[Kangaroo method in delivery room for full-term babies].

OBJECTIVE: Our objective was to determine the tolerance to kangaroo care in the delivery room. PATIENTS AND METHODS: We have studied 651 normal full term newborn infants delivered in our hospital during an 8 month period. Immediately after birth, the newborn infants were dried and placed in skin-to-skin contact between their mother's breasts. They remained in kangaroo care in the delivery room, during transportation to the post-partum area and in their room as long as two hours. The kangaroo care duration, the infant's post-kangaroo care axillary temperature, the mother's and infant's attitud

Quality improvement for neonatal resuscitation and delivery room care.

Quality improvement has become a foundation of neonatal care. Structured approaches to improvement can standardize practices, improve teamwork, engage families, and improve outcomes. The delivery room presents a unique environment for quality improvement; optimal delivery room care requires advanced preparation, adequately trained providers, and carefully coordinated team dynamics. In this article, we examine quality improvement for neonatal resuscitation. We review the published literature, focusing on reports targeting admission hypothermia, delayed cord clamping, and initial respiratory sup

Efficacy of occlusive wraps used for delivery room care.

BACKGROUND: Guidelines advise for more than 20 years to use occlusive plastic wraps for temperature management during delivery room care but data on efficacy of different types of wrap are still scarce. METHODS: A random sample of seven different types of plastic wrap was tested using prewarmed aluminium blocks. RESULTS: The most effective wrap increased the time to cool by 2°C by one-third for the core and by 100% for the surface whereas the least effective wrap led to even faster heat loss compared with no wrap at all. The least effective wrap concerning all capacities tested was made from

Cardiopulmonary resuscitation in newborn infants with ultrasound in the delivery room.

Point of care ultrasound has emerged as useful tool in cardiac arrest situations in adult population. Despite of these, there is not a protocol for cardiopulmonary resuscitation in the delivery room for newborns. We describe two case were ultrasound helped to rule out the cardiopulmonary arrest case and we propose and algorithm were ultrasound is integrated in the newborn resuscitation and may help to diagnose the cardiac arrest cause.

Noise as a risk factor in the delivery room: A clinical study.

INTRODUCTION: We aimed to investigate whether noise in delivery rooms is associated with impaired performance of obstetric teams managing major (≥1000 mL) postpartum hemorrhage. MATERIAL AND METHODS: We included video recordings of 96 obstetric teams managing real-life major postpartum hemorrhage. Exposure was noise defined as the occurrence of sound level pressures (SPL) above 90 dB. The outcome was high clinical performance assessed through expert ratings using the TeamOBS-PPH tool. RESULTS: The 23 teams unexposed to noise had a significantly higher chance of high clinical performance than

Reducing Brain Injury of Preterm Infants in the Delivery Room.

Cerebrovascular injury is one of the major detrimental consequences of preterm birth. Recent studies have focused their attention on factors that contribute to the development of brain lesions immediately after birth. Among those factors, hypothermia and lower cerebral oxygen saturation during delivery room resuscitation and high tidal volumes delivered during respiratory support are associated with increased risk of severe neurologic injury. In preterm infants, knowledge about causes and prevention of brain injury must be applied before and at birth. Preventive and therapeutic approaches, inc

Optimizing Care of the Preterm Infant Starting in the Delivery Room.

Proficiency in the care of the preterm neonate is paramount to ensuring safe and quality outcomes. Here we review several simple interventions combined with supportive and informative monitoring that assists the care team in facilitating this critical transitional phase of the care of the preterm newborn. We will discuss the use of checklists, avoidance of early cord clamping, resuscitation during delayed cord clamping, early administration of caffeine soon after birth, and the use of additional monitoring (electrocardiogram, carbon dioxide and respiratory function) during resuscitation. This

[Delivery room management: What's new in 2010 recommendations?].

For apneic or bradycardic babies born at term, it is best to begin ressuscitation in the delivery room with air rather than 100% oxygen. Administration of supplementary oxygen should be regulated by blending oxygen and air, and the concentration delivered should be guided by oximetry. Preterm babies less than 32 weeks gestation may not reach the same arterial blood oxygen saturations in air as those achieved by term babies. Therefore, blended oxygen and air should be given guided by pulse oximetry. Detection of exhaled carbon dioxide in addition to clinical assessment is recommended as the mos

Is gastric aspiration needed for newborn management in delivery room?

