Effects of atropine on learning and memory functions in dementia.

The effect of intravenous atropine (1 mg) or saline on mnemonic function was tested in patients with various forms of dementia and age-matched controls. Examinees consisted of 12 subjects having Alzheimer's disease (AD), multi-infarct dementia (MID), Parkinson's disease without dementia or with dementia, and normal aged controls. Wechsler's adult memory scale was used. Orientation, visual reproduction, and associative learning were not affected by this small dose of atropine in any of the subgroups. On the other hand, digit span and logical memory were selectively impaired by atropine in AD. T

[Modern concepts of "cerebrovascular dementia"].

The incidence of both atherosclerosis and demential increases with age and therefore the terms "cerebral atherosclerosis" or "cerebro-vascular dementia" are commonly used for any mental deterioration in elderly persons. These names depend on the proposition of a gradual narrowing of cerebral arteries as an inevitable accompaniement of ageing which ends in dementia through a progressive reduction of cerebral blood flow. This apparently reasonnable hypothesis has now been shown to be wrong. ;t has been established that first, senile dementia is not due to cerebral atherosclerosis in spite of the

From Binswanger's disease to leuokoaraiosis: what we have learned about subcortical vascular dementia.

The literature regarding subcortical vascular dementia associated with periventricular and deep white matter alterations is reviewed. Information pertaining to neuropathological, neuropsychological, and neuroradiological studies is emphasized. Based on this review and prior neuropsychological studies associating subcortical vascular pathology with greater deficits on tests of executive dysfunction and with relatively better performance on tests of delayed recognition memory, we conclude that vascular dementia associated with periventricular and deep white matter alterations can and should be r

Cytogenetic changes in patients with senile dementia.

Chromosome analyses were performed on lymphocytes from patients with Alzheimer type dementia, multi-infarct dementia, Down's syndrome and healthy controls. At least 100 cells were scored from each individual. A significant increase in aneuploidy was observed in the demented patients compared to controls but there was no difference between multi-infarct dementia and senile dementia of Alzheimer type. Nor was there any sex difference in the different groups. Also there was a significantly increased frequency of structurally altered chromosomes among the patients with senile dementia of Alzheimer

Prevalence of clinically diagnosed Alzheimer's disease and other dementing disorders: a door-to-door survey in Appignano, Macerata Province, Italy.

The purpose of this study was to investigate the prevalence of dementia in an Italian population using a door-to-door 2-phase design. As part of a social and health survey, we administered the Hodkinson abbreviated mental test to all persons over age 59 residing in the Commune of Appignano on January 1, 1987 (N = 778). We then investigated all subjects scoring 7 or less on the cognitive test following a standardized diagnostic protocol. We found 48 patients affected by dementia, yielding a crude prevalence ratio (cases per 100 population over age 59) of 6.2; prevalence ratios were 2.6 for Alzh

[Current concepts in vascular dementia].

Vascular dementias, VD, are dementias due to cerebrovascular lesions. Subgroups of VD include multi-infarct dementia, single infarct (or strategic infarct) dementia, subcortical ischemic vascular dementia, hemorrhagic dementia, hypoperfusion dementia. VD are also related to post-stroke dementia, mixed Alzheimer's disease and vascular dementia and vascular cognitive impairment. These various entities allow to characterize more homogenous subgroups within the heterogeneous group of vascular dementias. However, ambiguities in their definitions, associated with frequent overlaps as well as lack of

[Cerebral infarction: lacunae].

Lacunae are small deep cerebral infarcts which are believed to occur in hypertensive patients, producing characteristic clinical syndromes. Previous reports suggested to differentiate this type of stroke from other cerebrovascular diseases, but failed to provide any evidence that this classification is useful in clinical practice. We reviewed the present literature concerning pathogenesis, clinical features, diagnostic, and therapeutic aspects of lacunae and we established that current concepts in lacunar stroke are inadequately supported. Although lacunar syndromes are reported to be correlat

Motor apraxia in dementia.

Motor apraxia was assessed in 25 patients with presumed dementia of the Alzheimer type and 23 patients with presumed multi-infarct dementia. Apraxia was common in both groups and was usually only mild. It correlated most strongly with language-related impairments in the Alzheimer group, as has been found in other patient groups, whereas in the group with multi-infarct dementia the pattern of correlations was less clear. It was not strongly related to performance on tests involving constructional praxis or to age in either group. Implications of the findings for clinical assessment are noted.

Some observations on the spectrum of dementia.

A study was designed to generate epidemiological and clinical data on dementia, in a teaching hospital in India. It was conducted on 124 (94 male and 30 female) elderly patients (aged more than 60 years) presenting with clinical syndrome of dementia (DSM-3). Their age range was 64-78 (mean 65.7 4.1) years. Detailed clinical, biochemical, radiological and electrophysiological evaluation was done to establish etiology. Patients with psychiatric ailments, cranial trauma and tumors were excluded. The study period was 4.2 years. Multi-infarct dementia (MID) was observed to be commonest cause of dem

["Cerebro-active" drugs].

