Shared Decision-Making for Lung Cancer Screening: A Systematic Review.

BACKGROUND: Lung cancer screening (LCS) with annual low-dose CT scanning is recommended for eligible individuals. The Centers of Medicare and Medicaid Services stipulates that counseling and shared decision-making (SDM) are a prerequisite for LCS reimbursement; however, SDM effects and optimal strategies are poorly understood. RESEARCH QUESTION: What are the effects of LCS SDM tools and strategies? STUDY DESIGN AND METHODS: This was a systematic review of US-based randomized controlled trials or observational studies of LCS SDM with a comparator. We searched Embase and MEDLINE via Ovid and C

[Sex-Based Differences and Shared Decision-Making in Myasthenia Gravis].

Myasthenia gravis is an autoimmune disease of the neuromuscular junctions. Studies have shown that its prevalence is slightly higher in females, that female patients are more refractory to treatment, and that their post-treatment quality of life is more difficult to improve. Recently, multiple novel treatment options have become available, and more opportunities for shared decision-making are being observed in the process of treatment selection. Thus, it is necessary to choose the best treatment for each patient through detailed explanations regarding the disease and adequate consideration of

[Shared decision-making in acute life-threatening situations: two cases].

When making critical treatment decisions, shared decision-making (SDM) between healthcare providers and patients is essential. SDM involves discussing care options, considering patient preferences, and ensuring decisions align with patient values and medical conditions. This process becomes challenging in life-threatening emergencies, where time constraints hinder thorough discussions and coordination among healthcare providers, potentially leading to inappropriate care. Two cases highlight these challenges. Patient A, a 76-year-old man with acute aortic dissection, underwent surgery without c

The argumentative role of patient companions in (shared) decision-making.

OBJECTIVE: This study aims to examine the type of involvement of patient companions in the argumentative exchanges in consultations and explore when their contributions should be taken into account in shared decision-making (SDM). METHODS: A qualitative analysis was carried out using transcribed medical consultations (N = 10) between health professionals (doctors at a regional Dutch hospital), adult patients and informal patient companions. Insights from argumentation theory were used to develop an inventory of twelve theoretically distinct discussion situations involving patient companions,

Cognitive Biases and Shared Decision Making in Acute Brain Injury.

Many patients hospitalized after severe acute brain injury are comatose and require life-sustaining therapies. Some of these patients make favorable recoveries with continued intensive care, while others do not. In addition to providing medical care, clinicians must guide surrogate decision makers through high-stakes, emotionally charged decisions about whether to continue life-sustaining therapies. These consultations require clinicians first to assess a patient's likelihood of recovery given continued life-sustaining therapies (i.e., prognosticate), then to communicate that prediction to sur

Shared decision-making interventions for people with mental health conditions.

BACKGROUND: One person in every four will suffer from a diagnosable mental health condition during their life. Such conditions can have a devastating impact on the lives of the individual and their family, as well as society.  International healthcare policy makers have increasingly advocated and enshrined partnership models of mental health care. Shared decision-making (SDM) is one such partnership approach. Shared decision-making is a form of service user-provider communication where both parties are acknowledged to bring expertise to the process and work in partnership to make a decision.

Ethics and shared decision-making in paediatric occupational therapy practice.

BACKGROUND: Sharing information and decisions with children and their parents is critical in pediatric rehabilitation. Although the ethical significance and clinical benefits of sharing decisions are established, approaches for implementing shared decision-making in clinical practice are still developing. AIM: To explore the ethical challenges of sharing information and decisions with one family in pediatric occupational therapy. METHOD: We used a single qualitative in-depth interview with an occupational therapist to examine the ethical dimensions of sharing decisions. RESULTS: We found th

Patient Recommendations for Shared Decision-Making in Uterine Fibroid Treatment Decisions.

Uterine fibroids are noncancerous tumors associated with significant morbidity among symptomatic patients. While medical and surgical treatments have expanded, hysterectomy remains common. We interviewed women diagnosed with uterine fibroids (n = 47) to explore their experiences and recommendations for shared decision-making. A majority were non-Hispanic Black, highly educated (51.1%), and had graduate degrees (40.4%). Participants with both positive and negative provider experiences expressed a desire for a more proactive therapeutic approach, including a presentation by their provider of the

Nurse-led shared decision making with patients: A scoping review.

BACKGROUND: Shared decision making has gained increasing attention as part of person-centred care. However, research on shared decision making has largely focused on physician-patient interactions, and little is known about nurse-led shared decision making between nurses and patients. PURPOSE: To identify and synthesise empirical evidence on nurse-led shared decision making with patients or families, examining: 1) contexts and decision types, 2) intervention components, 3) reported outcomes and experiences, and 4) guiding frameworks. METHODS: A scoping review was conducted following Arksey a

Shared Decision Making in Colorectal Cancer Screening: A Scoping Review.

INTRODUCTION: Colorectal cancer (CRC) is easily detectable with screening, but due to a variety of factors, over a third of eligible people do not get screened. One barrier to people getting adequate screening is confusion about what type of test to use. Shared decision making (SDM) is a way for the clinician to help the patient decide about whether to get screened for CRC and if so, which test to use. This scoping review examines literature about shared decision making in CRC screening to identify key factors affecting a patient's decision. METHODS: We identified studies published from Janua

Shared Decision Making About Starting Anti-TNFs: A Pediatric Perspective.

Shared decision making (SDM) is central to patient-centered medicine and has the potential to improve outcomes for pediatric patients with inflammatory bowel diseases. We surveyed specialists about their use of SDM in the decision to start a tumor necrosis factor-α inhibitor in pediatric patients. Results were compared between those who reported using SDM and those who did not. Of 209 respondents, 157 (75%) reported using SDM. Physician/practice characteristics were similar between users and nonusers. There were no statistically significant differences between groups in the components deemed i

Toward personalized shared decision-making: a pilot study on an individualized patient decision aid.

Decision aids are tools that can reduce decisional conflict and enhance patient satisfaction in the decision-making process. However, tool heterogeneity and clinical barriers, such as physician time constraints, hinder their implementation. This study introduces an individualized patient decision aid, developed within the European I3LUNG project, to support shared decision-making (SDM) in non-small cell lung cancer. The present decision aid is a dynamic, online tool structured into five sections (overview, prognosis/treatments, comprehension quizzes, patient preferences, and support/FAQs) desi
