Patient- and Clinician-Related Factors Associated With the Reduction in Opioid Use Among Adults With Chronic Non-Cancer Pain: A Systematic Review.

Despite known risks, opioids are widely used for chronic non-cancer pain (CNCP). Clinical guidelines now recommend deprescribing, but factors predicting success are poorly understood. This systematic review aimed to identify clinician and patient factors associated with attaining successful opioid dose reduction or discontinuation in adults with CNCP. We searched major electronic databases on 18 March 2025 for observational studies evaluating factors associated with opioid reduction in deprescribing interventions, excluding studies on cancer/palliative care or primary substance use disorder. Supportive Interventions for Deprescribing in Adult Mental Healthcare: A Narrative Review.

BACKGROUND: Mental health disorders are prevalent and often treated with psychotropic drugs; however, concerns remain regarding their efficacy and long-term benefits. Likewise, there has been increasing attention on limiting psychotropic drug use to the minimum necessary level. Several studies have focused on deprescribing, often supported by various non-pharmacological interventions (NPIs). This study aimed to explore recent literature on the use of NPIs to support psychotropic drug deprescribing in adults. METHODS: We conducted a narrative review and performed literature search in major ele

Proton Pump Inhibitors (PPIs)-An Evidence-Based Review of Indications, Efficacy, Harms, and Deprescribing.

Proton pump inhibitors (PPIs) are among the most prescribed drugs worldwide owing to their proven efficacy in symptom control and mucosal healing for acid-related disorders including gastroesophageal reflux disease (GORD), peptic ulcer disease, Helicobacter pylori eradication, functional dyspepsia, and gastroprotection in high-risk patients. However, long-term use beyond approved indications is increasingly common and has raised safety concerns. Observational studies link chronic PPI use to a myriad of adverse outcomes such as enteric infections (e.g., Clostridioides difficile), nutrient defic

Deprescribing strategies for proton pump inhibitors.

Proton pump inhibitors (PPIs) are widely used for a number of gastric-acid related diseases. Although these agents demonstrate enhanced efficacy and tolerability compared with other acid-suppressive therapies, long-term use is associated with a number of adverse reactions. This article summarizes appropriate indications for short- and long-term use of PPIs and describes deprescribing strategies that clinicians can use to reduce unnecessary PPI use in their patients.

Implementability of opioid deprescribing interventions at transitions of care: A scoping review.

Continuation of opioids at transitions of care increases the risk of long-term opioid use and related harm. To our knowledge, no study has examined the implementability of opioid deprescribing interventions at transitions of care. Our scoping review aimed to identify the type of opioid deprescribing interventions employed at transitions of care and assess the implementability of tested interventions. Nine electronic databases were searched on 15 May 2023 for English-language studies of adults transitioning between care settings, where opioid deprescribing interventions targeting patients, clin

Pharmacist interventions to deprescribe opioids and benzodiazepines in older adults: A rapid review.

BACKGROUND: Many older adults are prescribed opioids and benzodiazepines (BZDs), despite increased susceptibility to adverse events. Challenges of deprescribing include fragmented care and lack of knowledge or time. Pharmacists are well-positioned to overcome these challenges and facilitate deprescribing of these medications. OBJECTIVES: We sought to evaluate interventions utilizing pharmacists to deprescribe opioids and BZDs in older adults. METHODS: We conducted a rapid review following a comprehensive literature search to identify interventions with pharmacist involvement for deprescribin

A Prescription for Deprescribing Antipsychotics: Managing Polypharmacy in Schizophrenia.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry.

Discontinuation Categories Underlying Gaps in Dispensing for Six Medication Groups.

PURPOSE: Accurately identifying medication discontinuations at scale is important for developing evidence about deprescribing. Gaps in dispensing often serve as proxies for discontinuation but are imprecise. We categorize reasons for gaps in dispensing to inform data-based methods to accurately identify medication discontinuations. METHODS: Using pharmacy dispensing data, we purposively sampled from a population of adults age 65+ with 2+ chronic conditions who experienced a 90-day gap in dispensing-with and without subsequent fills-of oral diabetes drugs, statins, proton pump inhibitors, drug

Deprescribing Self-Administered Epinephrine: Why, Who, and When?

Epinephrine is established as the first-line treatment for anaphylaxis. Self-administered epinephrine (self-epi) is widely prescribed to patients considered at higher risk of future reactions. However, ongoing provision of self-epi is not without consequences: there is a financial cost, and evidence suggests that self-epi has a negative impact on quality of life by reinforcing a patient's perception of vulnerability. The decision to prescribe or continue self-epi therefore requires balancing clinical evidence with patient preferences through shared decision making. Importantly, self-epi should

Development and Pilot Testing of an Algorithm-Based Approach to Anticholinergic Deprescribing in Older Patients.

BACKGROUND: Adverse anticholinergic drug reactions are common, yet evidence on how to reduce exposure to anticholinergic activity and reliably measure successful deprescribing is still scant. This study proposes an algorithm-based approach to evaluate and reduce anticholinergic load, and reports the results of its pilot testing. METHODS: Based on published evidence and expert opinion, a list of 85 anticholinergic drugs and 21 algorithms for reducing anticholinergic load, e.g., by recommending alternative drugs with lower risk, were developed. An accompanying test battery was assembled by focu

"Stabilise-reduce, stabilise-reduce": A survey of the common practices of deprescribing services and recommendations for future services.

BACKGROUND: Public Health England recently called for the establishment of services to help people to safely stop prescribed drugs associated with dependence and withdrawal, including benzodiazepines, z-drugs, antidepressants, gabapentinoids and opioids. NICE identified a lack of knowledge about the best model for such service delivery. Therefore, we performed a global survey of existing deprescribing services to identify common practices and inform service development. METHODS: We identified existing deprescribing services and interviewed key personnel in these services using an interview co

Patient Perceptions of Opioids and Benzodiazepines and Attitudes Toward Deprescribing.

BACKGROUND: Opioids and benzodiazepines (BZDs) pose a public health problem. Older adults are especially susceptible to adverse events from opioids and BZDs owing to an increased usage of opioids and BZDs, multiple comorbidities, and polypharmacy. Deprescribing is a possible, yet challenging, solution to reducing opioid and BZD use. OBJECTIVE: We aimed to explore older adult patients' knowledge of opioids and BZDs, perceived facilitators and barriers to deprescribing opioids and BZDs, and attitudes toward alternative treatments for opioids and BZDs. METHODS: We conducted 11 semi-structured i

A pharmacist integrated into a general practice in Australia: an evolving model of care in medicines optimization.

The general practice pharmacist (GPP) role in Australia is evolving. A pilot GPP model of care developed to optimize medicines for patients at risk of medicine-related harm was evaluated. The aims of this study were 2-fold: to evaluate the GPP model of care on medicines optimization, with a focus on deprescribing, in a population at risk of harm due to their medicines, or clinical condition, and to explore the perspectives of study participants. This single practice study involved two phases. Phase 1 (September 2019-May 2020): at risk patients were referred to the GPP for medication reconcilia
