BACKGROUND: Treatment decision tools have been developed in many fields of medicine, including psychiatry, however benefits for patients have not been sustained once the support is withdrawn. We have developed a web-based computerised clinical decision support tool (CDST), which can provide patients and clinicians with continuous, up-to-date, personalised information about the efficacy and tolerability of competing interventions. To test the feasibility and acceptability of the CDST we conducted a focus group study, aimed to explore the views of clinicians, patients and carers.

BACKGROUND: Chronic pain has emerged as a disease in itself, affecting a growing number of people. Effective patient-provider communication is central to good pain management because pain can only be understood from the patient's perspective. We aimed to develop a user-centered tool to improve patient-provider communication about chronic pain and assess its feasibility in real-world settings in preparation for further evaluation and distribution.
METHODS: To identify and prioritize patient treatment goals for chronic pain, strategies to improve patient-provider communication about chronic pai

BACKGROUND: Adolescent knee pain was historically viewed as a self-limiting condition. Still, it may severely impact health-related quality of life and physical activity, and almost half of adolescents may continue to experience pain into adulthood. Currently, no tool is available to support the consultation and shared decision-making process when an adolescent suffering from non-traumatic knee pain presents at clinical practice. By supporting shared decision-making and tailoring management strategies, such a tool could optimize treatment delivery and improve the prognosis of this common condi

BACKGROUND: High-alert medications are frequently responsible for adverse drug events and present significant hazards to inpatients, despite technical improvements in the way they are ordered, dispensed, and administered.
METHODS: A real-time surveillance application was designed and implemented to enable pharmacy review of high-alert medication orders to complement existing computerized provider order entry and integrated clinical decision support systems in a tertiary care hospital. The surveillance tool integrated real-time data from multiple clinical systems and applied logical criteria t

Temporal abstraction (TA) provides the means to instil domain knowledge into data analysis processes and allows transformation of low level numeric data to high level qualitative narratives. TA mechanisms have been primarily applied to uni-dimensional data sources equating to single patients in the clinical context. This paper presents a framework for multi-dimensional TA (MDTA) enabling analysis of data emanating from numerous patients to detect multiple conditions within the environment of neonatal intensive care. Patient agents which perform temporal reasoning upon patient data streams are

The U.S. Department of Defense (USDOD) service members are at risk of exposure to ionizing radiation due to radiation accidents, terrorist attacks and national defense activities. The use of biodosimetry is a standard of care for the triage and treatment of radiation injuries. Resources and procedures need to be established to implement a multiple-parameter biodosimetry system coupled with expert medial guidance to provide an integrated radiation diagnostic system to meet USDOD requirements. Current USDOD biodosimetry capabilities were identified and recommendations to fill the identified gaps

Decision-support systems in medicine should be equipped with a facility that provides patient-tailored information about which test had best be performed in which phase of the patient's management. A decision-support system with a good test-selection facility may result in ordering fewer tests, decreasing financial costs, improving a patient's quality of life, and in an improvement of medical care in general. In close cooperation with two experts in oncology, we designed such a facility for a decision-support system for the staging of cancer of the oesophagus. The facility selects tests based

It is not enough to merely manage medical information. It is difficult to justify the cost of hospital information systems (HIS) or intensive care unit (ICU) patient data management systems (PDMS) on this basis alone. The real benefit of an integrated HIS or PDMS is in decision support. Although there are a variety of HIS and ICU PDMS systems available there are few that provide ICU decision support. The HELP system at the LDS Hospital is an example of a HIS which provides decision support on many different levels. In the ICU there are decision support tools for antibiotic therapy, nutritional

This paper presents an overview of computerised decision support for clinical practice. The concept of computer-interpretable guidelines is introduced in the context of the @neurIST project, which aims at supporting the research and treatment of asymptomatic unruptured cerebral aneurysms by bringing together heterogeneous data, computing and complex processing services. The architecture is generic enough to adapt it to the treatment of other diseases beyond cerebral aneurysms. The paper reviews the generic requirements of the @neurIST system and presents the innovative work in distributing exe

Effective decision support systems rely on a clear understanding of real-world conditions and on developing strategies to manage them. In the current era of severe climate change, crop modelling has emerged as a valuable tool in fruit production. It enables farmers and researchers to mitigate the adverse effects of biotic and abiotic stresses by providing scientifically grounded, practical, and forward-looking solutions. This review aims to evaluate the influence of climate and management practices on fruit quality and to highlight how crop modelling can address challenges related to crop grow

Although evidence-based decision-making in dentistry is quickly evolving, large gaps remain in our clinical knowledge base regarding every day decisions and procedures. Especially in the absence of strong evidence, as is the case with mucogingival conditions, risk assessment and identification are important components of the clinical decision-making process. Utilization of clinical decision support (CDS) guides, frameworks and systems enhances chairside decision-making and improves delivery of patient care. This article introduces an Evidence-Based Clinical Decision Support Guide for mucogingi

BACKGROUND: AI systems are increasingly being evaluated for their potential role in medical decision-making. Pulmonary thromboembolism (PTE) represents an ideal test domain for evaluating AI clinical reasoning capabilities due to its high prevalence, significant mortality risk, and clinical complexity requiring integration of validated risk stratification tools, multiple imaging modalities, and nuanced treatment algorithms across diverse patient populations, including pregnancy, malignancy, and renal impairment. We compared the performance of large language models (LLMs) with specialist physic

The process of merging and benchmarking clinical and financial data is pivotal to the development of appropriate clinical pathways. Bristol Regional Medical Center (BRMC), facing the challenge of managed care organizations (MCOs), instituted this process and achieved significant cost savings, largely because of the working partnership between the administration and its medical staff. In DRG 89, Simple Pneumonia and Pleurisy, Age Greater than 17 with CC, data adjusted for severity of illness and cost of living were furnished to BRMC by HCIA Inc. Major benchmark or "best practice" variations wer

Patients are becoming more and more involved in clinical decision-making process. Several factors support this process. Advances in omics allows individualization of diagnosis and treatment. Patient awareness and easy availability of data on the Internet allows patients to become informed decision makers when it comes even to disease management. Mass media emphasize the issue of medical errors, making patients demanding for quality in medical care. In some healthcare settings, patents face a problem of interpreting medical data and making decisions on treatment tactics without having a doctor,

Wearable continuous glucose monitoring (CGM) sensors are revolutionizing the treatment of type 1 diabetes (T1D). These sensors provide in real-time, every 1-5 min, the current blood glucose concentration and its rate-of-change, two key pieces of information for improving the determination of exogenous insulin administration and the prediction of forthcoming adverse events, such as hypo-/hyper-glycemia. The current research in diabetes technology is putting considerable effort into developing decision support systems for patient use, which automatically analyze the patient's data collected by C
