Variables influencing manometric parameters of deglutitive and non-deglutitive upper esophageal sphincter: A study of 89 asymptomatic participants.

BACKGROUND: High-resolution manometry overcomes limitations imposed by axial and radial asymmetry as well as swallow and respiration-induced movement of the upper esophageal sphincter (UES), allowing for its reliable use in evaluation of dysphagic and reflux patients. The aim of this study was to determine normative values and their variability across position, sex, age, height, weight, BMI, and volume for clinically relevant deglutitive and non-deglutitive UES parameters. METHODS: We studied 89 asymptomatic volunteers age 19-90 years, (45 female) during 3-10 repetitions of dry, 5 and 10 ml w

Factors influencing lower esophageal sphincter relaxation after deglutition.

AIM: To study the relationship between upper esophageal sphincter (UES) relaxation, peristaltic pressure and lower esophageal sphincter (LES) relaxation following deglutition in non-dysphagic subjects. METHODS: Ten non-dysphagic adult subjects had a high-resolution manometry probe passed transnasally and positioned to cover the UES, the esophageal body and the LES. Ten water swallows in each subject were analyzed for time lag between UES relaxation and LES relaxation, LES pressure at time of UES relaxation, duration of LES relaxation, the distance between the transition level (TL) and the LES

Deglutition after supracricoid laryngectomy: compensatory mechanisms and sequelae.

This study is based on the videofluorographic exploration of deglutition in 14 patients who were treated by supracricoid laryngectomy. The choice of this population rests on two criteria: a 1-year postoperative delay, and absence of residual deglutition disorders elicited by patient history. Asymptomatic aspiration was seen in 6 cases. In the cricohyoidoepiglottopexies (CHEP), aspiration occurred uniquely in patients who did not recuperate satisfactorily from epiglottic dynamics. The deglutition sequelae are less invalidating relative to the cricohyoidopexies (CHP), with a possible recuperatio

[Radiologic differential diagnosis of neurologically-induced deglutition disorders].

This paper explains the differential diagnosis of neurological dysphagia. Special groups of diseases are described that can only be assessed by dynamic recording modalities like videofluoroscopy and high-speed cineradiography. The need for dynamic recording results from the physiological data of deglutition. The act of deglutition lasts only 0.7 s, but requires the well-coordinated action of 5 cranial nerves and 26 muscle groups. For a systematic analysis of the underlying neurological disease a precise description of possible pathological single observations from the oral cavity to the cardia

The pharyngeal phase of swallowing controls deglutitive inhibition.

The goal of this study was to determine which phase of swallowing causes deglutitive inhibition (DI). DI is inhibition of ongoing peristalsis by a new swallow. Pharynx and esophagus of decerebrate cats (n = 25) were instrumented to record esophageal peristalsis. Injecting saline into pharynx or esophagus activated primary (pharyngeal and esophageal phases) peristalsis repeatedly and randomly, or secondary (esophageal phase) peristalsis once, respectively. When a new primary peristalsis occurred during ongoing primary peristalsis, the occurrence of ongoing esophageal peristalsis was blocked whe

The Impact of Vocal Tremor on Deglutition: A Pilot Study.

OBJECTIVE: Vocal tremor (VT) poses treatment challenges due to uncertain pathophysiology. VT is typically classified into two phenotypes: isolated vocal tremor (iVT) and essential tremor-related voice tremor (ETvt). The impact of phenotypes on upper aerodigestive tract physiology during swallowing remains unclear. Qualitative and quantitative measures were employed to characterize tremor phenotypes and investigate the effects on swallowing physiology. METHODS: Eleven ETvt participants (1 Male, 10 Female; x̄ age = 74) and 8 iVT participants (1 Male, 7 Female; x̄ age = 71) swallowed 20 mL bolus

Deglutition Syncope: A Case Report and Review of the Literature.

Deglutition syncope (DS) is a rare, neurally-mediated syncopal syndrome arising from an aberrant vagotonic reflex during swallow-associated esophageal dilation. Its association with gastroesophageal disorders often prompts gastroenterology consultation. An 89-year-old man with recent dysphagia and otalgia was admitted after a syncopal episode occurred while eating. Esophageal imaging and endoscopy demonstrated no causative abnormalities. Maxillofacial imaging revealed chronic sinusitis and mastoiditis. Telemetry monitoring demonstrated high-grade atrioventricular block and pause associated wit

Effect of posture on deglutitive biomechanics in healthy individuals.

The purpose of this study was to assess displacement of oropharyngeal structures, particularly the hyoid bone and velum, and variations in timing of the pharyngeal stage of swallowing in the upright versus the supine position. Twelve Caucasian adult subjects between 19 and 27 years of age participated. Subjects were recorded swallowing 7 cc of liquid barium in the upright and supine positions. The hyoid bone had a significantly greater amount of anterior displacement while in the supine position compared to that of the upright position (p < 0.01). While in the upright position, the velum comes

[Quantitative evaluation of deglutition in the upper esophagus by scintigraphy].

