Early Allergen Introduction in Delaware: A Model to Address Healthcare Disparities and Population Healthcare Costs.

Food allergies introduce substantial clinical risks to patients and long-term economic burdens for families and health systems. In the past two decades, pediatric allergen introduction guidelines have come full circle, promoting the introduction of allergenic foods during infancy to support immune tolerance and reduce the likelihood of developing lifelong allergies. However, the translation of this clinical evidence into real-world policy and practice remains unevenly distributed; adoption barriers disproportionately affect families facing financial constraints, limited access to consistent gu

Consensus statement and clinical pathway for the management of colon cancer with peritoneal metastases in the state of Delaware.

BACKGROUND: Increasing data suggests that the combination of modern systemic therapies and Cytoreductive surgery with or without Hyperthermic Intraperitoneal Chemotherapy (HIPEC) may improve the outcome of patients with colon cancer with peritoneal metastases. Patient selection and sequence of treatments remains ill-defined. MATERIALS AND METHODS: A working group, the State of Delaware Peritoneal Surface Malignancies Task Force (DE-PSM-TF), was created including representatives from medical and surgical oncology from the acute care hospitals in Delaware. Leveraging Delaware's Public Health Resources to Mitigate Spread of Communicable Diseases in Congregate Settings.

While resources available to public health entities increased during the COVID-19 pandemic, the need to focus mitigation efforts on Delaware's most vulnerable citizens was evident. The higher risk of community-spread and complications resulting from COVID-19 associated with individuals residing in congregate settings such as homeless shelters and encampments prompted the Delaware Division of Public Health to leverage existing and new resources to provide technical assistance, education and to build valuable partnerships with community-based homeless service providers to reduce the public healt

Factors Associated with Antihypertensive Therapy Prescription in Youth Delaware Medicaid Recipients with Primary Hypertension Diagnosis.

BACKGROUND: Higher neighborhood deprivation is associated with hypertension diagnosis in youth. In this study, we assess if there is an association between neighborhood deprivation and antihypertensive therapy prescription among insured youth with a primary hypertension diagnosis. METHODS: Using a retrospective cross-sectional design, we assessed the proportion of youth with a diagnosis of primary hypertension prescribed antihypertensive therapy. We evaluated the proportion of youth prescribed antihypertensive therapy and compared prescribing patterns by area deprivation index (ADI), age, sex

Health & Housing in Delaware:: Matching Medicaid Claims and Encounters and the Community Management Information System Databases.

This study draws upon data from two databases: claims and encounters that were reimbursed by the state's Division of Medicaid and Medical Assistance (DMMA) and the Homeless Management Information System (HMIS) database that collects homeless services data on individuals experiencing homelessness. Records from both sets are matched to identify 838 adults who both experienced homelessness and were Medicaid eligible in 2019, and to select, through propensity score matching, an equal set of control observations who were similarly Medicaid-eligible but had no record of homelessness. Outcomes are co

Are you listening? Parental protective factors and early onset drinking among students with disabilities in Delaware.

BACKGROUND: Little is known about substance use disorder risk among youth with disabilities. The Delaware YRBS collects data on substance use and disability indicators. Data suggest that students with disabilities have a younger average age of onset for alcohol use, and therefore may be more at risk for developing an alcohol use disorder later in life. OBJECTIVE: To explore the relationship between parental listening and the age of substance use initiation among students with disabilities. METHODS: Using the 2017 Delaware High School YRBS, we conducted a logistic regression analysis to test

Challenge of a dual burden in rapidly aging Delaware: Comorbid chronic conditions and subjective cognitive decline.

BACKGROUND: Epidemiologic trends forecast a "dual burden"- increase in both physical chronic diseases and Alzheimer's disease (AD)- for Delaware. Estimating the burden and characteristics of this "dual burden" is critical. Cognizant of the unavailability of precise models to measure AD, SCD-a population-based measure- was used as an alternative. The primary objective was to delineate selected chronic conditions among Delaware adults with SCD in order to present: (i) prevalence of SCD by select sociodemographic characteristics, (ii) compare the prevalence of chronic conditions among people with

Opioid-stimulant trends in overdose toxicology by race, ethnicity, & gender: An analysis in Delaware, 2013-2019.

Recent upticks of stimulant presence in overdose deaths suggest the opioid epidemic is morphing, which raises questions about what drugs are involved and who is impacted. We investigate annual and growth rate trends in combined opioid-stimulant overdose toxicology between 2013 and 2019 for White, Black, and Hispanic male and female decedents in Delaware. During these years, toxicology shifted to illegal drugs for all with fentanyl leading the increase and opioid-cocaine combinations rising substantially. While combined opioid-cocaine toxicology grew among Black and Hispanic Delawareans, White

The impact of the baby friendly hospital initiative on healthcare utilization among newborns insured by Medicaid in Delaware.

