages of hemophilic ankle arthropathy. The aim of this procedure is to alleviate pain, improve function, and diminish bleeding episodes. Evaluation of patient-reported outcomes of arthroscopic debridement for hemophilic ankle arthropathies was evaluated in this study. METHODS: Fifteen patients with hemophilia who had undergone arthroscopic debridement between 2008 and 2017 were included in this study. Preoperative American Orthopaedic Foot & Ankle Society Score (AOFAS) and Foot Function Index (FFI) with its subscale

Periodontal debridement: still the treatment of choice.

UNLABELLED: Periodontal debridement (PD) remains a gold standard for the treatment of inflammatory periodontitis. BACKGROUND/PURPOSE: The evidence base regarding the causal relationship between oral biofilm and the host inflammatory response to the etiology of periodontal disease has substantially increased over the years. What has not changed significantly during that time is the conservative manner in which the disease can be treated with periodontal debridement (PD). Since dental hygienists, in particular, specialize in providing these procedures it is important to evaluate the evidence th

Maggot debridement therapy in the palliative setting.

Success rates of Maggot Debridement Therapy (MDT) differ, but range from 70% to 80%. In this article it is argued that wound closure is not always feasible and is not always the aim of the treatment. A patient is described in whom the intent of MDT was not wound closure, but infection removal, reduction of odor, and eventually prevention of a below knee amputation. This succeeded: the pain was diminished, the odor reduced, and the wound showed signs of healing. Still the patient died. In maggot literature, as with other wound treatments, outcome is recorded as closed or as failed. In our opini

Arthroscopic Elbow Debridement Using Anterocentral Transbrachialis Portal.

Arthroscopic debridement for elbow osteoarthritis has been widely used as a minimally invasive treatment; however, in some cases, it can be a technically demanding procedure, such as in patients with severe osteoarthritis with large spur formation and severe synovitis. The anterocentral transbrachialis portal is a recently developed portal for elbow arthroscopy, which allows easy and convenient access to the anterior compartment for sufficient debridement. This report describes in detail the anterocentral transbrachialis portal and its usefulness for debridement of the elbow joint in osteoarth

Successful thoracoscopic debridement of descending necrotizing mediastinitis.

Descending necrotizing mediastinitis results from odontogenic, deep neck infection that spreads along fascial planes into the mediastinum. Although the optimal surgical approach remains controversial, nearly half of the cases require mediastinal debridement by thoracotomy. We report a case of successful thoracoscopic debridement for descending necrotizing mediastinitis due to odontogenic infection that failed to be drained by transcervical approach. Because of less invasiveness as compared with standard thoracotomy, the thoracoscopic approach should be used as early as possible in case of unsu

Arthroscopic debridement alone for intercarpal ligament tears.

This study examined the role of arthroscopic debridement alone for complete and incomplete intercarpal ligament tears of the wrist. Forty-three wrists underwent arthroscopic evaluation for persistent wrist pain and were identified as having isolated scapholunate or lunotriquetral ligament tears treated by arthroscopic debridement alone of the torn ligament edges. At follow-up examination at an average of 27 months, 29 (66%) wrists having a complete scapholunate ligament tear and 36 (85%) wrists having a limited scapholunate ligament tear had either complete symptom resolution or improved sympt

Arthroscopic debridement for degenerative knee joint disease.

A series of 77 knees in 72 patients, ages ranging from 24 to 78 years (mean, 56 years), with moderate or severe degenerative arthritis, were treated by percutaneous debridement of the joint under arthroscopic visualization. Three per cent had a previous meniscectomy, and 81 percent had a tear of at least one meniscus. Additional pathologic problems included loose bodies in 21 percent, absent anterior cruciate ligaments in 13 percent, adhesions in 9 percent and chondrocalcinosis in 9 percent. Sixty-two patients with 68 knees were followed for at least six months, with a mean follow-up of 13.6 m

Arthroscopic debridement for soft tissue ankle impingement.

AIM: To assess the response to treatment in patients with soft tissue impingement of the ankle managed with arthroscopic debridement. METHODS: Forty-one ankle arthroscopies were performed for soft tissue impingement between April 2007 and April 2009. There were 26 men and 15 women and the mean age was 30.1 years. Arthroscopy was performed on an average of 21 months after injury. The Visual-Analogue-Scale Foot and Ankle (VASFA) score and Meislin's criteria were used to assess the response to treatment. RESULTS: The mean pre-operative VASFA score was 44.5. This increased to 78.3 postoperativel

Wound debridement: therapeutic options and care considerations.

Wound debridement is a critical component of promoting optimal healing for a wound with necrotic tissue. Although much is known about the multiple barriers to healing that necrotic detritus presents, much is still unknown about the best ways, timing, and approaches to constructing a healthy wound bed. Falanga's "black box," a metaphor for the unknown components of wound healing and debridement, should remind all practitioners that future research needs to address the continuing questions and issues associated with promotion of quality chronic wound healing outcomes.

Arthroscopic Extensor Carpi Radialis Brevis Tenotomy and Debridement Versus Debridement Alone for Refractory Lateral Epicondylitis: Clinical and MRI Evaluation.

BACKGROUND: Arthroscopic debridement of the extensor carpi radialis brevis (ECRB) tendon is a widely accepted procedure used in refractory lateral epicondylitis. However, residual pain occurs in some patients. PURPOSE: To investigate the clinical effectiveness of arthroscopic extended debridement (ECRB tenotomy and debridement) in the treatment of lateral epicondylitis. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Patients with refractory lateral epicondylitis were consecutively recruited for this study. They underwent traditional ECRB debridement (control group) or extended E
