Detection of right to left shunts in decompression sickness in divers.

The diagnostic accuracy of bubble contrast transthoracic and transesophageal echocardiography and transcranial Doppler sonography in the detection of patent foramen ovale in divers with decompression sickness was assessed. Transcranial Doppler has a better positive and negative predictive value than the other modalities.

Decompression sickness in women: a possible relationship with the menstrual cycle.

BACKGROUND: Women are increasingly participating in recreational scuba diving and the professional roles of women are expanding in the fields of aviation, space, and diving. Evidence exists that there may be a relationship between altitude decompression sickness (DCS) and the menstrual cycle, although diving studies to support such findings are limited. The aim of the present study was to investigate the presence of any relationship between the development of DCS in female sports divers, the phase of the menstrual cycle, and the use of the oral contraceptive pill (OCP). METHOD: Personal, dive

An evaluation of plasma volume expanders in the treatment of decompression sickness.

Each of 29 goats was instrumented with an ultrasonic flowmeter transducer around the left common carotid artery and a silastic catheter in the anterior vena cava. Following recovery from the surgery the goats were subjected to a dive protocol designed to elicit symptoms of decompression sickness (DCS). The goats were returned to the surface and, following a 20-min observation period, were either left untreated (controls) or treated with one of five different plasma volume expanders. The plasma volume expanders were evaluated based on their ability to decrease the severity of DCS and the number

Magnetic resonance imaging in breath-hold divers with cerebral decompression sickness.

The mechanism of cerebral decompression sickness (DCS) is still unclear. We report 2 cases of breath-hold divers with cerebral DCS in whom magnetic resonance imaging (MRI) demonstrated distinctive characteristics. One case presented right hemiparesthesia, diplopia, and gait disturbance after breath-hold diving into the sea at a depth of 20 m. Brain MRI with fluid-attenuated inversion recovery (FLAIR) sequence revealed multiple hyperintense lesions in the right frontal lobe, bilateral thalamus, pons, and right cerebellar hemisphere. The second case presented visual and gait disturbance after re

Recurrent inner ear decompression sickness associated with a patent foramen ovale.

Isolated inner ear injuries occurring during shallow scuba dives are an uncommon manifestation of decompression sickness in recreational divers. We describe a patient who presented with the typical symptoms of inner ear involvement after 2 independent dives within the decompression limits. The diver reported symptoms of unilateral (right-sided) hearing loss, tinnitus, and vertigo after dives to 35 and 50 m. After treatment with hyperbaric oxygen, his symptoms completely resolved. To confirm the hypothesis of inner ear decompression sickness (IEDCS), we examined the patient for a right-to-left

Diving methods and decompression sickness incidence of Miskito Indian underwater harvesters.

Diving conditions, dive profiles, and symptoms of decompression sickness (DCS) in a group of Miskito Indian underwater seafood harvesters are described. Dive profiles for 5 divers were recorded with dive computers, and DCS symptoms were assessed by neurological examination and interview. Divers averaged 10 dives a day over a 7-day period with a mean depth of 67 +/- 7 FSW (306 +/- 123 kPa) and average in-water time of 20.6 +/- 6.3 minutes. Limb pain was reported on 10 occasions during 35 man-days of diving. Symptoms were typically managed with analgesic medication rather than recompression. Ind

Magnetic resonance findings in scuba diving-related spinal cord decompression sickness.

Scuba diving is associated with risk of severe decompression sickness (DCS type II), which results from rapid reduction of the environmental pressure sufficient to cause the formation into tissue or blood of inert gas bubbles previously loaded within tissues as a soluble phase. DCS type II constitutes a unique subset of ischemic insults to the central nervous system (CNS) with primarily involvement of the spinal cord. Ten patients with diving-related barotrauma underwent neurologic examination. Two of them presented progressive sensory and motor loss in the extremities at admission and were pr

Progression of Cutis Marmorata Following Initial Hyperbaric Therapy for Decompression Sickness.

We present a 33-year-old diver who developed decompression sickness after a 26-m dive. Initially improving with hyperbaric oxygen therapy, his lower abdomen rash unexpectedly progressed to diffuse cutis marmorata. He required multiple additional recompression sessions for complete resolution, highlighting the need for close monitoring.

Decompression sickness in Miskito Indian lobster divers: review of 229 cases.

