Impact of HIV on childhood respiratory illness: differences between developing and developed countries.

The main differences of the impact of HIV on childhood respiratory illness between developed and developing countries, and particularly some countries in Africa, are the scale of the problem and the lack of resources to address problems of prevention, diagnosis, and management. Recent data from HIV-infected African children are reviewed and show that the pattern of respiratory disease in these children is not markedly different to the pattern that was reported from the USA and Europe prior to the use of antiretroviral therapy and routine Pneumocystis jiroveci pneumonia (PCP) prophylaxis for HI

Developed countries should not use inactivated polio vaccine for the prevention of poliomyelitis.

Taking into account the global status of polio, it seems evident that the continuing use of oral poliovaccine in all countries is the most obvious and prudent public health policy for the foreseeable future. Possible exceptions might include those countries which are not troubled by the added cost of the inactivated vaccine; whose health services are able to guarantee high levels of vaccine coverage; and which can expect to experience comparatively few importations of wild poliovirus. An important question is whether it is warranted at this time to recommend a combined schedule of inactivated

Population, development, and the environment: trends and key issues in the developed countries.

The quantitative relationship between environmental degradation (pollution) and the factors that influence it can be expressed by the identity: Pollution = population x (good/population) x (pollution/good), where "affluence" is expressed as good/population and the technology of production as pollution/good. Annual data for the emission of carbon monoxide and nitrogen oxides from mobile sources and for the use of pesticides and inorganic nitrogen fertilizer from agriculture, for the member countries of the Organization for Economic Cooperation and Development and the United States, were analyze

Diabetes in developing countries.

There has been a rapid escalation of type 2 diabetes (T2D) in developing countries, with varied prevalence according to rural vs urban habitat and degree of urbanization. Some ethnic groups (eg, South Asians, other Asians, and Africans), develop diabetes a decade earlier and at a lower body mass index than Whites, have prominent abdominal obesity, and accelerated the conversion from prediabetes to diabetes. The burden of complications, both macro- and microvascular, is substantial, but also varies according to populations. The syndemics of diabetes with HIV or tuberculosis are prevalent in man

Sarcoidosis in developing countries.

Sarcoidosis is seen in different parts of India and other developing countries with almost similar frequency as in the West. It was largely due to lack of awareness and non-availability of investigations for diagnosis that the disease was reported to be rare in the past. A combination of clinical, radiologic, and histologic criteria are used to diagnose sarcoidosis. A confident exclusion of other causes of granuloma formation, especially tuberculosis, is required. Absence of mycobacteria and of caseation in the histologic specimens and presence of skin anergy to tuberculin help make a diagnosi

Cancer in developing countries.

The developing countries are represented by incidence and mortality datasets from 16 populations. Trends are studied using age specific data from six cancer registries (three in Asia, three in Latin America) and national mortality datasets from Central and South America (three), the Caribbean (two), Asia (two) and Mauritius. In Africa, three cancer registries (in Nigeria, Uganda and Zimbabwe) provide time series of 15 years or more. Systematic examination of time trends is confined to five major sites (stomach, lung, breast, cervix uteri and colon-rectum), with a comment on observed trends in

[Hemoglobinopathies in developing countries].

The most important haemoglobinopathies in developing countries are reviewed in the light of new results elicited with modern research approaches. It has been shown that the sickle mutation originating in a localized region in West Africa arose independently of the mutation in East Africa and Asia. The frequency of alpha-thalassaemia has been underestimated in mediterranean and African countries. The inherited resistance to Plasmodium falciparum in the sickle cell disorders and thalassaemia has been elucidated to a large extent. The heterogeneity of alpha- and beta-thalassaemia has been investi

Cost for tuberculosis care in developed countries: which data for an economic evaluation?

Tuberculosis (TB) seems to be eradicated in developed countries. However, current migration flows and increasing use of immunosuppressive and biologic drugs for rheumatic diseases are increasing the risk of latent TB and TB onset for citizens of developed countries. Because little is known about the economic burden of TB in developed countries, we set out to describe the order and dimension of the costs of TB care in developed countries. A review of the literature indicated that the cost for anti-TB therapy is about $2000 US per patient. Costs of drugs associated with standard therapy for acti

Immunization strategies for the control of serogroup C meningococcal disease in developed countries.

In developed countries, the epidemiology of serogroup C meningococcal disease is characterized by unpredictable outbreaks and a bimodal distribution of cases, with the highest incidence rate among those below 1 year of age and a second peak in teenagers. Serogroup C meningococcal conjugate vaccines elicit a thymus-dependent immunological response that is already present in young infants and is characterized by the production of protective antibodies and the development of memory. Results from immunogenicity and effectiveness studies indicate that waning of immunity occurs over time, and the pr

Nosocomial tuberculosis in the AIDS era: strategies for interrupting transmission in developed countries.

In developed countries where tuberculosis is increasing in association with HIV infection, nosocomial transmission among patients and from patients to health care workers is being increasingly reported. Nosocomial tuberculosis among HIV-infected patients is difficult to prevent by conventional control measures because the clinical presentation of the disease may be atypical and confused with other respiratory infections, because the tuberculin skin test is less reliable, because cough generating procedures may increase the probability of transmission, because HIV-infected patients may progress

ACUTE DIARRHOEAL DISEASE IN LESS DEVELOPED COUNTRIES. I. AN EIDEMIOLOGICAL BASIS FOR CONTROL.

Some of the acute diarrhoeas of man are specific infectious diseases. Another and major proportion have no demonstrated infectious agent. No characteristic clinical pattern regularly distinguishes one specific disease entity from another, nor specific from non-specific infections, nor epidemic from endemic disease. Since clinical and laboratory differentiation is at present possible for only a small fraction of the diarrhoeas, an alternative in community control, especially where facilities are limited, is to regard all diarrhoeal disease as constituting a clinical syndrome, "acute undifferent

ACUTE DIARRHOEAL DISEASE IN LESS DEVELOPED COUNTRIES. 3. METHODS FOR PREVENTION AND CONTROL.

The programme presented in this article for controlling the diarrhoeas and dysenteries of less developed countries is based on epidemiological principles applicable to acute undifferentiated diarrhoeal disease-its specific as well as its non-specific elements. The dominant importance of weanling diarrhoea requires a main emphasis on maternal and child health procedures, with nutrition singled out for attention, along with public health education and medical care of patients: this in addition to the established worth of means for promoting environmental sanitation. The several features of the s

Cryptosporidiosis in developing countries.

Globally, Cryptosporidium infection continues to be a significant health problem where it is recognized as an important cause of diarrhoea in both immunocompromised and immunocompetent people. In developing countries persistent diarrhoea is the leading cause of death in children younger than five years of age, where it accounts for 30 to 50 percent of those deaths. Encouragingly an increasing number of investigations in developing countries employ molecular tools, significantly improving the quality of epidemiological information. This improved Cryptosporidium monitoring, with appropriate mole

Paraquat in developing countries.

The herbicide paraquat is considered safe by industry and the bulk of regulators worldwide. However, determinants of exposure from 30 years ago persist in developing countries. Little is known about systemic absorption from occupational exposures. The relationships between exposure determinants, levels of external exposure, biomarkers of exposure, and outcomes are not clear. High rates of severe acute poisonings have been documented. In addition, topical injuries occur in as many as 50% of exposed workers. Non-worker populations are also at risk, particularly children. Long-term and delayed he
