The association of prefrontal cortex response during a natural reward cue-reactivity paradigm, anhedonia, and demoralization in persons maintained on methadone.

Persons with opioid use disorder (OUD) often experience anhedonia and demoralization, yet there is relatively little research on the pathophysiology of anhedonia and demoralization in OUD treatment and recovery. In the current study, persons maintained on methadone (N = 29) underwent a natural reward-cue paradigm during functional near-infrared spectroscopy (fNIRS) imaging. Natural reward cues included highly palatable food, positive social interactions (e.g., a happy family at the dinner table), and emotional intimacy (e.g. couples embracing or kissing, but no erotic images). Participants als

Meaning-Centered Psychotherapy for Existential Distress and Demoralization in Advanced Cancer: A Narrative Review of Clinical Evidence and Implications for End-of-Life Care.

Existential distress and demoralization are common among patients with advanced cancer and are associated with impaired psychological well-being, reduced quality of life and difficulties in end-of-life adjustment. Meaning-Centered Psychotherapy (MCP), grounded in Viktor Frankl's logotherapy, was developed to address these existential dimensions of suffering. This narrative review synthesizes evidence from randomized and controlled studies evaluating MCP in adults with advanced cancer and discusses its clinical relevance within supportive and palliative care. Across studies, MCP demonstrated co

Latent class analysis differentiation of adjustment disorder and demoralization, more severe depressive and anxiety disorders, and somatic symptoms in patients with cancer.

OBJECTIVES: Demoralization as a form of existential distress involves poor coping, low morale, hopelessness, helplessness, and meaninglessness. In a secondary analysis of a cohort of German cancer patients, we aimed to explore latent class structure to assess the contribution that symptoms of demoralization make to anhedonic depression, anxiety, adjustment, and somatic disorders. METHODS: Measures of demoralization, depression, anxiety, physical symptoms, and functional impairment had been completed cross-sectionally by 1527 patients with early or advanced cancer. Latent class analysis used m

Frontal Lobe Functions, Demoralization, Depression and Craving as Prognostic Factors and Positive Outcomes of Patients with Heroin Use Disorder Receiving 6 Months of Methadone Maintenance Treatment.

Methadone maintenance therapy (MMT) is a well-established and effective treatment for heroin use disorders. Whether frontal lobe function and demoralization serve as suitable prognostic and outcome assessment factors remains unknown. A quasi-experimental study was conducted with a single-group repeated-measures design at a medical center and mental hospital in Taiwan. We enrolled 70 participants (39 completed treatments and 31 dropped out). Frontal lobe function, demoralization, depression, and craving at three time points were analyzed. There were differences between patients who completed th

Meaning and Psychedelics in Palliative Care: A Narrative Review.

CONTEXT: Meaning is a primary existential concern in those with advanced illnesses and functions as an important coping mechanism. Loss of meaning contributes to existential distress, and, in particular, may manifest as demoralization, a syndrome of poor coping that is associated with negative outcomes. Psychedelics are unique psychoactive compounds that, among other properties, are proposed to enhance meaning. In the palliative setting, psychedelic therapies are under investigation for existential distress, including demoralization. OBJECTIVE: To synthesize the literature on meaning in palli

An empirically derived taxonomy of common distress syndromes in the medically ill.

OBJECTIVE: Contemporary psychiatric classifications have not proved to be useful in the understanding and care of people with physical illness. Distress syndromes are common, but classifications fail to differentiate syndromes relevantly. We sought to take a fresh look at the common distress syndromes in the medically ill. METHODS: 312 medical inpatients were interviewed using a structured psychiatric interview [the Monash Interview for Liaison Psychiatry (MILP)] to elicit the presence of mood, anxiety and somatoform symptoms. A previously reported examination of these data using latent trait

Euthymia and fatigue in patients with psoriasis: Insights from a self-determination theory perspective.

Psoriasis affects both physical and psychological health. This study explored how demoralization and the satisfaction or frustration of basic psychological needs (autonomy, competence, and relatedness), and demoralization are related to euthymia and fatigue in psoriasis patients, and how these psychological factors are linked to disease severity. This cross-sectional study of 215 psoriasis patients assessed psychological variables using the Euthymia Scale, Basic Psychological Need Satisfaction and Frustration Scale, Demoralization Scale-II, FACIT-F, Patient Health Questionnaire-4, and Physicia

Prenatal exposure to air pollution, maternal psychological distress, and child behavior.

BACKGROUND: Airborne polycyclic aromatic hydrocarbons (PAHs) are pollutants generated by combustion of fossil fuel and other organic material. Both prenatal PAH exposure and maternal psychological distress during pregnancy have each been associated with neurodevelopmental problems in children. The goal was to evaluate potential interactions between prenatal exposure to airborne PAHs and maternal psychological distress during pregnancy on subsequent behavioral problems in children. METHODS: In a longitudinal birth cohort study, 248 children of nonsmoking white women in the coal-burning region

On the hierarchical structure of mood and anxiety disorders: confirmatory evidence and elaboration of a model of temperament markers.

The authors examined D. Watson's (2005) proposed reconceptualization of the diagnostic categories for mood and anxiety disorders for the Diagnostic and Statistical Manual of Mental Disorders--Fifth Edition (DSM-V) and tested an elaboration of the 2-factor (positive and negative activation) model of underlying temperament markers that incorporates A. Tellegen, D. Watson, & L. A. Clark's (1999a, 1999b) higher-order dimension of happiness-unhappiness (or demoralization; see A. Tellegen et al., 2003). In Study 1, 502 undergraduate students completed several symptom measures of mood and anxiety dis

Addressing Existential Distress Through Compassion: Feasibility and Preliminary Effects of Compassion With You (COMwithU), a Compassion-Based Intervention for Patients With Cancer.

BACKGROUND: Cancer-related distress extends beyond anxiety and depression to existential suffering, including loss of meaning, helplessness, uncertainty, and relational strain. Compassion-based interventions in psycho-oncology are promising, but existential outcomes such as demoralization and spiritual well-being have rarely been examined. Compassion With You (COMwithU) is a brief, five-session compassion-based intervention that centers on cultivating self-compassion, supported by the therapist's compassionate presence and by practices of extending compassion toward others. METHODS: This sing

An innovative approach to the assessment of mood disturbances in patients with acute coronary syndrome.

BACKGROUND: The clinical value of the identification of mood disorders in patients with acute coronary syndrome (ACS) is well established. However, assessment based on DSM criteria presents some limitations. This study aimed to provide an innovative strategy for evaluating the spectrum of mood disturbances in ACS. METHODS: A total of 288 patients with a first episode of ACS underwent interviews based on DSM-IV-TR criteria (major depressive disorder, minor depression, and dysthymia), Diagnostic Criteria for Psychosomatic Research-DCPR (demoralization and type A behavior), and the Clinical Inte

Factors associated with PTSD symptoms and quality of life among nurses during the COVID-19 pandemic: A cross-sectional study.

BACKGROUND: Healthcare workers, especially nurses, were one of the most vulnerable groups for developing posttraumatic stress disorder (PTSD) during the coronavirus disease 2019 (COVID-19) pandemic, which also affected their quality of life. However, only limited research has investigated the individual psychological factors as well as the environmental factors responsible for these effects of the pandemic. Demoralization is a state of loss of meaning and anhedonia, which we thought to be an important mediator between fear and PTSD among frontline nurses during the pandemic. This study aimed t
