Macrodactyly: decision-making and surgery timing.

Macrodactyly is a rare condition in which fingers, hands or limb growth is unregulated, resulting in overgrowth of tissues in the affected extremities. It is critical to properly assess these extremities for signalling pathway, psychological impact and potential surgical intervention, to achieve the best possible outcome for each patient. Treatment approaches can vary, and patient and family expectations weigh heavily on care complexity. Common surgical procedures may include epiphysiodeses, osteotomies, debulking procedures, carpal tunnel releases, toe transfers and amputations. The selection

Dynamic Selective Exposure during Decision-Making.

To understand dynamic changes in the likelihood that people would access and selectively expose themselves to information online, the present study examined the checking of account balances during simulated gambling. Sixteen participants played 120 hands of computer Blackjack for points, at higher or lower levels of risk (different point multipliers), and after each win or loss the computer recorded if participants checked their account balances. There were individual differences in checking rates. Participants who were more likely to check balances exhibited a selectivity of exposure to decis

Social Influences in Sequential Decision Making.

People often make decisions in a social environment. The present work examines social influence on people's decisions in a sequential decision-making situation. In the first experimental study, we implemented an information cascade paradigm, illustrating that people infer information from decisions of others and use this information to make their own decisions. We followed a cognitive modeling approach to elicit the weight people give to social as compared to private individual information. The proposed social influence model shows that participants overweight their own private information rel

Rodent models of adaptive decision making.

Adaptive decision making affords the animal the ability to respond quickly to changes in a dynamic environment: one in which attentional demands, cost or effort to procure the reward, and reward contingencies change frequently. The more flexible the organism is in adapting choice behavior, the more command and success the organism has in navigating its environment. Maladaptive decision making is at the heart of much neuropsychiatric disease, including addiction. Thus, a better understanding of the mechanisms that underlie normal, adaptive decision making helps achieve a better understanding of

Decision-making deficits among maltreated children.

Although maltreated children involved with child welfare services are known to exhibit elevated levels of health-risking behaviors, little is known about their decision-making processes leading to such tendencies. Research findings suggest that maltreated children exhibit developmental delays in neurocognitive and emotional regulation systems that could adversely impact their abilities to make decisions under conditions of risk. Whereas prior researchers have examined risky decision making as a global construct, maltreated children's decision making was examined in two contexts in the present

Decision making in narcolepsy with cataplexy.

OBJECTIVES: To investigate decision-making and addictive behaviors in narcolepsy-cataplexy (NC). NC is caused by the loss of hypothalamic neurons that produce hypocretins. The hypocretin system plays a crucial role in sleep, wakefulness, and energy homeostasis, and is also involved in emotion regulation, reward processing, and addiction. SETTING: Academic sleep center. PATIENTS: 23 subject with NC and 23 matched healthy controls. DESIGN: We used the Iowa Gambling Task (IGT) to assess decision making under ambiguity condition based on emotional feedback processing and the Game of Dice Task (

Academic decision making and prospect theory.

Two studies are reported that investigate the applicability of prospect theory to college students' academic decision making. Exp. 1 failed to provide support for the risk-seeking portion of the fourfold pattern predicted by prospect theory but did find the greater weighting of losses over gains. Using a more sensitive dependent measure, in Exp. 2 the results of the first experiment were replicated in terms of the gain-loss effect and also found some support for the fourfold pattern in the interaction between probabilities and gain versus loss. The greatest risk-seeking was found in the high p

Decision making in adults with ADHD.

OBJECTIVES: This study examined decision-making competence in ADHD by using multiple decision tasks with varying demands on analytic versus affective processes. METHODS: Adults with ADHD and healthy controls completed two tasks of analytic decision making, as measured by the Adult Decision-Making Competence (A-DMC) battery, and two affective decision tasks (the Balloon Analog Risk Task and the lowa Gambling Task). RESULTS: Although a majority of the ADHD participants were tested under medication, they showed impairments in both types of task. However, logistic regression analysis showed that

Decision-making of vestibular schwannoma patients.

BACKGROUND: Patients suffering from vestibular schwannoma can choose between four modalities of management: (1) wait-and-scan, (2) tumour resection, (3) radiosurgery and (4) fractionated radiotherapy. METHOD: This study is based on postal questionnaire survey of 739 vestibular schwannoma patients (survey response rate, 78%). It not only investigates the decision-making of patients, especially the medical consultation, but also further influences on the therapy decision and the patients' evaluation of possible side-effects of the treatment. RESULTS: Only a minority of the patients was informe

Brain correlates of risky decision-making.

