Impact of amoxicillin prophylaxis on the incidence, nature, and duration of bacteremia in children after intubation and dental procedures.

BACKGROUND: Controversy exists about the impact of prophylactic antibiotics on bacteremia after invasive dental procedures. The purpose of this double-blind, randomized, placebo-controlled study was to determine the impact of amoxicillin prophylaxis on the incidence, nature, and duration of bacteremia from nasotracheal intubation and dental procedures in children. METHODS AND RESULTS: Children were randomly assigned before surgery to the American Heart Association (AHA)-recommended dose of amoxicillin or to a placebo. Aerobic and anaerobic blood cultures were drawn at 8 specific time points a

[A 2-year evaluation of different methods of dental caries prophylaxis in a sample school-aged population].

Caries is currently considered to be the most widespread disease affecting mankind; it is calculated in fact that around 90% of the world population suffers from caries. Carious pathology therefore plays a key role, not least due to the high social costs which it entails, and, for this reason, research into appropriate methods of prevention which can to some extent limit the incidence of caries are of particular interest. MATERIALS AND METHODS. The study was performed in a group of 1475 selected primary-school children. After having subdivided the population in three groups, an initial dental

Dental procedures and endocarditis prophylaxis in patients with prosthetic heart valves: results of a questionnaire to 220 patients.

220 patients with prosthetic heart valves were surveyed regarding dental status, dental health, patient-dentist relationship, information on dental procedures and antimicrobial prophylaxis at the last visit to the dentist. The vast majority of these patients were on life-long anticoagulant treatment because of mechanical heart valve prostheses. Half of the patients had a regular dentist and customarily visited a dentist one or more times a year. One third of the patients were edentulous and 26.5% of the patients indicated dental disease, including chronic periodontitis. Only 30.4% of the patie

Inappropriate antibiotic prophylaxis before dental procedures in congenital heart disease: Insights from a nationwide Korean cohort.

BACKGROUND: The 2007 American Heart Association (AHA) guidelines strictly restrict antibiotic prophylaxis (AP) before dental procedures to patients at the highest risk of infective endocarditis (IE). However, national adherence to these guidelines in Korea, a country with high outpatient antibiotic use, remains unevaluated. METHODS: This population-based, retrospective cohort study using National Health Insurance Service System claims (2002-2022) included patients with atrial septal defect, ventricular septal defect, tetralogy of Fallot, or prosthetic cardiac valves. Dental claims within 14 d

Cost-effectiveness of dental antibiotic prophylaxis in total knee arthroplasty recipients with type II diabetes mellitus.

OBJECTIVE: Type II diabetes mellitus (T2DM) is prevalent in knee osteoarthritis (OA) patients undergoing total knee arthroplasty (TKA) and increases risk for prosthetic joint infection (PJI). We examined the cost-effectiveness of antibiotic prophylaxis (AP) before dental procedures to reduce PJI in TKA recipients with T2DM. DESIGN: We used the Osteoarthritis Policy Model, a validated computer simulation of knee OA, to compare two strategies among TKA recipients with T2DM (mean age 68 years, mean BMI 35.4 kg/m2): 1) AP before dental procedures and 2) no AP. Outcomes included quality-adjusted l

Rothia mucilaginosa Sepsis in a Kidney Transplant Recipient Following Dental Procedure Despite Clindamycine Prophylaxis: A Case Report.

Rothia mucilaginosa is an opportunistic Gram-positive coccus that colonizes the oral cavity. However, in immunocompromised patients, it may cause severe invasive infections. We report a case of R. mucilaginosa sepsis in a kidney transplant recipient following a dental procedure despite prophylactic clindamycin therapy. A 64-year-old female kidney transplant recipient receiving maintenance immunosuppression with tacrolimus, mycophenolate mofetil, and prednisone underwent dental treatment because of profound dental caries with gingival inflammation. Clindamycin was administered because of histor

The use of liquid formulation pentoxifylline and vitamin E in both established and as a prophylaxis for dental extractions "at risk" of osteoradionecrosis.

BACKGROUND: Osteoradionecrosis (ORN) of the jaws remains one of the most debilitating complications of radiotherapy (RT) in patients with head and neck cancer (HNC). Liquid pentoxifylline and vitamin E (PVe) presents an alternative formulation to tablets for patients with dysphagia or enteric feeding. OBJECTIVE: This study aimed to assess the clinical outcomes of using a liquid formulation of PVe for both established ORN and as a prophylaxis to avoid its occurrence after dental extractions. A secondary objective was to determine patient-reported side effects in relation to the liquid formulat

Antibiotic prophylaxis before dental procedures may reduce the incidence of osteonecrosis of the jaw in patients with multiple myeloma treated with bisphosphonates.

Osteonecrosis of the jaw (ONJ) can be a severe complication of patients with multiple myeloma (MM) treated with bisphosphonates. Dental procedures are a major risk factor for ONJ occurrence. We retrospectively analysed the data of 178 patients with MM to evaluate if antibiotic prophylaxis before dental procedures may prevent ONJ. A correlation between dental procedures, antibiotic prophylaxis, incidence of ONJ and relevant clinical features was performed. Overall nine out of 178 patients developed ONJ (5 year crude cumulative incidence: 7.7%). Only one case of ONJ was not correlated with denta

Post-NICE 2008: Antibiotic prophylaxis prior to dental procedures for patients with pulmonary arteriovenous malformations (PAVMs) and hereditary haemorrhagic telangiectasia.

Recently published guidance from NICE highlights that antibiotic prophylaxis is no longer required for patients with structural heart disease at risk of infective endocarditis. The American Heart Association has published similarly less interventive guidance. Individuals with pulmonary arteriovenous malformations and hereditary haemorrhagic telangiectasia are at risk of brain abscess from dental bacteraemias. In this article we explore why these patients do not fall into the groups considered by NICE and provide recommendations to reduce their risks of dental bacteraemias, including optimising

Understanding Clinical Practice Variation in Dental Prophylaxis After Total Shoulder Arthroplasty in ASES Surgeons: A Call for Shoulder-Specific Guidelines.

BACKGROUND: Periprosthetic joint infection (PJI) after total shoulder arthroplasty (TSA) is a severe complication with substantial patient and system burden. Although current total hip and knee arthroplasty guidelines generally discourage routine antibiotic prophylaxis for dental procedures, no shoulder-specific guidelines exist. In the absence of TSA-specific evidence, practice patterns may vary by surgeon experience. We hypothesized that the American Shoulder and Elbow Surgeons (ASES) members demonstrate heterogeneity in postoperative dental antibiotic prophylaxis practices after TSA and tha

Perioperative Antimicrobial Prophylaxis for Invasive Dental Procedures: A Systematic Review and Random-Effects Meta-Analysis of Randomized and Placebo-Controlled Studies.

Prophylactic antimicrobials are frequently administered before invasive dental procedures (including implant placement and extractions) to reduce early infectious complications and procedure-related bacteremia; however, clinical benefit remains debated and must be balanced against ecological disruption and antimicrobial resistance. Evidence from experimental work indicates that even a single prophylactic dose of amoxicillin may transiently perturb the oral microbiome and select resistant strains. A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-guided systematic re