To evaluate the function of deglutition quantitatively, radionuclide transit in the and upper esophagus was examined. Ten ml of water containing 185 MBq 99mTc pertechnetate was put into the mouth and isotopic counts were measured every 0.2 second in the oral cavity, upper and lower cervical esophagus, and upper mediastinum. The subjects were studied twice, once while sitting and once supine. Esophageal transit was evaluated with time-activity curves obtained in each region of interest. In twelve healthy volunteers transit of the radionuclide was significantly delayed in supine position compare

Coordination of deglutitive glottic closure with oropharyngeal swallowing.

The goals of this study were to quantify the temporal relationship between swallow-induced glottic closure and (a) signals of swallow initiation, such as hyoid bone movement, tongue base movement, and mylohyoid electrical activity; (b) pharyngeal peristalsis; (c) laryngeal elevation; (d) vestibular closure; and (e) oropharyngeal barium bolus transit. Eight normal subjects (age 20-30 yr) were studied by concurrent transnasal video laryngoscopy, pharyngeal intraluminal manometry, and submental surface electromyography. The manometric, electromyographic, and both video recordings were synchronize

[Neural control of deglutition and esophageal motility in mammals].

Swallowing is a complex motor sequence involving the coordinated contraction of many muscles of the buccopharyngeal cavity, the larynx and the oesophagus. Most of the muscles are striated except those of the distal oesophagus which, in human and some other species, are of the smooth type. During swallowing, usually divided into a buccopharyngeal and an oesophageal stage (peristalsis), the sequential activity of the muscles results from motor orders programmed by a rhombencephalic swallowing centre and conveyed to the periphery by efferent fibres belonging to various pairs of cranial nerves (Vt

Deglutition syncope: a rare cause of reflex syncope.

We present a rare case of deglutition syncope in a woman in her 20s, who presented with a ten-year history of presyncope and visual disturbances triggered by eating.Investigations, including a resting 12-lead ECG and ambulatory Holter monitoring, revealed first-degree heart block and reproducible, high-grade atrio-ventricular (AV) block with ventricular asystole during eating, which was abolished by intravenous atropine, confirming a vagally mediated process. The diagnosis of deglutition syncope was established based on the strong temporal relationship with food intake and the observed cardioin

The tongue: deglutition, orofacial functions and craniofacial growth.

So-called "primary" or "infantile" forms of deglutition, also termed lingual dyspraxia, are treated in different ways by orthodontists using various appliances to correct the condition and are also managed by speech-therapists and physiotherapists. The results obtained are often unstable. We have developed a more holistic approach to this disorder by attempting to grasp the underlying mechanisms in order to achieve more satisfactory correction. By establishing normal salivary deglutition more rapidly, this manual osteopathic technique complements the methods which use voluntary rehabilitation

[Computerized manometry and deglutition after pharyngolaryngeal tumor resections].

33 patients were examined after laryngopharyngeal surgery by means of computer manometry using 4-channel-pressure probes. After local tumour resection in the region of tonsils and lateral oropharyngeal wall a slight cranial decrease in pressure results. Resections of the soft palate, glossotonsillar groove, base of the tongue, and of the vallecula lead to a cranial release of pressure reducing the driving force of the tongue. The swallowing action is therefore delayed and completely uncoordinated resulting in dyskinesia of the PE segment. Stenosis in the PE segment after hypopharyngeal resecti

Deglutition after near-fatal choking episode: radiologic evaluation.

Acute airway obstruction during oral intake is a relatively common event that may be fetal if not relieved immediately. Deglutition was studied in 75 individuals who had experienced a near-fatal choking episode (NFCE) or sudden inability to breathe during food intake that was promptly relieved by means of a Heimlich maneuver, suctioning, or intubation. Videofluoroscopy supplemented by static imaging revealed abnormal stages of deglutition in 58 individuals: oral, 32; pharyngeal, 19; pharyngoesophageal segment (PES), 28; and esophageal, 23. Forty individuals aspirated a liquid bolus; this was m

[Changes in the deglutition reflex during the perioperative period].

The swallowing reflex is one of the major mechanisms protecting the lungs against aspiration of foreign material. Its complex time course requires a perfect coordination with ventilation, as the aerodigestive crossroads are common for both functions. Physiological factors as ageing, as well as mechanical and pharmacological agents, may contribute to swallowing reflex alterations. Moreover, endotracheal intubation and most anaesthetic agents may impair the swallowing function and favour the occurrence of aspiration. This review discusses the normal swallowing process and its alterations by agen

Validation of the Deglutition Handicap Index-Informal Caregiver questionnaire.

BACKGROUND: The Deglutition Handicap Index-Informal Caregiver (DHI-IC) questionnaire is a dysphagia assessment tool derived from the psychometrically validated Deglutition Handicap Index (DHI) self-reported dysphagia questionnaire in French. Like the DHI, the DHI-IC comprises 30 items, in 3 equal domains: Physical, Functional and Emotional. MATERIALS AND METHODS: A prospective study included 61 patients and 61 caregivers recruited from an ENT clinic dedicated to dysphagia. OBJECTIVES: To validate the DHI-IC as an assessment tool and to compare results with the DHI and swallowing endoscopy.