Understanding the neurocognitive basis of risk-taking behavior is an important issue, especially in economic decision-making. Classical behavioral studies have shown that risk-attitude changes across different contexts, but little is so far known about the brain correlates of processing of outcomes across such context shifts. In this study, EEG was recorded while subjects performed a gambling task. Participants could choose between a risky and a safer option, within two different contexts: one in which options yielded gains and losses of the same magnitude (Zero Expected Value context) and ano

Decision making in primary sacral tumors.

BACKGROUND CONTEXT: Primary tumors of the sacrum are extremely rare lesions. Their management is governed by an interplay of complex factors. Appropriate decision making is crucial to obtain the best possible outcome in terms of maximizing disease control while attempting to minimize neurological dysfunction. PURPOSE: Our study presents the results of a group of patients with primary tumors of the sacrum who were surgically treated by the same multidisciplinary team at a specialist oncology center over a relatively short period of time (5 years). STUDY DESIGN/SETTING: Patients were identifie

Parental decision-making in childhood vaccination.

Recent concerns about childhood vaccines such as the measles, mumps and rubella (MMR) vaccine have led to reduced vaccine uptake and increased probability of disease outbreaks. Psychological aspects of parental decision-making about vaccines are reviewed. Inconsistencies and gaps in the literature are highlighted and implications of what is known for public health are outlined. A decision theory model of the decision to vaccinate fits the facts well and generates practical strategies for uptake of MMR and similar childhood vaccines.

Decision making in contemporary shoulder arthroplasty.

Clinical experience with humeral implants has evolved over the past decade, along with a better understanding of shoulder anatomy and function. There is no question that surgeons are getting better at restoring normal anatomic relationships than in preceding decades. Whether or not this impacts implant longevity will only be known with time and further follow-up. Even over the short term, it is difficult to ascertain whether new prosthetic designs have improved patient function as well as has been implied by the related biomechanical studies. Most surgeons with experience using old and new sys

Osseointegration and self-management decision making.

PURPOSE: This study explores how individuals living with limb osseointegration (OI) make decisions about their self-management, including how they seek, evaluate, and apply health information. MATERIALS AND METHODS: Using a constructivist grounded theory approach, ten participants with OI from Canada, the United States, and Australia were interviewed via Zoom. Transcripts were analyzed using constant comparative methods to identify themes and develop a theory of self-management decision-making. RESULTS: Participants described a multifaceted decision-making process across four stages of their

Clinical Decision-Making Case: Febrile Infant.

AUDIENCE: This clinical decision-making (CDM) case is intended for emergency medicine residents of all levels, medical students, and fellows preparing for standardized oral board exams. INTRODUCTION: Fever in a neonate (infant <28 days old) is a medical emergency due to the high risk of serious bacterial infections (SBIs) like meningitis, sepsis, or urinary tract infections (UTIs).1-3 Compared with older infants and children, neonates have immature immune responses, reduced ability to localize infection, and limited physiologic reserve, which contribute to rapid clinical deterioration and inc

Improving shared decision-making in bronchiectasis.

Bronchiectasis is a heterogeneous lung disease. There is an increasing focus on personalised medicine in bronchiectasis, with targeted pharmacological interventions for inflammation, immunology and infection. Airway clearance techniques (ACTs) are non-pharmacological treatments used to manage bronchiectasis. Approximately half of patients with bronchiectasis perform ACTs. There have been attempts to personalise ACT prescriptions, including consideration of patient physiology, disease status and psychosocial factors. Guidelines suggest that patient preference or choice should be considered when

Decision-making in Hallux Valgus Correction.

Hallux valgus deformity is the most common surgical indication in the foot. The achievable result is highly dependent on preoperative planning and the choice of surgical procedure. Consideration of anatomic specifications, angulation at the MTP and intermetatarsal 1/2 (IMT), and joint status at the metatarsophalangeal and TMT joints provide the basis for this. Minor, moderate, and severe deformities can be assigned to different osteotomy heights and, within these, a surgical procedure. Each method, if applied appropriately, is suitable and its correct use allows simple operations with maximum