The prevention and treatment of age related mental diseases, including cerebrovascular dementia and Alzheimer's disease, are complicated by diagnostic problems and by the lack of specificity of the so-called "cerebroactive" drugs. These agents are frequently prescribed at the patient's or the relatives' request, without clear-cut evidence for their therapeutic efficacy. The economic consequences of these usually long term treatments should also be taken into consideration. A few "cerebroactive" agents may marginally slow down or modulate the progression of the various neuropathological process

[Cadasil--a new model for subcortical dementia].

We report 3 cases of Cadasil with dementia. In the 3 cases, the dementia had a subcortical and frontal presentation. It associated behavioural symptoms, amnesia, executive functions disturbances, bradyphrenia, slowing of information processing and frontal symptoms, without aphasia, apraxia or agnosia. One patient showed overt dementia before any focal neurological sign. Imaging of the brain was consistent with subcortical infarcts and leukoencephalopathy, without involvement of the cerebral cortex. We suggest that dementia and psychoaffective disturbances are major diagnostic criteria for Cada

Making the diagnosis of mixed and non-Alzheimer's dementias.

Community pathologists are often called on to perform autopsies to confirm clinical diagnoses of Alzheimer's disease, by far the most common cause of dementia. Diagnostic criteria have been provided by the Consortium to Establish a Registry for Alzheimer's Disease. Beyond pure and simple Alzheimer's disease, a significant proportion of dementia brains will feature Alzheimer's disease mixed with Lewy bodies, historically associated with idiopathic Parkinson's disease, or combined with various manifestations of cerebrovascular disease. Less commonly, the pathologist will encounter Lewy body dise

Somatostatin cerebrospinal fluid levels in dementia.

Somatostatin levels were measured in cerebrospinal fluid of patients with Alzheimer's disease, multi-infarct dementia and normal pressure hydrocephalus and compared with levels from a normal control group. All pathological groups showed a statistically significant decrease of somatostatin with respect to the control group, but no significant differences were found amongst them. A negative correlation was found between the Mini Mental State Test and the somatostatin levels in Alzheimer's disease patients but not in the other groups. Our results confirm that the lower levels of somatostatin in c

[Sonography of the cervical vessels. Value in the differential diagnosis of dementia syndromes].

Some dementias in the elderly are due to vascular diseases in particular arteriosclerosis of cerebral arteries. The question whether US examination of the extracranial cerebral supply vessels is a reliable means of recognizing this form of dementia has been investigated in patients with the clinical diagnosis of vascular dementia. Using direct/indirect Doppler and real-time techniques, we were able to detect pathological lesions in the extracranial supply vessels in only one-third of these patients. Thus, US examination of the vessels in the neck would appear to be of only limited differential

The pontomesencephalotegmental cholinergic system does not degenerate in Alzheimer's disease.

The pedunculopontine and laterodorsal tegmental nuclei comprising the pontomesencephalotegmental (PMT) cholinergic system were examined for the presence of neuropathology and loss of presumed cholinergic cells in patients diagnosed with Alzheimer's disease-senile dementia of the Alzheimer type (AD-SDAT), Parkinsonian dementia, and multi-infarct dementia. Although neurofibrillary changes and plaque-like entities were observed in both nuclei in AD-SDAT and Parkinson's disease dementia, these pathologic indices were not seen consistently in control individuals or in those with multi-infarct demen

On diagnosis of dementia: psychometric investigation and clinical psychiatric evaluation in relation to verified diagnosis.

Patients with early as well as manifest dementia were studied in a longitudinal way with psychometric and psychiatric methods. Tests such as vocabulary, attention test, block design test, verbal and spatial memory tests, reaction time test and examination for aphasia were used as well as a qualitative analysis of behaviour and personality. The differential diagnosis of Alzheimer's disease, Pick's disease and other dementias with fronto-temporal degeneration and multi-infarct dementia was based on diagnostic rating scales. Differences in cognitive profiles, qualities of behaviour and scores on

Genetics, ageing and dementia.

The contribution of genetic differences to variation in ageing and the relationship of ageing to certain types of dementia are discussed. Neuropathological changes commonly found in the ageing brain are present in more severe form in Alzheimer-type dementia, Down's syndrome, multi-infarct dementia, and a substantial number of patients with Parkinson's disease. An increased frequency of ageing-associated changes outside the brain have been reported in Alzheimer-type dementia, Down's syndrome, and multi-infarct dementia, although the evidence is generally meagre and in many cases requires furthe

CSF somatostatin-like immunoreactivity in dementia.

Concentrations of somatostatin-like immunoreactivity (SLI) in CSF were reduced in Alzheimer's disease (AD) and multi-infarct dementia (p less than 0.01), but not in normal-pressure hydrocephalus, Parkinson's disease, and Huntington's disease. This suggests that reduced SLI content in AD cerebral cortex is reflected in CSF. Chromatographic characterization of CSF SLI showed no differences between AD and controls. Concentrations of SLI in AD patients overlapped those in both normal subjects and patients with multi-infarct dementia, so that changes in CSF SLI have no diagnostic specificity.