BACKGROUND: The Miskito Indian lobster divers of Central America employ very provocative diving profiles and experience severe neurological decompression sickness (DCS) and/or arterial gas embolism (AGE). Scientific data are scarce regarding the clinical patterns of injury, response to treatment, and functional outcomes for such cases. METHODS: A retrospective review of 229 cases of DCS and/or AGE was conducted at 2 hyperbaric units in Central America. RESULTS: The following deficits were recorded on presentation: any neurological deficit: 94%; motor: 79%; sensory: 60%; urinary: 48%; reflex:

Oxy-helium treatment for refractory neurological decompression sickness: a case report.

A 49-yr-old male presented with paraparesis and urinary incontinence that appeared 10 min after surfacing from a dive. Treatment was started on an extended USN table 6, but the symptoms persisted. Twenty-four hours later, he was treated with oxy-helium table CX-30, resulting in marked improvement in gait and in sensory and motor function. Urodynamic examination indicated an upper motor neuron lesion; bilateral decreased amplitude of the somatosensory evoked potential was found on stimulation of the tibial nerves; no response below the upper limbs was elicited on central motor conduction time (

An observation of venous gas emboli in divers and susceptibility to decompression sickness.

INTRODUCTION: Decompression sickness (DCS) results from the formation of bubbles within the tissues and blood in response to a reduction in environmental pressure. Venous gas emboli (VGE) are common after diving and are usually only present in small numbers. Greater VGE numbers are an indication of decompression stress, and can be reliably detected using ultrasound imaging. AIM: To examine the relationship between production of VGE following a routine dive and the risk of DCS. METHODS: A matched population of divers with and without a history of DCS were monitored for the production of VGE a

[Rare case of brain damage in a primary decompression sickness in a diver].

Pressure disease causes very infrequently multifocal injury to the cerebral hemispheres with resulting organic brain disease. The described case was observed in a diver who after correctly performed decompression without any evident faults in decompressions during several years of diving suffered diffuse, multifocal injury to the cerebral hemispheres and right vestibular nuclei. The authors stress the fact that decompression tables differ in various countries. There is no uniform safety decompression table for the divers. Besides that, such factors as individual sensitivity and resistance of t

Changes in blood enzyme activity and hematology of rats with decompression sickness.

Plasma activities of lactic dehydrogenase (LDH), creatine phosphokinase (CPK), glutamic pyruvic transaminase (GPT), and alkaline phosphatase (ALP) were studied, along with hematological changes, in rats suffering from various degrees of experimental decompression sickness (DS). By 1 h after decompression, LDH and CPK activities were elevated in moderate and severe DS whereas GOT and GPT were elevated only in severe DS. ALP was reduced in all decompressed rats. Hematological changes indicated hemoconcentration, the degree of which paralleled the severity of DS. By 24 h after decompression, all

Neuroinflammation with increased glymphatic flow in a murine model of decompression sickness.

This project investigated glial-based lymphatic (glymphatic) function and its role in a murine model of decompression sickness (DCS). DCS pathophysiology is traditionally viewed as being related to gas bubble formation from insoluble gas on decompression. However, a body of work implicates a role for a subset of inflammatory extracellular vesicles, 0.1 to 1 µm microparticles (MPs) that are elevated in human and rodent models in response to high gas pressure and rise further after decompression. Herein, we describe immunohistochemical and Western blot evidence showing that following high air pr

Blood pressure in rats selectively bred for their resistance to decompression sickness.

INTRODUCTION: Susceptibility to decompression sickness (DCS) is characterised by a wide inter-individual variability whose origins are still poorly understood. This hampers reliable prediction of DCS by decompression algorithms. We previously selectively bred rats with a 3-fold greater resistance to DCS than standard rats. Based on its previously reported relation with decompression outcomes, we assessed whether modification in vascular function is associated with resistance to DCS. METHODS: The arterial pressure response to intravenous administration of acetylcholine (ACh, 5 μg.kg-1) and adr

Cutis marmorata in decompression sickness is associated with a patent foramen ovale.

A 39-year-old male commercial diver developed cutis marmorata after a dive. He had a full recovery after therapy in a hyperbaric oxygen chamber. Transthoracic echocardiography revealed an atrial septal aneurysm and a large shunt during normal respirations. This form of decompression sickness may progress to type II DCS, thus is important to identify and treat. Cutis marmorata as a result of diving is highly associated with an atrial septal defect or a large patent foramen ovale. It is particularly important to assess these patients for a right-to-left shunt as part of a medical evaluation prio
