Failure of a glass ionomer to remineralize apatite-depleted dentin.

Remineralization of demineralized dentin lesions adjacent to glass-ionomer cements (GICs) has been reported in the literature. This study tested the hypothesis that a strontium-based GIC can remineralize completely demineralized dentin by nucleation of new apatite crystallites within an apatite-free dentin matrix. Human dentin specimens were acid-etched, bonded with Fuji IX(GP), and immersed in a calcium-and-phosphate-containing 1.5X simulated body fluid (SBF) for 1-4 months. Polyacrylic acid and polyvinylphosphonic acid biomimetic analogs were added to the SBFs to create 2 additional reminera

In vitro remineralization of severely compromised bonded dentin.

Biomimetic remineralization is potentially useful for the remineralization of incompletely resin-infiltrated collagen matrices created by etch-and-rinse adhesives. In this study, we tested the hypothesis that structurally altered dentin collagen cannot be remineralized to the same hierarchical order and dimension seen in structurally intact dentin collagen. The remineralization medium consisted of a set Portland cement/simulated body fluid system containing polycarboxylic acid and polyvinylphosphonic acid as biomimetic analogs. Remineralization of air-dried, collapsed hybrid layers was apparen

Dentin: microstructure and characterization.

Dentin consists of several identifiable structures: tubules with cell processes and fluid; highly mineralized peritubular dentin; and intertubular dentin consisting mainly of collagen and deposited apatite. The structural organization and microstructural variations reflect formative influences, such as tooth size and shape, and alterations caused by age, insult, and disease. However, details of structure-chemistry-property relationships for this anisotropic biologic composite are limited. Materials scientists are focusing on an array of new spectroscopic, analytical, and imaging techniques tha

Dentine hypersensitivity--guidelines for the management of a common oral health problem.

UNLABELLED: Dentine hypersensitivity (DHS) remains a worldwide under-reported and under-managed problem, despite making some dental treatments more stressful than necessary and having a negative impact on the patient's quality of life. This article is designed to build dental professionals' confidence and remove any confusion regarding the diagnosis, prevention and treatment of sensitive teeth caused by dentine hypersensitivity in those patients known to be at risk. There is a need for simple guidelines, which can be readily applied in general practice. However, it is also obvious that one str

The pulp capping procedure in primary teeth "revisited".

The purpose of this review is to "revisit" an earlier paper (1992) on the subject of direct pulp capping in primary teeth and bring some new considerations for the procedure by the use of dentin bonding adhesives. It has come to be recognized that the customary employment of calcium hydroxide for this therapy has some shortcomings that reduce the prognosis for a favorable outcome. For at least a decade, many investigations have found that postoperative sensitivity, thermal stimuli, pulp inflammation and pathosis can be attributed not to the composition of various dental materials and their ins

Effect and Stability of Poly(Amido Amine)-Induced Biomineralization on Dentinal Tubule Occlusion.

In recent years, scientists have developed various biomaterials to remineralize human teeth to treat dentine hypersensitivity. Poly(amido amine) (PAMAM) dendrimers have become a research focus in this field. It has been demonstrated that PAMAM is able to create precipitates both on the surface of and within the dentinal tubules, however, there is little information about its effect on reducing dentine permeability in vitro. This study aimed to evaluate the in vitro effectiveness and stability of the fourth generation amine-terminated PAMAM on dentinal tubule occlusion, especially on dentine pe

Permeability and morphology of dentin after erosion induced by acidic drinks.

BACKGROUND: The aims of this study were to evaluate 1) the alterations of dentin permeability after single exposure of dentin to several acidic soft drinks with different acid composition; 2) the effectiveness of smear layer on dentin surface to prevent erosion of sound dentin; and 3) the role of brushing procedures. METHODS: Dentin discs from human third molars were prepared. Each disc was treated with 0.5 M neutral EDTA for 5 minutes to remove the smear layer and to calculate the maximum fluid flow rate for each disc (to which an arbitrary value of 100% was assigned) using a pressure appara

The Reparative Function of MMP13 in Tertiary Reactionary Dentinogenesis after Tooth Injury.

MMP13 gene expression increases up to 2000-fold in mineralizing dental pulp cells (DPCs), with research previously demonstrating that global MMP13 deletion resulted in critical alterations in the dentine phenotype, affecting dentine-tubule regularity, the odontoblast palisade, and significantly reducing the dentine volume. Global MMP13-KO and wild-type mice of a range of ages had their molar teeth injured to stimulate reactionary tertiary dentinogenesis. The response was measured qualitatively and quantitatively using histology, immunohistochemistry, micro-CT, and qRT-PCR in order to assess ch

Evaluation and treatment of bacterial microleakage.

Recent data have shown a correlation between pulp response and material biocompatibility when microleakage is prevented. Considering vital dentin, the dentist must keep in mind that it is an extension of the pulp, presenting the first line of response to the consequences of microleakage. Healing of the dental pulp is ensured when a clinical seal is provided to prevent bacterial microleakage. Microleakage is a biological phenomenon with several important clinical implications. The affective component clinically presents as dentin postoperative hypersensitivity due to the episodic hydrodynamic f

In vitro evaluation of dentin permeability of fluorotic primary teeth with a new electronic hydraulic conductance measurement system with photosensors.