BACKGROUND: The Baby Friendly Hospital Initiative was created to enhance breastfeeding, although its impact on infant healthcare utilization is unclear. Breast feeding infants are vulnerable to readmission soon after birth secondary to dehydration and hyperbilirubinemia. Breastfeeding can also protect infants from unnecessary health care utilization later in life by preventing infection. The objective of this study was to examine the impact of the Baby Friendly Hospital Initiative on readmissions and emergency department utilization among Medicaid births in Delaware. METHODS: The study was a

Is the Gender Gap in Overdose Deaths (Still) Decreasing? An Examination of Opioid Deaths in Delaware, 2013-2017.

OBJECTIVE: Drug overdoses among men have historically outnumbered those among women by a large margin. Yet, U.S. research on the first wave of the opioid epidemic involving prescription opioids has found women to be at increased risk. The current study considers if the narrowing gender gap in overdose deaths, as observed during the first wave, has continued into the most recent third wave, dominated by synthetic opioid deaths. This requires consideration of interactions between gender, age, and type of drug implicated. METHOD: Drawing on 2013-2017 Delaware toxicology reports for a total of 89

Epidemiology of West Nile virus: a silent epiornitic in Northern Delaware in 2007 without associated human cases.

ABSTRACT. We performed a 2-year longitudinal study (2006-2007) of West Nile virus (WNV) infections in wild birds, mosquitoes, and sentinel chickens at 6 WNV-endemic sites in northern Delaware. We determined virus infection rates of Culex pipiens and other mosquito vectors as well as seroprevalence and antibody titers of amplifying hosts. Endemicity status varied widely among the 6 sites based on 3 criteria-mosquito infections, sentinel chicken seropositivity, and wild bird seropositivity. A highly endemic site would display at least 2 of the 3 criteria during each year of the study, while a si

Preventive Behaviors during the COVID-19 Pandemic in Delaware's Underserved Communities: A Longitudinal Study (2021-2022).

The COVID-19 pandemic officially started in March 2020 and ended in May 2023. Preventive behaviors have been proven to be one of the most effective strategies for preventing COVID-19 transmission. Common preventive behaviors against COVID-19 include but are not limited to wearing a face mask, washing and sanitizing one's hands frequently, avoiding crowds, and avoiding traveling. Underserved communities and racial/ethnic minorities across the U.S. have been disproportionately affected by the COVID-19 pandemic. Using a three-wave longitudinal survey conducted from early 2021 to late 2022, the pr

Statewide Retrospective Review of Familial Pancreatic Cancer in Delaware, and Frequency of Genetic Mutations in Pancreatic Cancer Kindreds.

BACKGROUND: Considering the typical rapid progression and high mortality of pancreatic cancer (PC), early detection may lead to an improved outcome. To date, there is no safe, sensitive, and cost-effective screening strategy to detect PC. Currently, screening is focused on individuals at the highest risk of developing PC based on family history. A high-risk individual is defined as having two or more first-degree relatives with PC, or one first- or second-degree relative with PC with a confirmed mutation in a gene associated with PC. The BRCA2 gene is one of the most common genes linked to pan

The impact of a statewide training to increase child care providers' knowledge of nutrition and physical activity rules in Delaware.

BACKGROUND: Childhood obesity has been recognized as a national problem of epidemic proportions. Child care represents an ideal venue in which to address this problem, as many young children spend a significant amount of time and consume the majority of their meals in this setting. Recognizing this opportunity, Delaware recently enacted reforms to statewide licensing regulations designed to improve the quality of the nutrition-, physical activity-, and screen viewing-related environments in child care settings. METHODS: To facilitate the translation of these regulations into practices, a seri

Effects of contaminant exposure on reproductive success of ospreys (Pandion haliaetus) nesting in Delaware River and Bay, USA.

Despite serious water-quality problems and pollutant loading and retention, Delaware River and Bay (USA) provide important wildlife habitat. In 2002, we conducted a comprehensive evaluation of contaminant exposure and reproduction of ospreys (Pandion haliaetus) breeding in Delaware River and Bay. Sample eggs were collected from 39 nests and analyzed for organochlorine pesticides, polychlorinated biphenyls (PCBs), and mercury; a subset of 15 eggs was analyzed for perfluorinated compounds and polybrominated diphenyl ethers (PBDEs). The fate of each nest was monitored weekly. Concentrations of 10

Desire for Sterilization Reversal in Delaware, US: The Roles of LARC Use, Personal Abortion Acceptability, and Economic Disadvantage.

OBJECTIVE: To investigate the pathways that may elevate risks of desire for sterilization reversal, specifically through nonuse of long-acting reversible contraception (LARC), viewing abortion as unacceptable for oneself, and being economically disadvantaged. METHODS: We used chi-squared tests and binary logistic models to analyze data on 3422 women, including 299 sterilized women, from a population-representative sample of Delaware women aged 18-44 in 2021. We estimated models for female sterilization use and for desire for sterilization reversal. RESULTS: Among sterilized Delaware women ag