AIM: Gastric aspiration is still applied in many centres during delivery room management of the newborn without any supporting evidence. We aimed to determine whether gastric aspiration affects vital signs, oxygenation, nutrition and short-term prognosis of the newborn. METHOD: A total of 310 eligible healthy term newborns, identified from a total of 1300 live births, were randomly allocated to receive either gastric aspiration or standard care. During the first 20min, SpO(2), heart rate, cyanosis and retraction scores were recorded once in a min; and blood pressure, respiration rate and neur

Heat loss prevention for preterm infants in the delivery room.

OBJECTIVE: Preterm infants are prone to hypothermia immediately following birth. Among other factors, excessive evaporative heat loss and the relatively cool ambient temperature of the delivery room may be important contributors. Most infants <29 weeks gestation had temperatures <36.4 degrees C on admission to our neonatal unit (NICU). Therefore we conducted a randomized, controlled trial to evaluate the effect of placing these infants in polyurethane bags in the delivery room to prevent heat loss and reduce the occurrence of hypothermia on admission to the NICU. METHODS: After parental conse

Delivery room risk factors for meconium aspiration syndrome.

The objective of this study is to identify risk factors for meconium aspiration syndrome (MAS) in newborns born through meconium-stained amniotic fluid (MSAF). From May 27, 1994 to June 9, 1997 maternal and neonatal data were prospectively collected on all infants born through MSAF. Development of MAS was the primary outcome. Using bivariate and logistic regression analysis we identified risk factors for MAS. There were 8,967 births during this period: 7.9% (708 of 8,967) were delivered through MSAF. Respiratory symptoms developed in 6.8% (48 of 708) of births. Of these, 50% (24 of 48) were ex

Pediatric coverage of the delivery room: an analysis of manpower utilization.

OBJECTIVES: To determine the frequency and pattern of pediatric calls to the delivery room and the actual type of medical care administered to the newborn in the delivery room. STUDY DESIGN: This was a prospective observational study of 2554 births in a university-affiliated tertiary care hospital. Existing protocols required the attendance of a pediatric resident or neonatal fellow at all deliveries other than uncomplicated vaginal term births. The pediatrician's activity in the delivery room was characterized as either "medical care" or "minimal care." Results were analyzed by diagnostic ca

Labor room setting compared with the operating room for simulated perimortem cesarean delivery: a randomized controlled trial.

OBJECTIVE: To compare the labor room and operating room for perimortem cesarean delivery during simulated maternal arrests occurring outside the operating room. We hypothesized transport to the operating room for perimortem cesarean delivery would delay incision and other important resuscitation milestones. METHODS: We randomized 15 teams composed of obstetricians, nurses, anesthesiologists, and neonatal staff to perform perimortem cesarean delivery in the labor room or operating room. A manikin with an abdominal model overlay was used for simulated cesarean delivery. The scenario began in th

Impact of Simulation-Based Training on Maternal and Neonatal Outcomes in Delivery Rooms: A Systematic Review.

This systematic review examines the impact of simulation-based training (SBT) in delivery rooms on maternal and neonatal outcomes in high-resource settings. Given the persistently high rates of maternal and neonatal mortality worldwide, SBT has emerged as a promising method to enhance clinical performance, decision-making, and teamwork in obstetric emergencies. Eight studies published between 2014 and 2023 were included, covering over 177,000 deliveries. The composite analysis showed a significant reduction in adverse outcomes, including postpartum hemorrhage requiring transfusion and neonatal

[Delivery in free position perhaps a solution to change delivery in traditional position in delivery rooms in Africa: results of a prospective study in Cotonou in Bénin].

In most of Africa's birthing units, women are often forbidden to stand during labour and delivery. This study aimed to determine the effect of a freely vertical position (standing, sitting, crouching) compared with the traditional supine position on the course of delivery and on perinatal and maternal outcomes(including the satisfaction level of mothers). Methods: We compared a group of 490 women who chose to deliver in a freely vertical position with 490 women who delivered in a traditional position, at the Hopital de Ia mere et de l'enfant Lagune of Cotonou, from January 1, 2009, to December 3

Contemporary challenges and strategies in delivery room training for pediatric residents.

As pediatric and neonatal hospitalists assume increasing care for newborns in the delivery room and during the birth hospitalization, preparing pediatric trainees with foundational skills in neonatal resuscitation is increasingly critical. However, residency programs may face challenges in providing sufficient delivery room exposure and procedural opportunities given newer training requirements and updated care practices that reduce procedural interventions during neonatal resuscitation. In this Perspectives article, we examine factors contributing to these gaps, describe our institution's hos