OBJECTIVES: The permeability characteristics of dentin have been used in many in vitro studies to evaluate longitudinally the efficacy of various restorative and preventative procedures. The easiest way to evaluate dentin permeability is to calculate its hydraulic conductance (Lp) by fluid filtration method. There are researches on electronic hydraulic conductance measurement systems which can give more precise and reliable results of permeability of dentine than the classical system. To the authors' knowledge, there are no studies on bonding properties of restoratives to fluorotic primary tee

Dentine Remineralisation Induced by "Bioactive" Materials through Mineral Deposition: An In Vitro Study.

This study aimed to assess the ability of modern resin-based "bioactive" materials (RBMs) to induce dentine remineralisation via mineral deposition and compare the results to those obtained with calcium silicate cements (CSMs). The following materials were employed for restoration of dentine cavities: CSMs: ProRoot MTA (Dentsply Sirona), MTA Angelus (Angelus), Biodentine (Septodont), and TheraCal LC (Bisco); RBMs: ACTIVA BioACTIVE Base/Liner (Pulpdent), ACTIVA Presto (Pulpdent), and Predicta Bioactive Bulk (Parkell). The evaluation of the mineral deposition was performed through scanning elect

Biomechanical perspectives on dentine cracks and fractures: Implications in their clinical management.

OBJECTIVES: The present review discussed the biomechanical properties of cracks and fractures in crown and root dentine and attempted to explain why cracked teeth and vertical root fractures are so frequent despite the existence of multiple crack toughening mechanisms in dentine. The implications of this knowledge were used to justify how these defects are managed clinically. DATA, SOURCES AND STUDY SELECTION: Literature search was conducted on PubMed, Web of Science, and Scopus for a narrative review on fracture mechanics of crown and root dentine as well as the clinical management of cracke

Biomimetic remineralization of resin-bonded acid-etched dentin.

Degradation of denuded collagen within adhesive resin-infiltrated dentin is a pertinent problem in dentin bonding. A biomimetic remineralization scheme that incorporates non-classic crystallization pathways of fluidic amorphous nanoprecursors and mesoscopic transformation has been successful in remineralizing resin-free, acid-etched dentin, with evidence of intrafibrillar and interfibrillar remineralization. This study tested the hypothesis that biomimetic remineralization provides a means for remineralizing incompletely infiltrated resin-dentin interfaces created by etch-and-rinse adhesives.

Motif-designed peptide nanofibers as bioactive surface modifiers for biomimetic hydroxyapatite formation on demineralized porous dentin.

Dentin hypersensitivity (DH) is closely associated with mineral loss and subsequent exposure of dentinal tubules, which underscores the need for biomimetic strategies capable of restoring dentin as an organic-inorganic composite. In this work, a short peptide (KLVFFAKMLPHHGA) was designed by integrating a self-assembly motif with a mineralization-inducing segment, aiming to promote dentin remineralization. The peptide readily formed stable nanofibrous assemblies in ethanol containing 0.1% (v/v) trifluoroacetic acid (TFA; TFA : ethanol = 1 : 9) at 37 °C. These peptide nanofibers (PNFs) could be

Reducing the risk of sensitivity and pulpal complications after the placement of crowns and fixed partial dentures.

Sensitivity after cementation of a crown with glass-ionomer cement is often attributed to an adverse effect on the pulp by the luting agent. Most permanent restorative materials in common use today do not tend to irritate the pulp; the main cause of pulpal damage is infection, the bacteria originating in the smear layer or deep in the dental tubules, inaccessible to caries-excavating procedures. A poorly fitting provisional crown may expose cut dentin to the oral fluids, and mechanical trauma caused by frictional heat during preparation may also damage the pulp. The following precautions are r

Innate immune responses of the dental pulp to caries.

Various cells and inflammatory mediators are involved in the initial pulpal responses to caries. This review focuses on the cellular, neuronal, and vascular components of pulpal innate responses to caries. Discussion will include dentinal fluid, odontoblasts, neuropeptides, and neurogenic inflammation, which are not classic immune components but actively participate in the inflammatory response as the caries progress pulpally. Summaries of innate immune cells as well as their cytokines and chemokines in healthy and reversible pulpitis tissues are presented.

Sensory transduction in human teeth with inflamed pulps.

The effects of pulpal inflammation on the sensitivity of dentin to cold (5°C) and negative hydrostatic pressure (-300 mm Hg) stimuli in man were compared, since recent evidence suggests that these stimuli excite different classes of sensory receptors. Dentin was exposed in premolars in 14 participants aged 15-25 years. Stimuli were applied to etched dentin immediately after cavity preparation and after the cavity had been filled with gutta percha for 7 days. This treatment increased significantly the intensity of pain produced by cold, and at the same time decreased that evoked by negative pre

Impedance-Guided Localisation of Pulp Chambers and Canal Orifices in Calcified Root Canal Systems: Biological Foundations and Conceptual Application.

AIM: To outline the biological and electrochemical principles supporting impedance-guided localisation of pulp chambers and canal orifices in calcified root canal systems, and to present an accessory concept relating to impedance patterns at the coronal dentine-gutta-percha interface, potentially serving as a surrogate marker of fluid leakage associated with deficient non-conductive root fillings. SUMMARY: Calcific metamorphosis produces structural alterations that complicate localisation of pulp chambers and canal orifices. Adjunctive modalities such as CBCT, guided endodontics, microscopic
